Neural Plasticity Following Surgical Correction of Strabismus in Monkeys.

PURPOSE: Although widely practiced, surgical treatment of strabismus has varying levels of success and permanence. In this study we investigated adaptive responses within the brain and the extraocular muscles (EOM) that occur following surgery and therefore determine long-term success of the treatment. METHODS: Single cell responses were collected from cells in the oculomotor and abducens nuclei before and after two monkeys (M1, M2) with exotropia (divergent strabismus) underwent a strabismus correction surgery that involved weakening of the lateral rectus (LR) and strengthening of the medial…

Changes in Lateral Comitance After Asymmetric Horizontal Strabismus Surgery.

IMPORTANCE: Asymmetric horizontal strabismus surgery is often performed to correct primary gaze alignment without considering the symptoms that may result from misalignment in the patient's side gaze. Surgical choices influence alignment in side gaze and may contribute to functional and social deficits. OBJECTIVE: To identify the surgical procedures associated with changes of alignment in side gaze to help inform surgical planning for patients with horizontal strabismus. DESIGN, SETTING, AND PARTICIPANTS: The medical records of 1081 horizontal strabismus surgical procedures that were perform…

Necrotizing scleritis after strabismus surgery in thyroid eye disease.

Surgically induced necrotizing scleritis, which can lead to potentially devastating consequences, is a rare sequela to many types of ocular surgeries. We describe a 58-year-old woman who developed necrotizing scleritis following strabismus surgery for thyroid ophthalmopathy. Tectonic corneal patch grafting resulted in a favorable outcome.

Inferior oblique muscle palsy with "paradoxical" v-pattern strabismus.

BACKGROUND AND PURPOSE: A- and V-patterns are commonly encountered with cyclovertical muscle palsies. Inferior oblique (IO) muscle palsy produces and A-pattern caused mainly by the decreasing abducting action of the IO in upgaze and an increasing abduction action of the superior oblique in downgaze. V-pattern association with an IO palsy has not been, to our knowledge, reported before. The purpose of our paper is to report on a patient iwth IO palsy and a paradoxical V-pattern and explain the proposed pathophysiology behind the V-pattern. CASE REPORT: We report a 67 year old female with a 3 y

Anesthetic management for pediatric strabismus surgery: Continuing professional development.

PURPOSE: Strabismus surgery is one of the most common pediatric ophthalmic procedures. The purpose of this continuing professional development module is to update physicians on the anesthetic considerations of pediatric patients undergoing strabismus surgery. PRINCIPAL FINDINGS: The preoperative assessment is important, as patients undergoing strabismus surgery may have an associated neuromuscular disorder, congenital syndrome, or cardiac disease. Malignant hyperthermia is no longer considered as being an issue associated with strabismus. The laryngeal mask airway is used frequently and has b…

Surgical management of traumatic strabismus after combat-related injury.

Traumatic strabismus may result from orbital or head injury (closed or open). Patients may have other complex injuries that delay diagnosis and treatment. Twelve consecutive adult patients with persistent (> 3 month) strabismus after combat-related head, orbital, or eye trauma underwent strabismus surgery on one or both eyes. The main outcome measures were ocular alignment and diplopia resolution at least 6 months postoperatively. Seven of 12 patients (58%) had traumatic vision loss (< 20/200) in one eye in addition to the traumatic strabismus. Strabismus procedures were complicated intraopera…

Binocular vertical rectus muscle recession for comitant vertical strabismus.

BACKGROUND: Binocular vertical rectus muscle recession has not been formally evaluated in the correction of comitant vertical strabismus. METHODS: Eight patients with stable comitant vertical strabismus for at least 6 months were included. All underwent recession of the superior rectus muscle of the hypertropic eye combined with an equal or nearly equal recession of the inferior rectus muscle in the hypotropic eye. On the day before surgery, on one of the first three postoperative days, and at one year postoperatively, ocular alignment in vertical and horizontal gaze directions were measured …
