Antimicrobial resistance among invasive isolates of Streptococcus pneumoniae collected across Canada.

Between 2002 and 2003, 736 nonduplicate Streptococcus pneumoniae isolated from blood cultures were collected from 7 of 10 Canadian provinces (10 tertiary care centers). Microdilution broth susceptibility testing was performed using the method prescribed by the Clinical Laboratory Standards Institute. Of the isolates, 16.85% were nonsusceptible to penicillin and 5.4% were highly resistant. Of the S.pneumoniae, 14.1% had reduced susceptibility to erythromycin and 47% had been accounted for by the M phenotype. No isolates were recovered that were resistant to telithromycin. Only 6 isolates were r…

Streptococcus pneumoniae: the role of apoptosis in host defense and pathogenesis.

Programmed cell death or apoptosis is a recognised feature of infection with Streptococcus pneumoniae, and is observed during pneumococcal meningitis and pneumonia. The cholesterol-dependent cytolysin, pneumolysin, is a major trigger of apoptosis in the brain in association with pneumococcal production of hydrogen peroxide. Pneumococcal cell wall is also an important stimulus for apoptosis. Microbial factors and host factors combine in causing apoptosis in the brain, with hippocampal neurons being particularly susceptible. In pulmonary infection epithelial cell apoptosis contributes to tissue …

[Neonatal meningitis caused by atypical Streptococcus pneumoniae: case report and review].

Streptococcus pneumoniae is a rarely recognized cause of neonatal sepsis and/or meningitis, but it is associated with substantial morbidity and mortality. Traditionally, S. pneumoniae is identified in the laboratory by demonstrating susceptibility to optochin. However, the emergence of optochin-resistant organisms makes definite identification difficult when only phenotypic tests are taken as markers. We present the case of a severe early-onset neonatal meningitis due to an atypical strain of S. pneumoniae. Laboratory methods utilized to certify this species diagnosis are discussed.

PCR fingerprinting analysis for differentiation of Streptococcus pneumoniae reinfection versus relapse.

Polymerase chain reaction fingerprint profiles of isolates obtained during an episode of pneumococcal pneumonia with bacteremia differed significantly from profiles of isolates obtained from the same patient during a subsequent episode of pneumococcal meningitis with bacteremia. Polymerase chain reaction fingerprinting provides a means of differentiating new infection from relapse, and may be a simple molecular tool for comparison of Streptococcus pneumoniae isolates.

Activity of trovafloxacin against blood isolates of Streptococcus pneumoniae in Sweden.

The in vitro activity of the fluoroquinolone trovafloxacin (CP-99,219) against 257 blood isolates of Streptococcus pneumoniae obtained from Swedish hospitals was compared with those of commonly prescribed oral antibiotics and also with those of ciprofloxacin and ofloxacin against a collection of strains resistant (n = 6) or intermediately resistant (n = 22) to penicillin (Pc-R). The MICs of trovafloxacin for Pc-R strains of pneumococci ranged from 0.032 to 0.25 mg/l. No difference was seen between the clinical isolates and the Pc-R strains (MIC50 = 0.064 mg/l and MIC90 = 0.125 mg/l for both co…

Nosocomial epiglottitis associated with penicillin- and cephalosporin-resistant Streptococcus pneumoniae bacteremia.

We cared for a 4-year-old male with nosocomially acquired epiglottitis caused by Streptococcus pneumoniae. He had been receiving ceftazidime therapy when this infection was recognized. The S. pneumoniae isolate was of serotype 15B and was resistant to beta-lactam antibiotics, cephalosporins (including those with extended spectra), and trimethoprim-sulfamethoxazole. Clinicians and clinical microbiologists must be aware that cephalosporin susceptibility may no longer be assumed for penicillin-resistant S. pneumoniae isolates and that susceptibility testing for the extended-spectrum cephalosporin…

Rapid identification of Staphylococcus aureus and Streptococcus pneumoniae from blood cultures.

Simultaneous application of the lysostaphin sensitivity test for identification of Staphylococcus aureus and the deoxycholate test for the identification of Streptococcus pneumoniae was evaluated for reliability in rapid identification (1 h) of these organisms from blood cultures by using BACTEC 6B and 7C bottles. The procedure was applied to 127 cultures, 74 lysostaphin tests and 53 deoxycholate tests. Lysostaphin-tested organisms included 23 S. aureus, 40 Staphylococcus epidermidis, 1 Listeria sp., 1 Peptococcus sp., 2 Micrococcus sp., 2 diphtheroids, and 5 mixed cultures. Deoxycholate-teste…

Severe infection due to Streptococcus pneumoniae in asplenic renal transplant patients.

The medical records of 293 patients who underwent renal transplantation were analyzed for the occurrence of Streptococcus pneumoniae and Haemophilus influenzae infections in relation to splenectomy. Splenectomy was done in 236 (81%) graft recipients before or concomitant with transplantation. Bacteremia developed in five and fulminant sepsis in two from 3 to 32 months after splenectomy. No serious infections with these organisms occurred in the nonsplenectomy group. These results suggest that asplenia may be an additional factor predisposing transplant patients to serious infection. Prevention…

Changes in blood pH in rats after infection with Streptococcus pneumoniae.

Acid-base alterations in Streptococcus pneumoniae infection were studied in 80 male albino rats. Hematocrit and concentrations of plasma electrolytes, glucose, and total protein were also measured. At 3-h intervals throughout a 27-h study, four control and four infected rats were anesthetized with ether, and blood samples were taken. Arterial blood pH, Po2, and hematocrit increased in the infected group, whereas arterial Pco2, HCO3-, and venous Po2 decreased. Plasma K+ concentration increased slightly and glucose levels decreased in the infected rats as the sepsis progressed. No significant ch…

BinaxNOW Streptococcus pneumoniae Antigen Assay in Pleural Fluid for Detection of S pneumoniae in Children With Empyema.

Recent studies using polymerase chain reaction (PCR) demonstrate that blood and pleural fluid (PF) cultures are inadequate to identify pneumococcus in children with pneumonia and empyema, leading to unnecessary utilization of broad-spectrum, often toxic antibiotics. Identification of pneumococcus facilitates the de-escalation of antibiotics. We compared a point-of-care immunochromatographic test that detects pneumococcal C-polysaccharide antigen (BINAX) performed in PF and urine to cultures (blood and PF) and 2 different PCR tests in PF of 79 children with empyema. BINAX in PF had 100% sensiti

Immunoprophylactic potential against pneumonia caused by indigenous isolates of Klebsiella pneumoniae and Streptococcus pneumoniae in experimental mice.

Pneumonia is a life threatening public health concern particularly due to multidrug resistant (MDR) bacterial pathogens. The rising threat of these resistant bacterial strains has exacerbated the challenge of treating bacterial pneumonia, emphasizing the urgent need of alternative therapeutic strategies such as vaccine development by targeting combined localized MDR bacterial pneumonia pathogens. In this study, clinical samples of bacterial pneumonia pathogens were collected from tertiary care hospitals of Lahore, Punjab, Pakistan. Strains were isolated and characterized on biochemical and mol

Shift from Streptococcus pneumoniae to Klebsiella pneumoniae: a 15-year nationwide study of bacterial meningitis in Korea.

BACKGROUND: Bacterial meningitis is a medical emergency with high mortality and neurologic sequelae. Its epidemiology is changing with vaccination and evolving pathogens. We investigated current etiologic patterns of adult community-acquired bacterial meningitis in Korea and outcome determinants. METHODS: In this 15-year (2009-2023) retrospective study of 17 hospitals, 247 cases were analyzed for causative organisms, and 224 for 30-day outcomes. Risk factors for mortality and neurologic sequelae were determined. RESULTS: Streptococcus pneumoniae declined from 36.4% in 2009-2013 to 19.4% in 2…

Acute severe spinal cord dysfunction in a child with meningitis: Streptococcus pneumoniae and Mycoplasma pneumoniae co-infection.

UNLABELLED: Tetraplegia developed abruptly in an 11-y-old with pneumococcal meningitis. Magnetic resonance imaging showed multiple hyperintensities at the brainstem-spinal cord junction. Serological tests were positive for Mycoplasma pneumoniae (microparticle agglutination and specific IgMs). Erythromycin and dexamethasone were started promptly, and 10 d later the patient was discharged with normal neurological function. CONCLUSION: Tetraplegia during the course of pneumococcal meningitis in an 11-y-old girl disappeared after treatment with ceftriaxone, erythromycin and dexamethasone.

Role of Inflammatory Risk Factors in the Pathogenesis of Streptococcus pneumoniae.

Streptococcus pneumoniae (Spn) is a colonizer of the human nasopharynx (NP), causing a variety of infections in humans including otitis media, pneumonia, sepsis, and meningitis. The NP is an immune permissive site which allows for the persistence of commensal bacteria. Acute or chronic respiratory airway inflammation constitutes a significant risk factor for the manifestation of Spn infections. The inflammatory conditions caused by an upper respiratory viral infection or respiratory conditions such as allergic asthma and chronic obstructive pulmonary disorders (COPDs) are implicated in the dys…

Streptococcus pneumoniae sepsis as the initial presentation of systemic lupus erythematosus.

OBJECTIVE: Infections are among the most important causes of morbidity and mortality in patients with systemic lupus erythematosus (SLE) but are rare initial presentation of the disease. Therefore, in this study, we describe a case of Streptococcus pneumoniae sepsis in a young woman with previously undiagnosed SLE. CASE REPORT: A 23-year-old female patient was admitted to our outpatient clinic complaining of high fever (40°C), chills, fatigue, generalized myalgia, and cough with brown sputum for 5 days. Blood cultures grew gram-positive coccus defined as S. pneumoniae using standard procedure…

[Invasive isolates of Streptococcus pneumoniae in Serbia: antimicrobial susceptibility and serotypes].

INTRODUCTION: Streptococcus pneumoniae is one of the leading causes of bacterial meningitis and sepsis. Invasive pneumococcal disease is a significant medical problem worldwide, particularly in children, due to a huge increase of pneumococcal resistance to antibiotics. OBJECTIVE: The aim of the study was to investigate the antimicrobial susceptibility pattern of invasive pneumococcal isolates, as well as to determine whether decreased S. pneumoniae susceptibility to antibiotics was related to a particular serotype. METHODS: Antimicrobial susceptibility to 19 antibiotics was determined in 58 …
