[Plastic surgery reconstruction of the adult buried penis : Option or obligation?].

The adult buried penis afflicts the patient with shame and is also potentially associated with considerable urogenital complications. Due to obesity, chronic urogenital lymphedema or subsequent inflammatory urogenital conditions, such as a lichen sclerosus et atrophicus, the clinical appearance of the adult buried penis is extremely variable. Epidemiological data are non-existent. Therapeutically, a combination of various plastic surgery procedures is often necessary for the best esthetic and functional results. The therapeutic strategy is highly individual and the therapy itself is interdisci

Is ketorolac safe to use in plastic surgery? A critical review.

BACKGROUND: Ketorolac tromethamine is a nonsteroidal anti-inflammatory drug (NSAID) that provides postoperative pain control and reduces narcotic requirements. However, concerns regarding postoperative hematoma have limited its use in plastic surgery. OBJECTIVES: Our goal is to critically review the risk of bleeding with ketorolac in plastic surgery patients, with a focus on aesthetic surgery. METHODS: A PubMed/Medline literature search of clinical trials using the keywords "surgery" and "NSAID" yielded 2574 results. Of these results, 1036 included ketorolac and twelve involved plastic surge

Mastoid emissary vein: anatomy and clinical relevance in plastic & reconstructive surgery.

The mastoid emissary vein (MEV) is an anatomical structure with limited description in the literature and its importance is even less recognized in the plastic surgical field. Investigations in its anatomy and physiology have described its anthropological significance in transition to bipedalism and preferential intracranial venous flow into the vertebral plexus in the upright man. Inadvertant injury of vessels of this size pose a significant problem due not only to difficulty with haemostasis but also from their bidirectional flow and close proximity to the sigmoid sinus where cases of thromb

Barbed sutures in aesthetic plastic surgery: evolution of thought and process.

The evolution of barbed suture technologies and their application in the field of plastic surgery is now in its third decade. Much has been learned along the way. Initial excitement was often followed by disappointment as we learned more about the limited longevity of minimally invasive procedures and complications arising from various suture designs of the past. Some of the early designs, developed primarily for use in aesthetic facial procedures, included free-floating, bidirectionally barbed, nonabsorbable sutures; unidirectional barbed, nonabsorbable sutures; anchored, bidirectional, nonab

Plastic and reconstructive surgery approaches in the management of anal cancer.

Various reliable reconstructive options are available for treatment of perineal and perianal skin and soft tissue defects resulting from tumor ablation. Indications for TAR include the following: very low rectal cancers, in which low anterior resection or resection with coloanal anastomosis is not possible: persistent or recurrent anal cancer that has failed to respond to chemoradiation therapy; and previous rectal excision with either recurring colostomy complications or an unacceptable quality of life with a stoma. Of course, adequate surgical oncologic principles must not be compromised to

[Plastic surgery in patients with obesity late after horizontal banded gastroplasty].

The results of 361 plastic operations in 296 patients with morbid obesity late after horizontal gastroplasty were analyzed. Plastic and corrective operations aimed at removal of redundant lipocutaneous "aprons" at the anterior abdominal wall, thighs, thoracic wall, gluteal region and the arms, represent a final stage of surgical treatment of patients with morbid obesity. The indications, technique and the results of plastic operations performed from 1985 to 1998, are thoroughly elucidated. The analysis of early postoperative complications has established, that it a reasonable to perform such o

[Indications and concepts for plastic surgery therapy in chronic varicose ulcer].

In the treatment of extensive leg ulcers, surgical therapy is superior to conservative methods in many respects. Radical resection of sclerotic tissue around the ulcer and its causing veins is essential, followed by defect cover with mesh-skin grafts. Surgery leads to prompt healing with a shortened hospitalization and therefore decreases costs; it leads to a prompt relief of pain and a low rate of recurrence. Especially in the aged patient the indication for operation must be well-considered and discussed interdisciplinarily. Even though operative technique seems to be simple, it should be re

[Plastic surgery reconstruction of late sequelae following burns of the hands].

Late consequences of burns of the hands can be divided into four types depending on their extent and the involvement of the underlying tissue structures. Of 88 hands operated on, 43% were Type I, 28% were type II, 25% were type III, and only 4% belonged to the mutilating type IV. Clinical examples of types I and III are shown. Type I requires operation on the skin only to restore full function of the hand, whereas type III also needs repeated surgical treatment of the dorsal apparatus, tendons, ligaments and joints to improve hand movement.

[Opponens plastic surgery using the tendon of the m. extensor indicis].

For restoring the opposition of the thumb in lesions of the median nerve, numerous surgical procedures have been reported. In severe injuries with considerable scarring in the palmar wrist area the motor muscles of the forearm are useless and if the ulnar nerve is equally involved, opposition cannot be restored even with the Huber-Nicolaysen-Littler technique. In such cases we have recently come to carry out opponensplasties using the tendon of the extensor indicis with good success. The method is described and results are demonstrated.

[Facial plastic surgery as preventive measures for psychic lesions (author's transl)].

Facial plastic surgery offers numerous possibilities of removing disfigurements and deformities of the face and thus preventing or curing psychic lesions. The causes of such disturbances are pointed out. In selected examples the relevant aspects in childhood, in adults and in advanced age are emphasized. Up-to-date advances in microsurgical grafting methods and a more aggressive technique in neck-lifting show that successful plastic surgery of the face always has preventive or curative effects on psychic lesions.

Plastic surgery in hydronephrosis (Indications and follow-up. Isotope nephrographic studies).

Surgical interventions for the reconstruction of the renal pelvis were performed in 108 cases between 1962 and 1974. On follow-up, 3 to 6 months after operation the results were found good or satisfactory in 54 cases (50%). Isotope nephrography revealed a good or satisfactory fall of the curve in the excretion phase in 49 out of the 54 cases (94%), distinct retention curves having been found in the remaining 6%. In 35% of these cases isotope nephrography showed a far more complete urinary drainage than it had been assumed on the ground of radiography. In 63% of the cases the results of the two

[Bilaterally strangulated bilateral obturator hernia. Plastic surgery, using the round ligaments].

An unusual case of bilateral obturaptor hernia with double ileal strangulation is reported. An account is given of obturator hernia, its pathogenesis, and surgical management. Of the various possibilities that exist in this respect, the simplest and most resolutive, it is submitted, is that in which the ligamentum teres uteri is employed to close the hernial porta.

Current status of biomaterial's clinical applications in plastic and reconstructive surgery.

The use of biomaterials as replacement parts is not new. The concept has fascinated physicians for thousands of years. Using gold plates to reconstruct defects in the skull over 3000 years ago constituted the early record in the literature of use of biomaterials in implantology. The recent advances in space age technology and in clinical biocompatibility has been a major contributing factor in introducing new materials for use as implantology polymers. However, the ideal implant is still not there, and whenever a major reconstruction is required consideration should be given first to the use o
