Odontogenic Sinusitis: Clinical, Radiologic, and Quality-of-Life Characterization in a Prospective Study.

Background: Rhinosinusitis is an increasingly prevalent condition that compromises patients' quality of life and imposes a substantial burden on healthcare systems. Odontogenic sinusitis, a predominantly unilateral form of rhinosinusitis, accounts for up to 40% of unilateral maxillary sinusitis cases. This study aimed to characterize the clinical presentation, radiological extension, and quality-of-life impairment associated with odontogenic sinusitis. Methods: Thirty-eight patients with unilateral sinusitis caused by dental pathology/treatment were recruited. OS diagnosis criteria were establ

[Ectopic meningioma of the ethmoid sinus: case report and a review of the literature].

A rare case of ectopic meningioma of the ethmoid sinus is reported. A 57 year-old male patient was admitted with complaints of anosmia and headache. Computed tomography revealed a relatively high-density mass in the bilateral ethmoid and sphenoid sinuses. The mass was markedly enhanced after intravenous administration of contrast medium. The cerebral angiography showed tumor stains fed by bilateral internal and external carotid arteries with right side dominance. The tumor in the ethmoidal sinus was removed by otolaryngologists at first and then the tumor in the sphenoid sinus was removed usin

Nontraumatic Internal Carotid Aneurysms in the Paranasal Sinuses Presenting with Epistaxis: A Case Report.

Epistaxis due to rupture of a nontraumatic internal carotid artery (ICA) aneurysm in the paranasal sinus has rarely been reported. Here, we report a case of double ICA aneurysms located within both the sphenoid and ethmoid sinuses. A 78-year-old woman presented with recurrent massive epistaxis. Magnetic resonance angiogram (MRA) and cerebral angiogram showed two ICA aneurysms: one protruded into the sphenoid sinus and the other protruded into the ethmoid sinus. Intra-aneurysmal coil embolization was performed for both aneurysms. The patient recovered completely, and a follow-up MRA 3 years lat

Extramedullary hematopoiesis: a rare occurrence in the sinonasal tract.

Extramedullary hematopoiesis (EMH) is a systemic reaction to inadequate hematopoiesis. We report two exceedingly rare cases of EMH involving the paranasal sinuses. The first patient, a 30-year-old man, presented with a maxillary sinus mass. The lesion was excised by endoscopic surgery: definitive histology identified foci of EMH within an inflammatory fibromyxoid pseudotumor. The second case occurred in a 29-year-old man affected by intermediate beta-thalassemia. He was hospitalized with a diagnosis of sphenoid sinus mucocele secondary to an ethmoid lesion. The patient underwent endoscopic exc

[The facial approach to the median structures of the skull base].

Numerous routes of access to the medial basal structures of the cranium have been described, largely because of the wide variety of lesions observed in deep localizations. Access can be achieve via trans-sinusal, transfacial (trans-sphenoidal rhinoseptal, mediofacial or Lefort I), trans-oro-pharyngeal and numerous other routes. An examination of the principals involved, their development and the technical modalities demonstrate the advantages and disadvantages of each and their specific indications. Access is particularly interesting with the frontal trans-sphenoid, Lefort I osteotomy and tran

Prolonged visual disturbance secondary to isolated sphenoid sinus disease.

OBJECTIVES/HYPOTHESIS: The objective was to evaluate the efficacy of endoscopic sinus surgery in patients with isolated sphenoid sinus disease with visual disturbances of more than 72 hours' duration. STUDY DESIGN: Retrospective review. METHODS: A sequential series of patients with isolated sphenoid sinus disease with visual disturbances who underwent endoscopic surgery performed by the same surgeon between 1995 and 2002 were reviewed. Patients with malignant tumors were excluded from the study. Patients were evaluated preoperatively and postoperatively to record the effects of this approach

Allergic Fungal Sinusitis.

A 42 year old male presents with worsening pain and an increase in thick chronic drainage of the left sinus. Image studies show complete opacification of the left frontal sinus, left sphenoid sinus, and the left maxillary sinus. The patient was taken to the operating room and tissue for microscopic evaluation was obtained. The microscopic findings were classic for allergic fungal sinusitis: areas of alternating mucinous material and inflammatory cell debris and abundant Charcot-Leyden crystals. Cultures were performed and the patient began steroid therapy and desensitization therapy.

Sphenoid sinus osteitis after endoscopic transsphenoidal surgery: a bone-centered postoperative entity.

PURPOSE: Despite advances in endoscopic skull base surgery, postoperative sinonasal inflammation remains poorly characterized. In particular … low-up, all of the patients were symptom-free; one patient required a second revision for recurrent sphenoid sinusitis at sixth months follow up. CONCLUSION: Sphenoid sinus osteitis represents a rare but clinically relevant bone-centered complication following ETS, distinct from conventional rhinonasal morbidity. Its exclusive association with sellar reconstruction …

Optic Neuropathy due to an Ethmoid Mucocele: A Case Report and Literature Review.

Mucoceles of the paranasal sinus commonly involve the frontal sinuses, the ethmoid sinuses, and rarely the maxillary or sphenoid sinuses. They often present with sinus pain or pressure, but rarely can present with more severe symptoms such as changes in mental status or vision due to expansion and invasion through the skull base or orbit. A 62-year-old male presented with optic neuropathy, a relative afferent pupillary defect with proptosis and lateral gaze palsy of the left eye. The patient was found to have a large mucocele extending from the left posterior ethmoid sinus into the left orbita

Non-epithelial tumors of the nasal cavity, paranasal sinuses and nasopharynx: a clinicopathologyic study. VIII. Adipose tissue tumors (lipoma and liposarcoma).

In a study of 256 non-epithelial neoplasms involving the nasal cavity, paranasal sinuses and nasopharynx, only two lesions were classified as adipose tissue tumors (one lipoma and one liposarcoma). The lipoma was found incidentally when tissue removed from the maxillary sinus was studied histologically, the patient having been treated with a Caldwell-Luc procedure for maxillary sinusitis. The liposarcoma produced a large mass involving the nasal cavity, maxillary sinus, ethmoid area, hard palate and maxillary, temporal and sphenoid bones. The tumor extended into the cranial cavity and eventual

[The endoscopy of paranasal sinuses. Diagnostic and therapeutic possibilities (author's transl)].

This paper deals with further experiences in endoscopy of the maxillary sinus and in possibilities of endoscopy of frontal and sphenoidal sinuses. Some coloured photographs taken during endoscopy are shown. As the result of 301 sinuscopies of maxillary sinus performed, it could be shown that in 36% of the cases x-ray findings didn't exactly correspond with the endoscopy findings. In further 22% there was no correlation at all between the x-rays and findings at operation. This shows the great importance of sinuscopy and reveals its diagnostic values. The author used the endoscopy of the maxilla

Extramedullary hematopoiesis of the sphenoid sinus associated with hereditary spherocytosis: A case report.

Extramedullary hematopoiesis is a rare cause of a sinonasal mass that presents unique diagnostic and treatment challenges. While there are numerous reports of patients with hereditary spherocytosis developing extramedullary hematopoiesis in the posterior mediastinum, involvement in the paranasal sinuses has not previously been described. Here, we present the first case of a patient with hereditary spherocytosis developing extramedullary hematopoiesis in the sphenoid sinus.

[Mediofacial degloving in extramedullary plasmacytoma with orbital invasion].

Extramedullary solitary plasmacytoma is a rare plasmatic cell tumor occurring in the head and neck. We report the case of a patient with a mass that arose from the sphenoidal base, extended toward the right paranasal sinuses, and invaded the orbit. The clinical characteristics, diagnostic criteria, and therapeutic problems are discussed and reviewed in the literature, with special attention to an alternative therapeutic approach, midfacial degloving.

Primary atypical carcinoid tumour of the sphenoid sinus rostrum.

Primary carcinoid tumors of the nasal cavity and sinuses are exceedingly rare. An accurate histopathological diagnosis is crucial to optimal investigation and management. We present a case of a primary atypical carcinoid tumor arising from the sphenoid rostrum without evidence of associated carcinoid syndrome. This rare but important differential diagnosis of a nasal tumor is discussed and important unique management issues are highlighted.

Paraganglioma of the sphenoid sinus appearing as labile hypertension.

Paragangliomas, also known as glomus tumors, are unusual, generally benign tumors most commonly found in association with major vessels and cranial nerves. Only a small number of these tumors have been described arising in the paranasal sinuses, and none of these has been physiologically active. The following case report describes a metabolically active paraganglioma arising in the sphenoid sinus.

Organized Hematoma of the Sphenoid Sinus With Acute Blindness: Insight Into Pathogenesis of Disease.

Sinonasal organized hematomas (OHs) are rare lesions that primarily localize to the maxillary sinus. The rate of growth of these masses has not been described in the literature. We present a case of a 59-year-old gentleman with polyostotic fibrous dysplasia who presented with acute loss of vision in the left eye from an expanding OH of the sphenoid sinusitis. After expanded endonasal, transpterygoid approach and debulking, patient experienced significant vision improvement. Close follow-up imaging preoperatively allowed radiologic documentation of the rate of OH growth and this is presented in…

Bilateral cavernous sinus thrombosis due to mucormycosis.

A 49-year-old man requiring hemodialysis for end-stage renal disease developed orbital cellulitis and bilateral central retinal artery occlusions. Although cranial computed tomography demonstrated ethmoid and sphenoid sinusitis, two surgical biopsy specimens of sinus mucosa were nondiagnostic. Autopsy findings confirmed cerebral mucormycosis with bilateral cavernous sinus and carotid artery thrombosis. The patient had been taking deferoxamine for hemochromatosis, and this chelating agent may have been the predisposing factor in developing mucormycosis. We discuss the limitations of current neu…

The Comparison of Piriform Aperture, Paranasal Sinuses, and Cranial Dimensions.

OBJECTIVE: Piriform aperture, paranasal sinuses, and the cranium dimensions were compared with each other and we investigated the alterations depending on the age and gender in these structures. Before the endoscopic sinus surgery, anatomic variations in sinusoidal region and the occurring differences should be considered. The detection of these variations plays important roles in the prevention of complications which may happen in surgery or in obtaining a successful surgical result. MATERIALS AND METHODS: Piriform aperture maximum width, frontal sinuses, sphenoidal sinuses, maxillary sinuse

[Subdural empyema and cerebritis as a sequela of dentogenous suppurative pansinusitis].

BACKGROUND: Complications and local extension of dentogenous and paranasal sinus infections most often involve the periorbit and frontal cranium. Because of the widespread use of antibiotics, intracranial extension of maxillofacial sinusitis is rarely seen today. Nevertheless, the clinician must be aware of the potential for these complications, because late recognition of this condition and delay in treatment can increase morbidity and mortality rates. CASE REPORT: A case report of sinogenic intracranial complications is presented, with sinogenic empyema, cerebritis, and subdural empyema. Th
