Efficacy and safety of stereotactic radiosurgery for petroclival meningiomas: A systematic review and Meta-Analysis.

BACKGROUND: Managing petroclival meningiomas (PCMs) is challenging due to their deep-seated location and proximity to critical anatomical structures. Resection aimed at achieving gross total resection (GTR) has been the primary therapeutic approach for PCMs; however, GTR is associated with considerable complications. Stereotactic radiosurgery (SRS) has emerged as an effective and safe minimally invasive option for managing petroclival meningiomas. This study evaluated the role of primary or adjuvant SRS in the treatment of petroclival meningiomas. METHODS: Following a comprehensive literature

Microsurgical resection of a large petroclival meningioma through an extended retrosigmoid approach: how I do it.

BACKGROUND: Petroclival meningiomas are challenging tumors. Several skull base approaches have been proposed in the last decades, with variable rates of postoperative morbidity and extent of resection. METHODS: We herein reported the step-by-step microsurgical resection of a large petroclival meningioma through an extended retrosigmoid approach. Detailed surgical technique has been accompanied by a 2D operative video. CONCLUSION: The extended retrosigmoid approach allowed for a safe gross total resection of the tumor, as confirmed by the postoperative MRI. The patient did not experience any

Bone defect closure after resection of sphenoorbital meningioma.

UNLABELLED: Sphenoorbital meningiomas (SOM) are a subgroup of skull base tumors with soft tissue component in the orbit and anterior and/or middle cranial fossa. According to different authors, SOMs account for 2-12% of all intracranial meningiomas. Reconstruction of bone defects after resection of SOM has own nuances. Along with cranial vault repair, patients encounter with cosmetic defects following facial skull lesion, ophthalmic symptoms due to orbital defects, dental and functional problems associated with opening of the mouth in case of damage to maxilla and mandible. Predominant infiltr

Surgical Limitations of the Microscopic Transciliary Supraorbital Keyhole Approach to the Anterior and Middle Skull Base.

BACKGROUND: The microscopic transciliary SupraOrbital keyhole (mtSO) approach has been used for a wide variety of anterior and middle fossa pathologies, including aneurysms, meningiomas, craniopharyngiomas, and other skull-base tumors. Several clinical series have proven its efficacy and safety, but detailed anatomical demarcations of the anterior and middle cranial base exposure are lacking. Our aim was to define the surgical limitations of the mtSO approach to the ipsilateral and contralateral anterior and middle skull base. METHODS: Five cadaveric specimens (10 sides) were studied with ima

A case series of percutaneous transforamen ovale biopsies in cavernous sinus lesions.

BACKGROUND: The majority of cavernous sinus lesions are meningiomas, for which treatment (fractioned radiotherapy or radiosurgery), if indicated, is usually initiated upon image-based diagnosis. However, this region can be affected by a wide variety of pathological processes and the risk of misdiagnosis exists. As pathological diagnosis can be obtained by biopsy through the foramen ovale in selected cases, we asked the question as to whether systematically performing this procedure before treatment would provide additional, relevant diagnostic information. METHODS: All the cases referred to o

Comprehensive proteomic analysis reveals distinct functional modules associated with skull base and supratentorial meningiomas and perturbations in collagen pathway components.

Meningiomas are brain tumors that originate from the meninges and has been primarily classified into three grades by the current WHO guidelines. Although widely prevalent and can be managed by surgery there are instances when the tumors are located in difficult regions. This results in considerable challenges for complete surgical resection and further clinical management. While the genetic signature of the skull base tumors is now known to be different from the non-skull base tumors, there is a lack of information at the functional aspects of these tumors at the proteomic level. Thus, the cur

[Metastatic skull base tumors (biology, clinical presentation and evaluation)].

Metastatic skull base tumors are rare and complicated lesions. They involve cranial nerves and craniobasal vascular structures causing multiform clinical presentation. Despite modern neurovisualization techniques, risk of preoperative diagnostic pitfall remains high. Final diagnosis and planning of further treatment are possible only after surgical removal of tumor with subsequent immunohistochemical verification.

[Chondroma of the skull base and maxilla].

INTRODUCTION: Chondromas are uncommon benign tumors of cartilaginous origin. Although chonodroma of the jaw is extremely rare, the commonly involved are the anterior portion of the maxilla, condyle and coronoid process. A chondroma is a painless, slow growing tumor causing destruction and exfoliation of teeth. On a radiograph, the tumor appears as a cyst-like radiolucent lesion, while some are sclerotic. The borders are usually ill-defined. Irregular calcifications may be seen with radiolucencies and then it is an osteochondroma. MATERIAL AND METHODS: We present a patient with a resected and

News in the 5th edition of the WHO Classification of Tumours of the Head and Neck: Tumours of the Sinonasal Tract and Skull Base Tumors.

The World Health Organization (WHO) recently published the 5th edition of head and neck tumors. This edition describes both existing entities and a group of emerging entities, along with updates regarding taxonomy and detailed characteristics of tumors and tumor-like lesions. Sinonasal tumors and skull base tumors represent a heterogeneous group of tumors with significant histological variability and overlap in imaging methods. An important change in the 5th edition of the WHO classification is the relocation of recurrent soft tissue, hematolymphoid, and neuroectodermal tumors into a separate

Combined endonasal, endo-orbital and transcranial craniofacial resection of invasive anterior skullbase sinonasal carcinoma: 2D endoscopic video.

BACKGROUND: Sinonasal malignancies, in which squamous cell carcinomas are the most common are rare and difficult to treat given the location and anatomical structures involved. Sinonasal malignancies often present late due to non-specific and benign symptoms. Partial resections are often associated with poorer outcomes [1]. Combined approaches can improve extent of resection of tumour involving sinonasal tumours with significant intracranial extension and allows skull base reconstructions from above with pericranium and below with a nasal septal flap [2]. Complete resection has been shown to i

Total Petrosectomy for the Total Resection of Sphenopetroclival Meningioma: 2-Dimensional Operative Video.

Sphenopetroclival meningioma is a most formidable meningioma. Many patients have few preoperative deficits and surgery has the potential of severe neurological complications. Surgical treatment is challenging due to brainstem compression, the involvement of multiple cranial nerves and cerebral vessels. Wide tumor exposure, multiple dissection axis, and short distance are paramount factors in the quest of achieving total removal of Simpson grade I, including the involved dura and bone. The posterior petrosal, transtentorial presigmoid approach offers a wide and shallow operative field.1-7 When

How I do it: the mononostril endonasal transethmoidal-paraseptal approach.

BACKGROUND: The use of endoscopes in transnasal surgery offers increased visualization. To minimize rhinological morbidity without restriction in manipulation, we introduced the mononostril transethmoidal-paraseptal approach. METHODS: The aim of the transethmoidal-paraseptal approach is to create sufficient space within the nasal cavity, without removal of nasal turbinates and septum. Therefore, as a first step, a partial ethmoidectomy is performed. The middle and superior turbinates are then lateralized into the ethmoidal space, allowing a wide sphenoidotomy with exposure of the central skul

Modification of Gill-Thomas-Cosman frame for extracranial head-and-neck stereotactic radiotherapy.

PURPOSE: A modification of a commercially available, noninvasive, relocatable, stereotactic Gill-Thomas-Cosman (GTC) head frame is presented for treatment of extracranial lesions of the head and neck, base of the skull, and inferior nasopharyngeal region. METHODS AND MATERIALS: Skull-based and nasopharyngeal lesions cannot be treated with the GTC frame because it obstructs the beam path. To treat those lesions, the GTC frame was modified without compromising the integrity, flexibility, or use of the treatment software. The modification uses a set of aluminum extension rods of variable lengths
