Sensor detection and pacing for impending reflex syncope.

AIMS: Reflex syncope occurs infrequently but may benefit from pacing intervention. However, reflex syncope poses special problems for successful cardiac pacing. Between episodes, monitoring by the implanted device is required to provide warning of an impending syncope and initiate pacing therapy if appropriate. This article aims to review available devices and their function in different forms of reflex syncope. METHODS AND RESULTS: The first device dedicated to address detection of VVS was the Rate Drop Response (RDR) (Medtronic Inc., MN, USA). It used a modified rate hysteresis approach to …

Wide QRS Tachycardias With Syncope: Two for One?

BACKGROUND: The differential diagnosis of wide complex tachycardias can be challenging but is essential for correct diagnosis and treatment. CASE SUMMARY: We describe the case of a patient presenting with 2 wide complex tachycardias, both with left bundle branch block-like morphology, left superior axis, and slurred onset. The diagnosis of pre-excited atrial flutter with 1:1, respectively, 2:1 conduction was made after careful analysis of the tracings. Electrocardiogram in sinus rhythm showed short PR but inapparent pre-excitation. An invasive electrophysiological study confirmed an anterogra

Near-Fatal Mastocytosis Mimicking as Malignant Vasovagal Syncope.

BACKGROUND: Severe apparent reflex syncope may have other causes. CASE SUMMARY: A young girl was investigated for recurrent faints expected to be caused by reflex syncope, but ultimately complicated by cerebral ischemic lesions. A diagnosis of systemic mastocytosis (bone marrow mastocytosis) was finally made, after documentation of short-living abnormal coagulation tests and transient elevated tryptase levels. DISCUSSION: Severe mastocytosis-related vasoplegia can be the underlying cause of syncope and prolonged cerebral ischemia. Mastocytosis is a clonal disorder stemming from an acquired s…

Syncope due to vitamin-B12 deficiency - case report.

To our knowledge, a patient with recurrent syncope due to vitamin B12 deficiency without hematologic manifestations has not been reported before. We present the case of an 83-year-old man who experienced a third episode of syncope, characterized by sudden loss of consciousness without convulsions and rapid recovery. His medical history included ischemic stroke 22 years earlier and multiple bowel resections following cholecystectomy 21 years ago. The examination revealed a severe vitamin B12 deficiency and bradycardia on ECG. The examination for heart disease was inconclusive. The syncope was a…

Hyperaldosteronism Presenting as Pre-syncope: A Case Report.

Primary Aldosteronism (PA) is a common but often underrecognized cause of secondary hypertension. While typically associated with hypokalemia, profound muscle weakness is rarely a presenting symptom, especially in the absence of significant electrolyte derangement. We present a case of a 65-year-old male with hypertension and dyslipidemia who presented with transient acute attacks of profound weakness and presyncope, without loss of consciousness or other focal neurologic signs. Initial evaluation, including cardiac, neurologic, and metabolic testing, was unrevealing. Mild hypokalemia (potassi…

Rapid changes in cerebrovascular compliance during vasovagal syncope.

PURPOSE: The compensatory mechanisms supporting cerebral perfusion throughout head-up tilt (HUT) in patients with vasovagal syncope (VVS) remain unclear. We tested the hypothesis that increased cerebrovascular compliance (Ci) and decreased cerebrovascular resistance (CVR) support cerebral blood velocity (CBV) during pre-syncope in VVS. METHODS: Finger arterial blood pressure (ABP) and right middle cerebral artery blood velocity (CBV) were recorded in 15 individuals diagnosed with VVS (n = 11 female, mean age: 40 ± 16 years, mean body mass index: 24.9 ± 4.0 kg/m2) at supine rest and during HUT…

Increased adrenocortical activity in patients with vasovagal syncope.

OBJECTIVE: Syncope is a transient loss of consciousness resulting from cerebral hypoperfusion. Vasovagal syncope (VVS) is a form of orthostatic intolerance (OI). Its clinical signs such as dizziness and hypotension may mimic symptoms of adrenal insufficiency. The objective of this study was to evaluate the adrenal gland function in patients with vasovagal syncope after stimulation with synthetic adrenocorticotropic hormone (ACTH). DESIGN: Case-control study on patients with VVS and healthy controls. METHODS: The study involved 42 participants, including 27 patients diagnosed with VVS using t…

Syncope in older adults: challenges, approach and treatment.

Syncope can have devastating consequences, resulting in injuries, accidents or even death. In our ageing society, the subsequent healthcare usage, such as emergency room presentations, surgeries and hospital admissions, forms a significant and growing socioeconomic burden. Causes of syncope in the older adult include orthostatic hypotension, carotid sinus syndrome, vasovagal syncope, structural cardiac abnormalities, cardiac arrhythmias and conduction abnormalities. As stated in the recently published World Falls Guidelines, syncope in older adults often presents as falls, which is either due …

Syncope vs. Seizure: Ictal Bradycardia and Ictal Asystole.

BACKGROUND: Ictal arrhythmia is a rare condition that causes arrhythmic manifestations induced by epileptic seizures, including asystole or bradycardia. Ictal asystole (IA) is a very rare condition found in patients undergoing video-encephalography (EEG) monitoring. It is often related to temporal lobe epilepsy and can cause syncope, which can lead to injury or even death. Case Presentation. Two patients with epilepsy showed symptoms of syncope. Both patients underwent 4-day ambulatory EEG tests and were diagnosed with IA. Following the tests, the patients were implanted with a permanent pacem…

Syncope in ICD recipients: a single centre experience.

AIMS: There is little evidence of the impact of syncope in implantable cardioverter-defibrillator (ICD) patients in routine community hospital care. This single-centre retrospective study sought to evaluate the incidence and prognostic significance of syncope in consecutive ICD patients. METHODS AND RESULTS: Data were collected on consecutive patients undergoing first ICD implantation between January 2009 and December 2019. The primary endpoints were the first occurrence of all-cause syncope, all-cause mortality, and all-cause hospitalization. Multivariate Cox proportional hazard models were …

Ganglionated Plexus Ablation Procedures to Treat Vasovagal Syncope.

Vasovagal syncope (VVS) refers to a heterogeneous group of conditions whereby the cardiovascular reflexes normally controlling the circulation are interrupted irregularly in response to a trigger, resulting in vasodilation, bradycardia, or both. VVS affects one-third of the population at least once in their lifetime or by the age of 60, reduces the quality of life, and may cause disability affecting certain routines. It poses a considerable economic burden on society, and, despite its prevalence, there is currently no proven pharmacological treatment for preventing VVS. The novel procedure of …

Syncope and subsequent traffic crash: A responsibility analysis.

BACKGROUND: Physicians are often asked to counsel patients about driving safety after syncope, yet little empirical data guides such advice. METHODS: We identified a population-based retrospective cohort of 9,507 individuals with a driver license who were discharged from any of six urban emergency departments (EDs) with a diagnosis of 'syncope and collapse'. We examined all police-reported crashes that involved a cohort member as a driver and occurred between 1 January 2010 and 31 December 2016. We categorized crash-involved drivers as 'responsible' or 'non-responsible' for their crash using …

Early and late-onset syncope: insight into mechanisms.

AIMS: Unexplained syncope is an important clinical challenge. The influence of age at first syncope on the final syncope diagnosis is not well studied. METHODS AND RESULTS: Consecutive head-up tilt patients (n = 1928) evaluated for unexplained syncope were stratified into age groups <30, 30-59, and ≥60 years based on age at first syncope. Clinical characteristics and final syncope diagnosis were analysed in relation to age at first syncope and age at investigation. The age at first syncope had a bimodal distribution with peaks at 15 and 70 years. Prodromes (64 vs. 26%, P < 0.001) and vasovaga…

Nonpenetrating trauma resulting in hemopericardium presenting as syncope.

Hemopericardium is a life-threatening condition that needs to be treated promptly to prevent serious complications. It typically results from penetrating trauma directly into the pericardial sac resulting in cardiac tamponade and death if not promptly treated. High clinical suspicion should prompt further workup even in the absence of significant injury. We present a case of a 38-year-old man who presented with blunt force trauma from a rock while mowing the lawn that resulted in syncope and hemopericardium.

Electrocardiographic Patterns in Patients with Neurally Mediated Syncope.

The baseline electrocardiogram (ECG) is less informative in neurally mediated syncope (NMS) than in arrhythmic syncope. However, some of the ECG patterns present in NMS can have diagnostic and prognostic value in such patients. Electrocardiographic documentation of a syncopal spell and thus identification of the ECG changes can be performed during tilt table test (TTT) or during prolonged ECG monitoring. This work reviews the specific ECG patterns in NMS, which are primarily related to the cardioinhibitory reflex. In addition, there are other ECG findings present in patients with NMS that are …

Micturition syncope: a rare presentation of bladder paraganglioma.

A 68-year-old woman presented with episodes of headache, palpitations, sweating and poorly controlled hypertension for the past 6 years. These symptoms were, at times, associated with micturition, and there were few episodes of micturition syncope as well. She had elevated 24-hour urinary normetanephrine and was found to have a paraganglioma arising from the urinary bladder infiltrating the sigmoid colon. She underwent laparotomy with excision of the bladder paraganglioma, following which her symptoms subsided. Paragangliomas are extra-adrenal catecholamine-producing tumours. Bladder paragangl…

Recurrent Septic Shock and Syncope: An Unusual Combination.

Immunoglobulin G (IgG) deficiency disorder is a form of dysgammaglobulinemia, where proportional levels of immunoglobulin G isotype is reduced as compared to other immunoglobulin isotypes. Common clinical presentations of IgG deficiency disorder are recurrent sinusitis and/or lower respiratory infections. However, IgG deficiency manifesting as recurrent septic shock in absence of upper and lower respiratory infection has never been reported, in our search for literature from world over. With this rare case, we highlight the importance of investigating IgG levels in clinical scenarios of recurr

Syncope: Clinical Study and Outcome of Diagnostic Evaluation.

BACKGROUND: Syncope is a common clinical presentation in emergency department. The diagnostic workup for syncope causes significant man-hour loss and expensive investigations. Most often the battery of investigations does not lead to any conclusive diagnosis. MATERIALS AND METHODS: This was a descriptive study in which included 50 consecutive patients with the diagnosis of syncope. These patients were admitted to a tertiary care hospital between Sep 2009 to Aug 2011. Patients of both sexes above 12 years of age were included in the study.
