Socioeconomic aspects of spinal imaging: impact of radiological diagnosis on lumbar spine-related disability.

Low back pain presents a difficult problem for patients and their doctors. The symptom affects the majority of people at some point in their lives and usually has a benign natural history. However, as a society, we consume tremendous resources to diagnose and treat painful spinal disorders. Magnetic resonance imaging (MRI) facilitates the "medicalization" of low back pain due to its exquisitely sensitive depiction of pathoanatomy. Unfortunately, many of these findings are present in normal subjects. Radiologists should recognize the poor correlation between MRI findings and significant, treata

Unawareness of cognitive deficit in Alzheimer disease and related dementias.

The objective of this study was to determine whether unawareness of cognitive deficit is disease-specific. One hundred thirty-two patients were studied, grouped according to diagnosis of definite or probable Alzheimer disease, vascular dementia, geropsychiatric control, or geriatric control. Diagnosis was the independent variable, and unawareness of cognitive deficit was the dependent variable. The Mini-Mental State Examination score was used as a dementia severity covariate. Analysis of covariance was significant (F = 8.0, p < 0.0001). Follow-up mean comparisons showed the Alzheimer disease g

Cattell's 16 PF and PSY inventory: relationship between personality traits and behavioral responses in patients with acute myocardial infarction.

The aim of this study was to investigate the relationships between personality and behavioral responses in patients with acute myocardial infarction. In a first step, a new instrument (PSY Inventory) for assessment of six behavioral characteristics (Sense of Responsibility, Energy and Competitiveness, Obsessive Behavior, Anger and Hostility, Stress-related Disturbances, Time Urgency) was developed by using factor analysis on intercorrelations of responses from 524 subjects of the general population. Internal consistency reliability for each of the PSY subscales was estimated by Cronbach alpha

Mental health of mothers of children with juvenile rheumatoid arthritis: appraisal as a mediator.

Examined direct and mediated relations between condition parameters and maternal mental health for 53 mothers of 2- to 11-year-old children with juvenile rheumatoid arthritis (JRA). Multivariate analyses revealed that when considered simultaneously, indices of both biological severity and functional severity were associated significantly with maternal mental health. Further, mother's appraisals of the impact of the child's illness on the family partially mediated the effects of medication type and child's functional status on mother's mental health. Results provide support for conceptual model

Psychological distress in husbands of eating disorder patients.

Psychological distress scores of 21 eating-disordered (ED) women and their husbands were compared with those of two matched groups of 21 maritally distressed (MD) and 21 maritally nondistressed (ND) control groups. In contrast to previous clinical reports, ED husbands in the present group did not report significantly more psychological distress than did ND husbands, and actually reported less interpersonal sensitivity and hostility than did MD husbands.

[Differential stress management pattern of seropositive and seronegative polyarthritis patients. A brief comment on the publication, "Polyarthritis patients with and without detected rheumatoid factor: a psychological personality comparison"].

46 seropositive female RA patients were compared to 28 seronegative female RA patients using the Stress Coping Questionnaire of Janke et al. (1985). In comparison to seropositive patients, seronegative patients exhibit a stress coping behaviour characterized by more self-pity but less substitutive gratification. They too show fewer attempts at controlling their reactions.

Development of the Perceived Stress Questionnaire: a new tool for psychosomatic research.

A 30-question Perceived Stress Questionnaire (PSQ) was validated, in Italian and English, among 230 subjects. Test-retest reliability was 0.82 for the General (past year or two) PSQ, while monthly Recent (past month) PSQs varied by a mean factor of 1.9 over 6 months; coefficient alpha > 0.9. General and/or Recent PSQ scores were associated with trait anxiety (r = 0.75), Cohen's Perceived Stress Scale (r = 0.73), depression (r = 0.56), self-rated stress (r = 0.56), and stressful life events (p < 0.05). The General PSQ was higher in in-patients than in out-patients (p < 0.05); both forms were co

General hospital psychiatry: implications for occupational therapy--one case study.

The hospitalization policy in general hospital psychiatric wards is for crisis intervention. The high cost in such a ward in Israel (in comparison with the cost in psychiatric hospitals) is justified since the patient is treated both physically and mentally. Some cases include patients who suffer from chronic diseases and aggravation of their physical condition causes severe mental reactions. In these cases, the short-term psychiatric admission is often in conflict with the condition of the patient and the desired treatment programme. The occupational therapist whose professional values are fo

Lifetime diagnoses in patients with somatoform disorders: which came first?

Thirty inpatients with somatoform disorders were examined with the structured clinical interview SCID for psychiatric lifetime diagnosis. In the present diagnoses, we found a concordance of 63% for somatoform and affective disorders and the lifetime comorbidity of both disorders was 87%. Additionally, patients with somatoform disorders frequently had a history of other psychiatric disorders (for example, anxiety disorders, 40%). For 73% of patients with somatoform disorders and a history of affective disorders, the onset of the somatoform disorder was prior to the onset of another psychiatric

[Deficits in school performance in children and adolescents with chronic asthma].

At the time of their admission to the Buchenhöhe Jugendorf asthma centre (an institution for medical, professional and social rehabilitation), an investigation was made into the cases of 186 children and youths (average age: 14.0), the majority of whom suffered from severe asthma. The investigation, focused on the patients' ailment symptomatology and its effect on school progress, showed that one third of the children had spent considerable time hospitalized in intensive care (an average of 4.4 x). Amongst other findings were: That one third on the children and youths had not taken part in spo

Preparing patients for cancer chemotherapy: effect of coping preparation and relaxation interventions.

Sixty cancer chemotherapy patients were randomly assigned to one of four treatments: (a) relaxation training with guided relaxation imagery (RT), (b) general coping preparation package (PREP), (c) both RT and PREP, or (d) routine clinic treatment only. All patients were assessed on self-report, nurse observation, family observation, and physiological measures and were followed for five sequential chemotherapy treatments. Results indicate that the PREP intervention increased patients' knowledge of the disease and its treatment, reduced anticipatory side effects, reduced negative affect, and imp

Eight cases of patients with unfounded fear of AIDS.

This article describes eight cases of patients treated at the Psychiatric Department of the University Central Hospital of Turku (UCHT) Finland who all had as a common feature an unfounded fear of AIDS. Three of the patients committed suicide and one of them had overt suicidal tendencies. An unfounded fear of AIDS may be a sign of psychiatric disturbance with increased suicidal risk. Increased fear of AIDS seems to have correlation with media and counselling services. Owing to the fact that these patients primarily seek medical help from other fields of medicine than psychiatry, they are a new

Assessing psychosocial adjustment of children with chronic illnesses: a review of the technical properties of PARS III.

Four groups of investigators in the Research Consortium on Chronic Illness in Childhood have used the Personal Adjustment and Role Skills Scale (PARS) III to assess the psychosocial adjustment of children with chronic physical illnesses and no mental impairment. The PARS III consists of 28 items that measure psychosocial functioning in six areas: peer relations, dependency, hostility, productivity, anxiety-depression, and withdrawal. Analyses of the measure's reliability and validity, using a total combined sample of 450 school-age children (ages 5-18 years) with a variety of chronic illnesses

Hypochondriacal fears and beliefs in agoraphobia.

In order to evaluate hypochondriacal fears and beliefs in agoraphobia, the authors administered the self-rated Illness Attitude Scales to 18 agoraphobic patients. The patients reported hypochondriacal concerns similar to those of patients with hypochondriasis. After agoraphobia had been treated with exposure therapy in ten patients, hypochondriacal concerns did not differ significantly from those of normals. The findings suggest that hypochondriacal concerns are substantial in agoraphobia and that these wane when anxiety decreases.

[Coping with illness in rheumatoid arthritis].

Rheumatoid arthritis, a lifelong chronic disease, creates serious psychosocial as well as medical problems. We investigated coping with rheumatoid arthritis in 50 patient. The examination of coping-strategies used by R. A.-patients seems to show no fundamental difference to the coping behavior of patients with other chronic diseases. Biomedical conditions of the disease (Rheumafactor sero-positiv, sero-negativ) are related to different coping-strategies. Patient without psychiatric problems have a better medical health condition and, compared to depressive R. A.-patient prefer more active and

[The disorder of space analysis as a key to understanding anosognosia in right-sided cerebral infarct].

The dextrocerebral stroke is often underestimated as a result of its deficits--in contrary to the left cerebral stroke with the aphasia--because the patient's neglect of the plegic side is transferred to the therapist. A key to understanding this is the hypothesis that the anosognosia results in a loss of spatial analysis. After a survey of the different functions of the left and right hemispheres the author demonstrates the symptoms, diagnosis and therapeutic prognoses. The therapy needs to be continued for several months. In addition, the patient's social environment is important.

Personality in torticollis: a controlled study.

The Eysenck Personality Questionnaire, the trait scales of the Leyton Obsessional Inventory, the Spielberger State Trait Anxiety Inventory, and a questionnaire assessing marital status and harmony were completed by 100 torticollis patients and a control group of 49 cervical spondylosis sufferers. Information regarding employment status, and events preceding onset of their complaint was also obtained. The two groups did not differ in terms of any of the personality dimensions evaluated, or in their self-reports of events prior to onset of their illness. The groups differed significantly with re

The clinical use of mindfulness meditation for the self-regulation of chronic pain.

Ninety chronic pain patients were trained in mindfulness meditation in a 10-week Stress Reduction and Relaxation Program. Statistically significant reductions were observed in measures of present-moment pain, negative body image, inhibition of activity by pain, symptoms, mood disturbance, and psychological symptomatology, including anxiety and depression. Pain-related drug utilization decreased and activity levels and feelings of self-esteem increased. Improvement appeared to be independent of gender, source of referral, and type of pain. A comparison group of pain patients did not show signif
