Analysis of apex and transitional vertebra of the spine according to pelvic incidence using orientation and position parameters.

OBJECTIVE: To identify the different apex and transitional vertebra according to the shape of the pelvis of individuals despite their difference in sagittal alignment using our measurement system. METHODS: Full-spine X-rays using EOS in standard stand-position of 99 volunteers were selected (47 women, 52 men, mean age 31 years old). Validated 3D reconstruction technique allows extraction of spinopelvic parameters, and position and rotation of each vertebra and lumbar disks. Subjects were divided into three groups: low PI (lowPI, n = 37), moderate PI (midPI, n = 52), high PI (highPI, n = 10),

Complications of Thoracolumbar Adult Spinal Deformity Surgery.

Adult spinal deformity (ASD) is a challenging problem for spine surgeons given the high risk of complications, both medical and surgical. Surgeons should have a high index of suspicion for medical complications, including cardiac, pulmonary, thromboembolic, genitourinary and gastrointestinal, renal, cognitive and psychiatric, and skin conditions, in the perioperative period and have a low threshold for involving specialists. Surgical complications, including neurologic injuries, vascular injuries, proximal junctional kyphosis, durotomy, and pseudarthrosis and rod fracture, can be devastating f

Orthopaedic manifestations of neurofibromatosis type 1.

Neurofibromatosis type 1 (NF-1) is an autosomal dominant disease that affects 1 in 3,000 persons worldwide. Café-au-lait macules and peripheral nerve sheath tumors (ie, neurofibromas) are the most commonly recognized manifestations of NF-1. However, NF-1 affects multiple organ systems, and a multidisciplinary approach to treatment is required. Management of the orthopaedic manifestations of NF-1 is often difficult. The most complex manifestations are scoliosis (dystrophic and nondystrophic), congenital pseudarthrosis of the tibia, and problems related to soft-tissue tumors. Metabolic bone dise

Orthopaedic manifestations of neurofibromatosis in children: an update.

Neurofibromatosis is one of the most common genetic disorders affecting mankind. Despite extensive basic science research, the diagnosis still is based largely on well-defined clinical criteria, which often present gradually during childhood. Approximately 50% of patients have significant musculoskeletal manifestations, with scoliosis and congenital pseudarthrosis of the tibia most common. Approximately 20% of children with Type I neurofibromatosis present with scoliosis with or without the classic dystrophic features, such as vertebral scalloping and rib penciling. Dystrophic curves portend r

Dens Axis Vertebroplasty Combined with C 3 Vertebral Body Arthroplasty. Case Study.

Spinal tumors are most commonly located in the thoracic and lumbar spine, less commonly in the cervical section of the spine. Diagnosis is usually late and surgery is not radical. Myeloma is one of the most frequent metastatic spinal tumors. Surgical treatment of osteolytic lesions in the spine involves posterior and anterior stabilization, full or partial tumorectomies as well as decompression of neural structures. Bone cement injection has been increasingly used in plastic surgery of vertebral bodies and is most frequently performed percutaneously in the thoracic and lumbar spine. Cervical v

Spine system changes in soldiers after load carriage training in a plateau environment: a prediction model research.

BACKGROUND: Low back pain is the most common spinal disorder among soldiers, and load carriage training (LCT) is considered the main cause. We aimed to investigate changes in the spine system of soldiers after LCT at high altitudes and the change trend of the lumbar spine and surrounding soft tissues under different load conditions. METHODS: Magnetic resonance imaging scans of the lumbar spines of nine soldiers from plateau troops were collected and processed. We used ImageJ and Surgimap software to analyze changes in the lumbar paraspinal muscles, intervertebral discs (IVDs), intervertebral

Typical persistent organic pollutant exposure induces body axial curvature through binding to Kdm1a transcriptionally activating expression of urp2 in zebrafish.

Body axial curvature is a common toxic effect of fish after exposure to environmental pollutants, but the molecular mechanism remains unclear. To investigate the underlying mechanisms, zebrafish were exposed to different persistent organic pollutants (POPs) including polychlorinated biphenyl (PCB) and 2,2',4,4'-tetrabromodiphenyl ether (BDE-47). PCB40 was chosen as the typical POP to discover the mechanism because of its remarkable body axial developmental toxicity. The PCB40 exposure reduced the density of spinal cord cilia. We observed the upregulation of neuropeptides urotensin II-related p

Radiographic paraspinal muscle size in adult spinal deformity: a systematic review and meta-analysis.

PURPOSE: Reduced trunk muscle quantity or quality, commonly assessed using imaging-based metrics, has been associated with adverse outcomes in spine surgery. However, evidence specific to adult spinal deformity (ASD) surgery remains heterogeneous and methodologically limited. This systematic review aimed to synthesize existing muscle metrics and surgical outcomes in patients undergoing ASD surgery. METHODS: A systematic search of PubMed and EMBASE was performed to identify studies evaluating imaging-based muscle quantity or quality in adult patients (≥ 18 years) undergoing thoracolumbar corre

State of the art review: Vertebral Osteotomies for the management of Spinal Deformity.

Spinal deformity is a complex condition caused by various etiologies (degenerative, neuromuscular, congenital, developmental, traumatic, neoplastic, idiopathic) leading to clinical deformity, axial back pain, and neurologic deficits. Patients presenting with severe deformities require vertebral osteotomies to achieve the necessary curve correction for radiographic and clinical improvement. The three major vertebral osteotomy techniques commonly used at this time are the posterior column osteotomy (PCO), pedicle subtraction osteotomy (PSO), and vertebral column resection (VCR). The different ve

Characteristics and risk factors for proximal junctional kyphosis in adult spinal deformity after correction surgery: a systematic review and meta-analysis.

There are still controversies on characteristics and risk factors for proximal junctional kyphosis (PJK) in adult spinal deformity (ASD) patients. The objective of this study is to explore the characteristics and risk factors for PJK in ASD. A systematic online search in databases including PubMed, EMBASE, Web of Science, and the Cochrane Library was performed to identify eligible studies. OR and weight mean difference with 95% CI were used to evaluate characteristics and risk factors. A total of 31 studies were finally included. ASD patients with PJK had larger proximal junctional angle (PJA)

Postural changes in orthodontic patients treated with clear aligners: A rasterstereographic study.

BACKGROUND: Correlation between malocclusions and body posture has been discussed in the last decades, but there is still a lack of consensus in existing literature. Rasterstereography allows tridimensional reconstruction of the spine, starting from the back surface analysis. So far studies which tested modifications of rasterstereographic parameters during orthodontic treatment, comparing with those obtained from untreated control group, are not available. Clear aligner treatment produces alteration of vertical height due to the occlusal coverage and, subsequently, a stimulation of periodonta

Cervical sagittal balance: a biomechanical perspective can help clinical practice.

PURPOSE: In this article, we summarize our work on understanding the influence of cervical sagittal malalignment on the mechanics of the cervical spine. METHODS: Biomechanical studies were performed using an ex vivo laboratory model to study the kinematic and kinetic response of human cervical spine specimens in the setting of cervical sagittal imbalance. The model allowed controlled variations of C2-C7 Sagittal Vertical Alignment (C2-C7 SVA) and T1-Slope so that clinically relevant sagittally malaligned profiles could be prescribed, while maintaining horizontal gaze, and their biomechanical

Spinal deformity in aged zebrafish is accompanied by degenerative changes to their vertebrae that resemble osteoarthritis.

Age-related degenerative changes within the vertebral column are a significant cause of morbidity with considerable socio-economic impact worldwide. An improved understanding of these changes through the development of experimental models may lead to improvements in existing clinical treatment options. The zebrafish is a well-established model for the study of skeletogenesis with significant potential in gerontological research. With advancing age, zebrafish frequently develop gross deformities of their vertebral column, previously ascribed to reduced trunk muscle tone. In this study, we asses

Vertebral Body Tethering for Thoracolumbar Curvatures in Adolescent Idiopathic Scoliosis: Radiographic and Clinical Outcomes at 2-6-Year Follow-Up.

Background: The gold standard treatment for adolescent idiopathic scoliosis (AIS) is posterior spinal fusion (PSF). However, long-term consequences of PSF can include reduced spinal flexibility, back pain, and intervertebral disc degeneration. Vertebral body tethering (VBT) is a non-fusion alternative that preserves motion. We investigated the outcomes of VBT for the treatment of thoracolumbar (TL) major AIS in the largest single-surgeon series with a minimum 2-year follow-up (FU). Methods: We performed a retrospective single-center review. Inclusion criteria were AIS, Lenke 5/6 curvature, and

Dura Vibration Difference Between PTP Bone Resection and Upper Facet Joints En Bolc Resection in Posterior Thoracic Decompression.

OBJECTIVE: In thoracic posterior decompression surgery, the traditional pedicle-to-pedicle (PTP) approach may have limitations in achieving complete decompression and may also pose potential risks of injury to the spinal cord. Through comparative analysis with the PTP method, the study explored the safety of posterior thoracic decompression via the pedicle-ossification tunnel (POT), aiming to provide a more scientific and safer clinical surgical pathway selection. METHODS: Combined with preoperative image data and intraoperative operation images, the POT decompression method was deeply analyz
