[Management of neuromuscular block during general anesthesia : Results of a survey in Germany in 2020 compared to the recommendations of the first European guidelines in 2023].

Since the last surveys on the utilization of muscle relaxants in general anesthesia in 2000 and 2005, it can be assumed that the introductio … o improve. The most frequently used drug for rapid sequence induction is now rocuronium rather than succinylcholine. Sugammadex is now available as a drug in 86% of the responding anesthesia departments. The concept of complete relaxation for laparoscopic surgery called deep block is used by 60% of those surveyed. The results are analyzed and discussed in the con …

Delaying initiation of electroconvulsive treatment after administration of the anaesthetic agent and muscle relaxant reduces the necessity of re-stimulation.

OBJECTIVES: To investigate the effect of delaying initiation of electroconvulsive therapy (ECT) after administration of anaesthetic agent and muscle relaxant. METHODS: A retrospective cohort study utilizing a case-based analysis comparing number of re-stimulations, length of seizures, number of ECTs per series and stimulation dosage before and after introducing a new treatment regimen. In 2013, ECT was initiated approximately 60-90 seconds after administration of thiopental and succinylcholine. This interval was increased to 120 seconds in 2014. Ninety-three patients were included (40 in 2013…

Prolonged neuromuscular block associated with cholinesterase deficiency.

RATIONALE: Hereditary genetic mutations may cause congenital cholinesterase deficiency. When succinylcholine and mivacurium are applied on cholinesterase-deficient patients during general anesthesia, prolonged postoperative asphyxia occurs, which is an uncommon but very serious complication. PATIENT CONCERNS: A previously healthy 30-year-old female presented prolonged spontaneous breathing recovery after general anesthesia. DIAGNOSES: After the patient's postoperative spontaneous breathing recovery delayed, the plasma cholinesterase was found to be 27 U/L, which was far below the normal leve…

General Anaesthesia Protocols for Patients Undergoing Electroconvulsive Therapy: Retrospective analysis of 504 sessions over a five-year period at a tertiary care hospital in Oman.

OBJECTIVES: This study aimed to review general anaesthesia protocols for patients undergoing electroconvulsive therapy (ECT) at a tertiary c … iod. All of the patients underwent a uniform general anaesthetic regimen consisting of propofol and succinylcholine; however, they received different doses between sessions, as determined by the treating anaesthesiologist. Variations in drug doses between sessions in the same patient could not be attributed to any particular factor. Self-limiting tachycardia and …

A medical health report on individuals with silent butyrylcholinesterase in the Vysya community of India.

BACKGROUND: Butyrylcholinesterase (BChE; gi:116353) deficiency has adverse effects on the response to succinylcholine and mivacurium. A physiological function of BChE is to inactivate octanoyl ghrelin. We determined the health effect of complete absence of BChE in humans. METHODS: Clinical tests of cardiac, lung, liver, and kidney function, body weight, sperm counts and motility were performed on 5 men, age 20-32 y, in the Vysya community of Coimbatore, India who had silent BChE. Postmortem tissues from 2 cadavers with wild-type BChE were assayed. RESULTS: Test results were normal, except fo…

[Anesthetic-induced heart arrest. A case report of 2 infants with previously unrecognized muscular dystrophy].

We report two boys aged 4 and 10 months who suffered cardiac arrests after induction of anaesthesia. Both infants had no personal or family history of myopathy. In both cases anaesthesia was induced by inhalation with halothane and N2O/O2 (70/30). To facilitate tracheal intubation both were given succinylcholine after the administration of atropine. The 4-month-old developed muscle rigidity and cardiac arrest occurred immediately after tracheal intubation. Resuscitation was unsuccessful. Laboratory findings during resuscitation showed elevated serum potassium levels of more than 10 mmol/l and …

Rocuronium antagonized by sugammadex for series of electroconvulsive therapy (ECT) in a patient with pseudocholinesterase deficiency.

We report the anesthetic management of a patient with catatonic schizophrenia and pseudocholinesterase deficiency, using the nondepolarizing neuromuscular blocking drug, rocuronium, reversed by its specific reversal agent, sugammadex, for a series of electroconvulsive therapy sessions. Rocuronium and sugammadex were used every 48 hours for 8 consecutive times and proved to be an effective and safe combination in a situation where succinylcholine was contraindicated.

[Effect of anesthesia on the condition of newborn infants following cesarean section].

A comparative study of the anesthesia at cesarean section with application of d-tubocurarine in 602 cases and application of only succinylcholine in 69 cases before delivery showed no differences in the Apgars-scores of the newborn. Only severe obstetric complications and long duration of the anesthesia influenced the clinical condition of the infants. There was no correlation between the doses of barbiturates or tubocurarine given to the mother and the Apgars-scores.

Anaesthetic considerations in the Prader-Willi syndrome: report of four cases.

The anaesthetic management of four paediatric patients with the Prader-Willi syndrome is reported. The syndrome is characterized by obesity, mental retardation, genital hypoplasia, hypotonia, and diabetes mellitus. All patients were anaesthetized with halothane. Succinylcholine or pancuronium were used for muscle relaxation, without evidence of abnormal response. Common anaesthetic difficulties in this syndrome are obesity, hypotonia, disturbance in thermoregulation, arrhythmias, diabetes mellitus and convulsions.

[Current diagnosis and therapy of malignant hyperthermia].

Malignant hyperthermia is an autosomal dominant disorder which results in a severe reaction to anesthetic agents in approximately 0.0005 to 0.5% of patients exposed to general anesthesia. Malignant hyperthermia is characterized by severe muscle rigidity, myoglobunuria, high fever, tachycardia, and arrhythmia precipitated by depolarizing muscle relaxants (succinylcholine) and inhalational anesthetic agents (halothane). When MH occurs, the anesthetic agents must be discontinued immediately and dantrolene should be administered.

The comparative effects of aminoglycoside antibiotics and muscle relaxants on electrical field stimulation response in rat bladder smooth muscle.

It has been reported that several aminoglycoside antibiotics have a potential of prolonging the action of non-depolarizing muscle relaxants by drug interactions acting pre-synaptically to inhibit acetylcholine release, but antibiotics itself also have a strong effect on relaxing the smooth muscle. In this study, four antibiotics of aminoglycosides such as gentamicin, streptomycin, kanamycin and neomycin were compared with skeletal muscle relaxants baclofen, tubocurarine, pancuronium and succinylcholine, and a smooth muscle relaxant, papaverine. The muscle strips isolated from the rat bladder w…

Malignant hyperthermia: pharmacology of triggering.

Over the past 50 yr, many drugs have been implicated as triggers of malignant hyperthermia (MH), a potentially fatal pharmacogenetic disorde … ons for the preparation of anaesthetic machines when used for known or suspected MH patients. While succinylcholine enhances the response of potent inhalation anaesthetics, its role as an inherent trigger of the condition is controversial. Non-depolarizing neuromuscular blocking drugs appear to protect against the development of MH and this may be by blocking exc …

Masseter muscle rigidity and malignant hyperthermia susceptibility.

Seventy-seven patients who developed masseter muscle rigidity (MMR) after receiving succinylcholine to facilitate tracheal intubation were evaluated for malignant hyperthermia (MH) susceptibility by in vitro halothane and caffeine contracture tests. Thirty-nine patients were diagnosed as MH-susceptible. Neither age, sex, nor type of surgery or anesthesia distinguished MH-susceptible from nonsusceptible patients. Two susceptible and two nonsusceptible patients had evidence of a myopathy. Fifty-two patients had serum creatine phosphokinase (CPK) levels measured in the perioperative period. Altho…

Mechanical properties of normal and malignant hyperthemia susceptible porcine muscle: effects of halothane and other drugs.

Malignant hyperthemia, which can be initiated in susceptible humans and swine by the volatile anesthetic halothane, appears to result from abnormal responses in skeletal muscle. We have inferred the primary defect in susceptible muscles by observing their responses to certain drugs. Furthermore, we compared the responses of cut muscle cell preparations, such as those used in the diagnostic caffeine test, with those of intact muscle cells. Specifically, we investigated the effects of halothane, caffeine, succinylcholine and catecholamines on the mechanical properties of intact muscle cells from…

Responses of feline esophagus to cervical vagal stimulation.

We used an in vivo feline model to study striated and smooth-muscle esophageal responses to cervical vagal stimulation (VS). Circular esophageal contractions were measured by intraluminal pressure sensors and longitudinal contractions by a force transducer. Responses to VS were recorded before and after giving succinylcholine, atropine, or both. Circular contractions characteristic of striated and smooth muscle were recorded from the mid- and distal esophagus, respectively. With increases in VS frequency, the striated muscle showed rapid, repetitive twitches progressing to tetany. These fast c…

Neostigmine-induced prolonged neuromuscular blockade in a patient with atypical pseudocholinesterase.

A patient with unrecognized atypical pseudocholinesterase was given succinylcholine and then vecuronium before neuromuscular monitoring was instituted. Subsequently, when neostigmine and glycopyrrolate were given to reverse what was thought to be a nondepolarizing block, the patient became further relaxed, and his trachea could not be extubated for more than 10 hours. In this report, we discuss drug interactions, phase II block, and the importance of timely neuromuscular monitoring.
