[Psychogenic non epileptic seizures: a review].

OBJECTIVE: This paper summarizes the recent literature on the phenomena of psychogenic non epileptic seizures (PNES). DEFINITION AND EPIDEMIOLOGY: PNES are, as altered movement, sensation or experience, similar to epilepsy, but caused by a psychological process. Although in the ICD-10, PNES belong to the group of dissociative disorders, they are classified as somatoform disorders in the DSM-IV. That represents a challenging diagnosis: the mean latency between manifestations and diagnosis remains as long as 7 years. It has been estimated that between 10 and 30% of patients referred to epilepsy …

Psychogenic movement disorders.

All varieties of movement disorders may be mimicked by a psychogenic disorder, most commonly tremor, dystonia, and myoclonus. Approximately 3% of patients seen in specialty clinics have a psychogenic movement disorder (PMD). The diagnosis of a PMD depends on not just ruling out an organic movement disorder, but moreover, recognizing features from the history and examination that are inconsistent or incongruous with an organic movement disorder. Most PMDs represent a conversion disorder, sometimes as part of a somatoform disorder; less common diagnoses include a factitious disorder or malingeri

Physical symptoms and physician-experienced difficulty in the physician-patient relationship.

BACKGROUND: The practice of medicine has many expected and accepted challenges, but all physicians experience some patients as difficult to a degree that transcends these expectations. Physician-experienced difficulty is associated with a syndrome of three characteristics: patient psychopathology, abrasive interpersonal styles, and multiple physical symptoms. OBJECTIVE: To assess the roles played by the number of physical symptoms and by specific symptoms in determining whether physician-experienced difficulty occurs. DESIGN: New analyses of epidemiologic survey data from the Primary Care Ev

Psychological treatment of chronic pain: a selective review.

Chronic pain occurs commonly in patients who have either physical or mental disorders, or both. Almost any psychiatric diagnosis may be involved, but affective and somatoform disorders predominate, together with adjustment disorders and psychological factors affecting physical condition. Psychological treatments for chronic pain have developed rapidly within the last 15 years. Most treatment is provided in the form of multi-faceted 'packages' incorporating a range of methods which are derived from operant or cognitive-behavioural theories. A selective review of these approaches leads to the co…

Foregut symptoms, somatoform tendencies, and the selection of patients for antireflux surgery.

A large variety of foregut symptoms can occur in patients with gastroesophageal reflux disease (GERD), which can overlap with other disorders such as somatoform disorders and dyspepsia. Due to unclear diagnostic situations, these patients are often not adequately treated. The aim of this study was the evaluation of patients with foregut symptoms, referred for possible antireflux surgery, regarding their relationship with GERD and somatization tendencies based on control data from an unselected population. Symptom evaluation and somatization screening were initiated both in volunteers and in pa…

[Functional somatic syndrome in dental practice].

Functional somatic syndromes (FSSs) are common in dental as well as medical practice. Many patients with unexplained symptoms in oro-maxillo-facial areas visit dentists, but they are not diagnosed and treated properly. Temporomandibular disorder, atypical facial pain, and glossodynia (burning mouth syndrome) are included in dental FSSs. These diseases overlap with each other and with FSSs in other organs, such as myofacial pain syndrome, tension-type headache, fibromyalgia, and chronic fatigue syndrome. They coexist with mental disorders, such as anxiety disorder, mood disorder, and somatoform

DSM-IV hypochondriasis in primary care.

The object of this study was to assess the prevalence and correlates of the DSM-IV diagnosis of hypochondriasis in a primary care setting. A large sample (N = 1456) of primary care users was given a structured interview to make diagnoses of mood, anxiety, and somatoform disorders and estimate levels of disability. The prevalence of hypochondriasis (DSM-IV) was about 3%. Patients with this disorder had higher levels of medically unexplained symptoms (abridged somatization) and were more impaired in their physical functioning than patients without the disorder. Of the various psychopathologies e…

Psychogenic movement disorders.

Psychogenic movement disorders are common, but the diagnosis may be difficult. Visual appearance alone is typically not sufficient to make a diagnosis, but such information is certainly important. That a movement is bizarre can be helpful, but still must be considered thoughtfully since organic movement disorders can have endless variety. The diagnosis should rest on positive findings such as paroxysmal nature, maximum severity at or near onset, variability of tremor direction, frequency and amplitude, entrainment of tremor, distractability and suggestibility, and wildly swaying gait and balan

Use of Generic Large Language Model Chatbots for Mental Health Issues Among Patients Seeking Psychotherapy: Cross-Sectional Mixed Methods Analysis in a German University Hospital Outpatient Clinic.

BACKGROUND: Large language model-based chatbots (LLM-CBs) are increasingly used as mental health support tools. Risks and harms are discusse … he included diagnosis groups and was higher for anxiety and eating disorder diagnoses and lower for somatoform disorders. Post hoc multivariable analyses suggest that higher use in eating disorders and lower use in somatoform disorders were attenuated after adjusting for age, while higher use for anxiety disorders persisted. LLM-CBs were generally evaluated positively …

[Münchhausen syndrome: diagnosis and management].

The authors review the literature on Münchhausen syndrome, and ilustrate the clinical features of the disorder with the description of a characteristic case. Diagnosis and differential diagnosis are discussed with regard to other somatoform disorders such as conversion disorder and somatization disorder as well as to malingering and schizophrenia. The awareness of general practitioners and surgeons regarding this syndrome may avoid the exposure of these patients to serious complications of unnecessary medical and surgical procedures. The management of Müchhausen syndrome is aggravated by the l…

[Predictors of treatment duration for inpatients with mental disorders--a systematic literature review].

OBJECTIVES: In a systematic literature review we examined patient-related predictors of inpatient treatment duration for mental disorders. METHODS: The databases Medline, Embase, Psyndex, PsycINFO and EBMR were systematically reviewed for studies in the years 1990-2009 regarding treatment duration of inpatients with ICD-10 diagnoses F3-F6. Also, their methodological quality was evaluated. RESULTS: Twenty-one studies examining 28 predictors (sociodemographic, clinical) were included. Longer treatment durations were found for patients living alone and with higher levels of initial symptom seve

[Prevention and therapy of post-traumatic disorders from a biopsychosocial viewpoint].

Traumatic events such as accidents, rape or industrial disasters usually have biological, psychological and social consequences. Depending on the context and the type of trauma, psychiatric disorders may develop; acute stress reaction and post-traumatic stress disorder as well as unspecific disorders like depression, anxiety and somatoform disorder. In this paper, the traumatic event is understood as a potential crisis trigger. A definition of the term crisis is introduced in which the concept of identity plays a central role. Five interdependent areas of identity are presented: bodily experie

Functional facial and tongue movement disorders.

Functional movement disorders (FMDs) affecting the eyelids, tongue, and other facial muscles are often underrecognized because their phenomenology has not been fully characterized. Nevertheless, these disorders are more common than previously thought. In this chapter we will discuss the phenomenology as well as the clinical and instrumental diagnosis of facial FMDs. Facial FMDs should be considered when a patient exhibits any combination of the following features: (1) fixed unilateral facial contractions, especially with lower lip, with or without ipsilateral jaw involvement, of maximal severi

[Hysteria in children: Briquet's syndrome (a case report)].

In this case report, the authors studied the difficulties to set up the diagnosis of hysteria in the Briquet's Syndrome. The description presented here is about an eleven years old girl patient without any particular family background and who, after her tonsils have been out against her will, presented a hysteria in the form of Briquet's Syndrome. All the explorations showed no abnormalities. It is the questioning, and the analysis of facts from informations by the patient and her family which allowed to conclude the diagnosis of hysteria. She received a chemotherapy, associated with a psychot

Histrionic personality disorder in women with somatization disorder.

The clinical distinctions between histrionic personality disorder and somatization disorder have frequently been blurred. In this study, 60 women with somatization disorder were found to have histrionic personality disorder. A DSM-III diagnosis of histrionic personality disorder did not significantly help to improve the diagnosis of somatization disorder. A diagnosis of histrionic personality disorder should stimulate a search for better-validated Axis I diagnoses.

Classification of anxiety.

Current psychiatric classifications of anxiety are examined critically. In the European literature a specific cluster of symptoms is diagnostic of anxiety neurosis and their subsidiary classification is based on precipitating factors and duration of symptoms. In the American literature, exemplified by DSM-III, greater emphasis is placed on symptomatic classification and panic is given separate diagnostic status, both alone and in conjunction with agoraphobia. In both classifications generalized anxiety is at the bottom of a diagnostic hierarchy so that all other symptoms take precedence. It is
