The result of unilateral brow suspension in severe unilateral ptosis is said to be cosmetically and functionally not satisfactory. For the reason of symmetry, the myectomy of the levator muscle on the healthy side and the bilateral brow suspension have been recommended (Beard's law). However, we have successfully operated the affected side only in several patients without this far-reaching supplement.

The results of transvaginal needle suspension in 36 patients with stress urinary incontinence who failed previous operation were reviewed. Eighteen patients underwent a Stamey endoscopic needle suspension, and a Peyrera-Raz cystourethropexy was performed on the remainder. Different principles for providing stable and durable suspension of the bladder neck and proximal urethral tissue are employed in endoscopic bladder neck suspension for female stress urinary incontinence. Two cases of symptomatic neonatal hepatic arteriovenous malformation (AVM) are presented. Both were treated with percutaneous transcatheter embolization and a commercially available polyvinyl alcohol suspension. Both infants died soon after AVM embolization. Obstructive uropathy after an operation for stress incontinence was treated by transvaginal urethrolysis in 13 women. Bladder outlet obstruction developed after retropubic urethropexy in 6 patients, after needle suspension in 4 and after a pubovaginal sling operation in 3. Transvaginal urethrolysis was accompanied by a needle suspension in 2 patients, an Inglemann-Sundberg denervation in 1 and a pubovaginal sling in 1. Of 13 patients 10 are voiding and none has stress incontinence but 3 continue to perform intermittent self-catheterization.

Simplified but effective operative techniques have made surgery for stress incontinence less hazardous and traumatic and more acceptable to these patients. Transvaginal needle vesical neck suspensions, by avoiding splitting of the fascia of the abdominal wall, decrease postoperative discomfort and convalescence. Various modifications of the original Pereyra needle vesical neck suspension are currently in use. Probably the simplest and most effective is a technique reported by Raz, which the authors employ as an outpatient procedure and describe here.

Successful treatment of mandibular fractures involves proper fracture reduction and immobilization for an adequate length of time. A simple wire fixation technique that can be used in many situations involves suspension wiring from the base of the anterior nasal spine to a pair of circummandibular wires. The technique may be used alone or adjunctively with other methods of fixation. It offers several advantages over other methods, particularly in the treatment of pediatric mandibular fractures.

Maximum weight bearing observed, using a static weighing scale, in above-knee amputees wearing an airsplint, with a foot, was 10 kg and in below-knee amputees 11 kg with inflation pressures of 40 mmHg. Endoscopic vesicourethral suspension is an acceptable procedure for the treatment of true stress urinary incontinence, and is associated with a high success rate and little morbidity. Endoscopic vesical neck suspension was performed in 60 patients, with cure of the incontinence in 49 (82 per cent). A higher success rate was achieved in patients without a prior hysterectomy and/or surgical procedure to correct incontinence. Abnormalities on urodynamic testing may be somewhat suggestive of outcome. However, these studies cannot be used to predict success or failure.

Sixty female patients with clinically demonstrable stress incontinence of urine have had a Stamey operation during the last 3 years. The results have been good in curing stress incontinence, whilst the majority of the failures have been in patients with severe detrusor instability or a chronic cough. In recent years, uterine suspension has become in infrequent operation on most gynecologic services. While numerous types of suspensions have been described, the round ligament fixation to the rectus sheath described by Gilliam is commonly employed because of its ease, effectiveness, and low complication rate. Laparoscopic visualization of the pelvic organs can allow this type of suspension, requiring only superficial incisions down to the rectus fascia, and thereby markedly reducing the procedure's morbidity and the patient's hospitalization.
