Zero-profile interbody fusion in single and two-level anterior cervical discectomy and fusion: a novel surgical system and its indications.

Anterior cervical discectomy and fusion (ACDF) is considered gold standard for anterior management of cervical degenerative disease, yet traditional interbody and plate constructs often require substantial soft-tissue exposure and may be associated with increased postoperative morbidity and dysphagia/prevertebral swelling. Moreover, anterior plate prominence and prolonged retraction have been implicated in postoperative dysphagia and prevertebral soft-tissue irritation, motivating interest in low-profile fixation strategies. The goal of this paper is to introduce a novel zero-profile interbody

Reconstruction of patella fractures with the tension band technique: A review on clinical results and tips and tricks.

INTRODUCTION: The goals of surgical treatment of patellar fractures are a biomechanically stable joint and congruent restoration of the retro patellar joint surface. Surgical treatment strategies for patellar fractures have evolved from tension band in combination with wire cerclages to new devices. METHODS: The modified anterior tension band (MATB) technique for fixation of patellar fractures consists of two longitudinal 1.8 mm Kirschner wires (K-wires) and an 18-gauge stainless steel wire looped in a figure-of-8 pattern over the anterior aspect of the patella. The K-wires should be inserted

Functional outcomes and complications following combined locking plate and tunneled suspensory device fixation of lateral-end clavicle nonunions.

BACKGROUND: Lateral-end clavicle fractures have a high rate of nonunion that can lead to ongoing pain and loss of shoulder function. The operative techniques used to manage such nonunions vary, and data on postoperative functional outcomes and complications are limited, with no consensus on the optimal surgical treatment. Our goal was to present the outcomes and complications of a new indication for combined locking plate and tunneled suspensory device fixation in the operative management of lateral-end clavicle fracture nonunions. METHODS: A consecutive series of 38 patients (mean age, 46 ye

Chest-wall reconstruction in case of infection of the operative site: is there any interest in titanium rib osteosynthesis?

OBJECTIVES: To describe the management of thoracic reconstructions in the presence of primary chest-wall infection (PCWI) or secondary deep chest-wall infection (SCWI), focussing on local tolerance of a titanium rib osteosynthesis system. METHODS: PCWI included infected chest wall tumours (CWT), infected T3 non-small-cell lung carcinoma (NSCLC) and open flail chest. SCWI was defined by deep infection of previous thoracic-wall reconstructions. Infection was identified by preoperative bacterial analysis of the tumour or surgical site. In PCWI, a one-step procedure combined extensive resection o

The reuse of orthopaedic devices in trauma care.

UNLABELLED: Orthopaedic surgery relies on specialized devices to ensure effective management of musculoskeletal conditions, but escalating costs have compromised care. This has prompted patients and providers worldwide to explore alternative approaches such as reuse of devices. The benefits of reuse include economic savings, improvement in the availability of resources, and environmental conservation. There are, however, risks associated with reuse including concerns regarding mechanical integrity and sterilization. The medicolegal landscape surrounding reuse further complicates the establishm

Aqueous infiltration into an implantable miniaturized telescope.

The implantable miniaturized telescope (IMT) is the first intraocular magnifying system proposed for optical correction in patients with age-related macular degeneration (ARMD). The optical component is embedded in a carrying device designed as an intraocular lens that is implanted after cataract surgery. In this study, we report findings on an IMT that was explanted because of aqueous infiltration into its optic and describe the configuration of this device and the surgical technique required for its implantation. The patient, a 75-year-old male with bilateral cataract and nonexudative ARMD,

Outcomes of distal biceps tendon reattachment using the ToggleLoc™ fixation device with ZipLoop™ technology with single mini-open technique.

Anatomical repair of distal biceps tendon ruptures has been shown to restore elbow supination and flexion strength. Here, we report the outcomes of distal biceps tendon reattachment using the ToggleLoc fixation device with ZipLoop technology through a single incision. This was a retrospective study of 38 patients with a mean age of 49.5 years. The mean follow-up time was 15 months (range 4/28). The average time to surgery was 21 days. The fixation button was introduced in a bone tunnel and the tendon passed through a bone window using the ToggleLoc™, which allows the tendon to be tensioned usi

Proximal femoral nail antirotation (PFN-ATM) fixation of extra-capsular proximal femoral fractures in the elderly: retrospective study in 102 patients.

BACKGROUND: The best surgical strategy for extra-capsular proximal femoral fractures (PFFs) is controversial in the elderly. Poor bone quality and neck screw instability can adversely affect the results with currently available fixation devices, which predominantly consist in dynamic hip screw-plates and proximal reconstruction nails. HYPOTHESIS: The helical blade of the proximal femoral nail antirotation (PFN-A™) achieves better cancellous bone compaction in the femoral neck, thereby decreasing the risk of secondary displacement. MATERIALS AND METHODS: We retrospectively reviewed consecutiv
