Advances in biomedical technology have transformed dying into a prolonged, technological process in which clinical attention can drift from the person to physiological parameters. This narrative review argues that phenomenology offers an essential ethical corrective by foregrounding the lived experience of dying and complementing procedural bioethics, with particular attention to South Asia, where cultural, spiritual, and familial dimensions strongly shape end-of-life decisions. Drawing on classical phenomenologists and contemporary scholars of illness experience, the review synthesises insigh

End-of-life care is often approached from the perspective of dignity and respect for autonomy, which enable individuals to choose how to spend their final moments. However, in a critical care setting, where older people may be incapable of making decisions or may become temporarily so due to an altered level of consciousness, the burden of decision-making falls on their loved ones and the medical team. How is dignity preserved when treatment choices clash with socio-cultural issues and the sometimes conflicting interests of those involved? Through short accounts drawn from a qualitative study

Palliative sedation (PS) is a morally complex end-of-life practice that sits at the intersection of medicine, law, religion, and culture. While often framed as a purely clinical intervention aimed at alleviating intractable suffering, this article argues that PS functions—particularly in pluralistic societies such as Israel—as a form of ethical mediation and cultural accommodation. Through a philosophical-theoretical inquiry, this paper explores PS not as a universally justified act, but as a “cultural solution”: a pragmatic ethical practice that enables care to proceed amid deep moral disagre

BACKGROUND: Electronic Palliative Care Co-ordination Systems are central to end-of-life care policy despite critical gaps in the evidence base underpinning their use. It is unclear how Electronic Palliative Care Co-ordination Systems are being implemented across England and there is a lack of research to understand their use in routine care.
AIM: To understand how Electronic Palliative Care Co-ordination Systems are currently being used in routine care and to guide the development of interventions to support their optimal implementation and maximise patient benefit.
METHOD: A mixed-method ap

BACKGROUND: Social homecare workers (personal aides/assistants) are crucial for people wishing to receive end-of-life care at home.
AIM: To determine current UK social care policy priorities and gaps regarding end-of-life care provision by homecare workers, including support and training for this workforce.
DESIGN: Policy document review and content analysis; eligible documents were current UK policy documents informing homecare worker practice/service delivery, identified by team expertise, networks, study partners and bibliography review of included documents. We quantified (existence, fre

INTRODUCTION: Many tools and interventions developed from research projects are not properly implemented in health and social care practice. Engaging with stakeholders is an important part of the implementation process when developing, sharing and embedding new tools. The Victoria & Stuart Project was a 2-year project that co-designed a toolkit of resources and approaches to support end-of-life care planning for people with intellectual disabilities. It had an inclusively delivered dissemination and engagement workstream to raise awareness of the toolkit throughout the project. In this paper,

BACKGROUND: Palliative care supports the physical, emotional, social and spiritual needs of people with serious life-limiting illness. Future research must align with the priorities of people approaching the end of their lives, and those close to them.
AIMS: To undertake a refresh of the James Lind Alliance Palliative and End of Life Care Priority Setting Partnership, to identify and prioritise areas for future research.
DESIGN: The James Lind Alliance process was applied, between May 2023 and February 2025. An initial online survey collected areas for future research from participants. Thes

BACKGROUND: Making a care-related decision is a complex cognitive process. Patient decision aids could provide information on potential options about risks and benefits, incorporate individual values and preferences, and help people with dementia or their family carers make decisions about palliative and end-of-life care.
OBJECTIVE: This systematic review aimed to critically evaluate and synthesize evidence on the effectiveness of digital decision aids to support decision-making in palliative and end-of-life care for patients with dementia, their family carers, or clinicians.

This paper argues that philosophical practice can complement existing medical ethics structures by offering a publicly accessible space for discourse and negotiation of basic concepts that are relevant to ethical decision making. The potential of collaboration becomes particularly evident by the example of assisted dying: it raises a wide range of philosophical questions which, however, tend to remain unarticulated where there is urgency for action and focus on ethical dilemma. In this paper, we first provide an analysis of the neglect of profound questions in ethics consultation and why these

Healthcare practitioners are regularly faced with treating patients at the end of their life, and this can be very daunting. This article hopes to help the practitioner have an approach to managing end-of-life care that makes it less distressing. The symptoms at the end-of-life include delirium and/or agitation, breathing changes, skin changes, sleeping more, decrease in need for food and drink, incontinence, and increased secretions. These symptoms are discussed and practical ways of management are given. The article further discusses how to approach the difficult conversation with the family

OBJECTIVES: Breathlessness is the most significant symptom in those dying of COVID-19. Historically, though, it has often been palliated poorly at end of life. The aim of this work was to assess whether breathlessness in patients dying from COVID-19 was being managed appropriately.
METHODS: A multicentre, retrospective analysis of clinical data was undertaken. Patients who had died of COVID-19 across three acute hospitals over a 2-month period were included. Those already prescribed background opioids and those who died in intensive care were excluded. Data were collected from clinical notes,

CONTEXT: Parkinson's disease and related disorders (PDRD) are fatal neurodegenerative disorders characterized by a fluctuating course that can complicate prognostication. The "surprise question" (SQ: "Would you be surprised if your patient died in the next year?") has been used to identify patients with limited prognosis but has not been assessed in PDRD.
OBJECTIVES: To determine the validity of the SQ in predicting 12-month mortality in PDRD.
METHODS: Data was analyzed from 301 patients and 34 community-based neurologists who were participating in a clinical trial of outpatient palliative c

BACKGROUND: Law 219/2017 was approved in Italy in December 2017, after a years-long debate on the autonomy of healthcare choices. This Law, for the first time in Italian legislation, guarantees the patient's right to request for withdrawal of life-sustaining treatments, including mechanical ventilation (MV).
OBJECTIVE: To investigate the current status of MV withdrawal in amyotrophic lateral sclerosis (ALS) patients in Italy and to assess the impact of Law 219/2017 on this practice.
METHODS: We conducted a Web-based survey, addressed to Italian neurologists with expertise in ALS care, and me

BACKGROUND: 'Most patients want to die at home' is a familiar statement in palliative care. The rate of home deaths is therefore often used as a success criterion. However, providing palliative care and enabling patients to die at home in rural and remote areas may be challenging due to limited health care resources and geographical factors. In this study we explored health care professionals' experiences and reflections on providing palliative care to patients at the end of life in rural Northern Norway.
METHODS: This is a qualitative focus group and interview study in rural Northern Norway

OBJECTIVES: The claim of a right to pain relief was made in recent decades by pain professionals, courts, and patient advocacy groups and likely contributed to increased opioid prescribing, overdose deaths, and addictions, but the origins and nature of this right have not been investigated.
MATERIALS AND METHODS: Relevant clinical, ethical, and legal literature concerning patient rights to pain care was reviewed.
RESULTS: The record describes the effort to improve end-of-life and cancer pain care in the 1980s and 1990s, which simultaneously legitimated pain relief as an independent goal of m

BACKGROUND: People can live for many months without knowing why their body is failing prematurely before being diagnosed with motor neurone disease (MND); a terminal neurodegenerative disease which can be experienced as 'devastating' for the person and their family.
AIM: This study aimed to explore the meaning of supporting a loved one with MND to die.
METHODS: This study uses reflection and autobiographical story to connect with broader cultural, political and social meaning and understandings of dying.
FINDINGS: Four themes were identified relating to the end-of-life trajectory of MND. Lo

Quality end-of-life care is a central issue in nursing homes, requiring the assessment of individual and family needs by health professionals. Although previous instruments have been developed, they usually rely on family reports and have been adapted from other clinical contexts (hospital or primary care). It is important to consider how health care professionals working in nursing homes perceive what is necessary to achieve quality end-of-life care. In this study, the objective was to develop an instrument to assess quality of end-of-life care in the context of Spanish care homes. A 24 item

PURPOSE: In the palliative oncology setting, genetic assessment may not impact on the patient's management but can be of vital importance to their surviving relatives. Despite care of the family being central to the ethos of palliative care, little is known about how hereditary aspects of cancer are addressed in this setting. This review aims to examine current practices, identify practice barriers and determine the genetic information and support needs of patients, family members and health providers.
METHODS: Key databases were systematically searched to identify both quantitative and quali

BACKGROUND: The benefits and burdens of artificial nutrition (AN) and artificial hydration (AH) in end-of-life care are unclear. We carried out a literature review on the use of AN and AH in the last days of life of cancer patients.
MATERIALS AND METHODS: We systematically searched for papers in PubMed, CINAHL, PsycInfo and EMBASE. All English papers published between January 1998 and July 2009 that contained data on frequencies or effects of AN or AH in cancer patients in the last days of life were included.
RESULTS: Reported percentages of patients receiving AN or AH in the last week of li
