[Bilateral luxatio erecta of the shoulder joint--a rare injury. Management and therapy in polytrauma patients].

The case of a 36-year-old male patient is reported who fell from 20 m, sustaining injuries to the abdomen and pelvis and fractures of both arms and the left leg as well as erect dislocation of both shoulders (luxatio erecta humeri). The injury on the right was subcoracoid, that on the left subglenoid-a compound dislocation of the humerus head through the axilla. Based on this case and the pertinent literature, the pathophysiology, diagnosis and treatment of this rare injury are discussed. After immediate closed reduction, soft tissue damage, fractures or neurovascular lesions should be operate

Shoulder Arthrodesis With Reamer Irrigation Aspiration Bone Grafting in Patients With Brachial Plexus Injury.

Brachial plexus injuries (BPI) can result in irreversible functional loss of shoulder girdle musculature leading to humeral head subluxation and instability. The resultant mechanical pain can be debilitating and unresponsive to neuropathic pain medications. Surgical intervention, in the form of shoulder arthrodesis (SA), stabilizes the joint and can help to relieve this mechanical pain. Outcomes after SA have been variable in the general population, with high reported rates of nonunion. This is of particular concern in patients with BPI as they often have disuse osteopenia. Here, we describe t

Rare coracoid fractures presenting with superior shoulder suspensory complex injury: A case series.

Although coracoid fractures are rare fractures, their treatment is critical as they form the coracoacromial arch and superior shoulder suspensory complex (SSSC), which are important structures for shoulder biomechanics. In this case series, clinical, radiological, and demographic characteristics of three male patients who received treatment due to coracoid fracture presenting with traumatic injuries were discussed. The fractures were classified according to the Ogawa and Eyres classifications, and fracture fixation was achieved surgically with the cannulated screws. All patients had a right co

Trunk-rotation flexibility in collegiate softball players with or without a history of shoulder or elbow injury.

CONTEXT: Throwing is a whole-body motion that requires the transfer of momentum from the lower extremity to the upper extremity via the trunk. No research to date examines the association between a history of shoulder or elbow injury and trunk flexibility in overhead athletes. OBJECTIVE: To determine if injury history and trunk-rotation flexibility are associated and to compare trunk-rotation flexibility measured using 3 clinical tests: half-kneeling rotation test with the bar in the back, half-kneeling rotation test with the bar in the front, and seated rotation test in softball position pla

Outcomes associated with a structured prenatal counseling program for shoulder dystocia with brachial plexus injury.

OBJECTIVE: We examined outcomes that were associated with a novel program to identify patients who are at high risk for shoulder dystocia with brachial plexus injury. STUDY DESIGN: The program included a checklist of key risk factors and a multifactorial algorithm to estimate risk of shoulder dystocia with brachial plexus injury. We examined rates of cesarean delivery and shoulder dystocia in 8767 deliveries by clinicians who were enrolled in the program and in 11,958 patients of clinicians with no access to the program. RESULTS: Key risk factors were identified in 1071 of 8767 mothers (12.2

Case report and rehabilitative management of a complete musculocutaneous nerve injury caused by shoulder dislocation.

Anterior shoulder dislocation can secondarily cause nerve injury and/or rotator cuff tear. An elderly male with a recent shoulder dislocation and multiple medical comorbidities was transferred to a rehabilitation inpatient service in order to maximize his functional status. Physical exam suggested a brachial plexus injury. Electrodiagnostic testing confirmed a complete musculocutaneous and a partial axillary nerve lesion. This specific combination of nerve lesions is a previously unreported complication of anterior shoulder dislocation. Prognosis and treatment are discussed, particularly the u

Handedness as a predictor of increased risk of knee, elbow, or shoulder injury, fractures and broken bones.

Several recent studies suggest that left-handers have higher susceptibility to accidental injury. Two studies investigated the suggested link between sinistrality and accidental injury by assessing the prevalence of joint problems, bone breaks, and fractures, which may be the sequelae of such mishaps. Study 1 used retrospective responses on medical history checklists completed by 1064 males on active duty with the United States Air Force. It found that left-handers were at significantly higher risk for knee problems (RR = 2.74), elbow and shoulder problems (RR = 3.03), and also swollen and pai
