Ballistic impact response of a fluid/structure coupling-based modification of human thorax modelling.

Studying the mechanical response of the human thorax submitted to ballistic impact is a challenging field of research. For ethical reasons, it is not possible to perform tests on the human body. Numerical simulations are therefore one of the alternatives for evaluating the mechanical response of the human thorax. In earlier research, a simplified human thorax submitted to low-velocity impacts without ballistic protection was modelled. Not all internal organs in the thorax were taken into account. The only considered organs were the heart, lungs, trachea and aorta, with the space between them b…

Esophageal atresia with right aortic arch: An experience of thoracoscopic repair through left thorax.

Right aortic arch (RAA) is one of congenital cardiovascular anomalies associated with esophageal atresia (EA). The surgical treatment for EA with RAA is still challenging. Although most pediatric surgeons are familiar with the right-sided approach, the division of the tracheoesophageal fistula and the anastomosis of the esophagus through right thorax are often difficult in cases of RAA. There are a few reports on thoracoscopic repair for EA with RAA. We report a case of EA with RAA treated by left-sided thoracoscopic approach. With left-sided thoracoscopic approach, identification and anastomo…

Emergency chest wall reconstruction in open pneumo-thorax from gunshot chest: A case report.

Chest trauma, penetrating or blunt is common in this era of motor vehicle accidents, violence and terrorism in South Asia. Islamabad is the capital of Pakistan but there is no dedicated chest surgery unit in any government sector hospitals. Gunshot chest, is therefore managed by general surgery team in our tertiary care setting i.e. Federal Government Polyclinic Hospital and Post Graduate Medical Institute, Islamabad. We report a case of gunshot chest with lung contusion and open pneumothorax with a chest wall defect of 10 x 15 cm. in March 2015, this young man presented in emergency departmen…

Thorax masculinization in a transsexual patient: Inferior pedicle mastectomy without an inverted T scar.

Transsexual individuals with gender dysphoria or gender identity disorder are rare, with a prevalence reported to range from 0.002% to 0.014%. Studies have shown that mastectomy yields significant improvements in body image and self-esteem in female-to-male transsexual patients. In patients with grade III breast ptosis, mastectomy with a nipple-areolar complex (NAC) graft is the most commonly used technique, although it has several disadvantages. In the case described herein, a bilateral mastectomy preserving the NAC in an inferior pedicle was performed. Additionally, a thin superior thoracic

Bioprosthetic mitral valve implantation for active mitral valve endocarditis in third degree thorax burn.

Infective endocarditis remains a devastating complication in the thermally injured patient. The treatment of infective endocarditis is primarily medical, but surgical intervention is often mandatory when various complications arise. We believe that this is the first reported case of mitral valve replacement in a patient with third degree burns to the thorax. We also discuss surgical indications and treatment options in this clinical setting.

MRI "road-map" of normal age-related bone marrow. II. Thorax, pelvis and extremities.

We retrospectively reviewed T1-weighted MR images of 381 patients aged from 7 days to 24 years to evaluate the bone marrow change in thoracic wall and shoulder, pelvis and proximal femur and upper and lower extremities. The patients included in the study were without history of bone marrow disease. A grade of from 1 to 4 was assigned to the marrow signal intensity of the examined anatomic segments. The signal intensity of all anatomic segments was as low as or lower than that of muscle in all patients younger than 2 months, reflecting underlying hematopoietic marrow. The first segments to beco

Intercostal nerve block for postoperative somatic pain following surgery of thorax and upper abdomen.

From 1948 to 1973, intercostal nerve block was used 10,941 times or a total of approximately 100,000 individual nerves were blocked. Junior staff (residents) performed 95% of the blocks. The local anaesthetic solution of choice used 0.25 or 0.5% bupivacaine (Marcaine) with adrenaline. Total milligram doses of bupivacaine as high as 400 mg were employed. The duration of the blocks was 9-18 hr. No severe systemic toxic reactions occurred, e.g. disorientation, convulsions, etc. The incidence of pneumothorax was 0.073%.

Explaining and predicting the increased thorax injury in aged females: age and subject-specific thorax geometry coupled with improved bone constitutive models and age-specific material properties evaluated in side impact conditions.

Predicting and understanding thorax injury is fundamental for the assessment and development of safety systems to mitigate injury risk to the increasing and vulnerable aged population. While computational human models have contributed to the understanding of injury biomechanics, contemporary human body models have struggled to predict rib fractures and explain the increased incidence of injury in the aged population. The present study enhanced young and aged human body models (HBMs) by integrating a biofidelic cortical bone constitutive model and population-based bone material properties. The …

Utilization of a thorax anthropomorphic phantom for dose assessment and scanning protocol synchronization in computed tomography.

Optimizing radiation dose while maintaining image quality remains a key challenge in computed tomography (CT). This study used a thorax-specific anthropomorphic phantom to assess patient dose, image quality, and artefact presence under automated tube current modulation settings and to evaluate the effect of tube voltage on Hounsfield Unit (HU) response for different iodine concentrations. Dose metrics, noise levels, and image quality indicators were extracted from Digital Imaging and Communications in Medicine (DICOM) metadata and image analysis. The study examined how patient positioning affe…

[Investigation of factors influencing the quality of chest compressions in dogs using a realistic thorax model].

SUBJECT AND OBJECTIVE: The quality of chest compressions is crucial for the success of resuscitation. To date, there is limited veterinary medical evidence on this topic. The influence of the rescuer's position on the quality of resuscitation was investigated in order to determine the most effective approach. MATERIAL AND METHODS: Using a realistic thorax model equipped with an "intracorporeal" pressure sensor, 30 participants performed chest compressions for 5 minutes each in three scenarios (randomized order): in Scenario 1 kneeling beside the model; in Scenarios 2 and 3 standing at a table…

Surface Electromyography Measures of Muscle Activity in the Abdomen, Thorax, and Neck in Professional Trumpet Players.

BACKGROUND: Trumpet playing requires precise embouchure, breath control, and postural support, engaging facial and respiratory muscles as well as muscles within the abdomen, thorax, and neck. Research on muscle activity during trumpet playing focuses mainly on embouchure and its involved facial muscles, whereas studies investigating torso muscles are scarce. METHODS: In this observational study, the activity of the abdominal, thoracal, and neck muscles of 24 male professional trumpet players between 20-45 years of age were measured with surface electromyography (sEMG) during playing. Electrod…

Angular motion of the thorax during the golf swing: a comparison of two orientation angle sequences.

Orientation angles are commonly used to describe complex angular motions of the body. Selecting the most appropriate rotation sequence for a given segment's motion is crucial. The purpose of this study was to develop a set of generalisable, primary axis-centric sequence selection strategies and to compare the lean direction-lean-rotation (LDLR) sequence, selected for thoracic motion during golf driving based on the strategies, with the conventional rotation-bend-side bend (RBSB) sequence in assessing the level of inter-angle cross-talk. The RBSB method consistently revealed a higher level of i

HEAD AND THORAX ELEVATION PREVENTS THE RISE OF INTRACRANIAL PRESSURE DURING EXTRACORPOREAL RESUSCITATION IN SWINE.

Aim: Head and thorax elevation during cardiopulmonary resuscitation improves cerebral hemodynamics and ultimate neurological outcome after cardiac arrest. Its effect during extracorporeal cardiopulmonary resuscitation (E-CPR) is unknown. We tested whether this procedure could improve hemodynamics in swine treated by E-CPR. Methods and Results: Pigs were anesthetized and submitted to 15 minutes of untreated ventricular fibrillation followed by E-CPR. Animals randomly remained in flat position (flat group) or underwent head and thorax elevation since E-CPR institution (head-up group). Electric s…

Using Radiomics as Prior Knowledge for Thorax Disease Classification and Localization in Chest X-rays.

Chest X-ray becomes one of the most common medical diagnoses due to its noninvasiveness. The number of chest X-ray images has skyrocketed, but reading chest X-rays still have been manually performed by radiologists, which creates huge burnouts and delays. Traditionally, radiomics, as a subfield of radiology that can extract a large number of quantitative features from medical images, demonstrates its potential to facilitate medical imaging diagnosis before the deep learning era. In this paper, we develop an end-to-end framework, ChexRadiNet, that can utilize the radiomics features to improve t

A Pilot Study to Evaluate Early Predictive Value of Thorax Perfusion-CT in Advanced NSCLC.

BACKGROUND: The role of perfusion computed tomography (pCT) in detecting changes in tumor vascularization as part of a response to antiangiogenic therapy in non-small cell lung cancer (NSCLC) remains unclear. METHODS: In this prospective pilot study (IMPACT trial, NCT02316327), we aimed to determine the ability of pCT to detect early changes in blood flow (BF), blood volume (BV), and permeability (PMB), and to explore whether these changes could predict the response at day +42 in patients with advanced, treatment-naive, non-squamous NSCLC treated with cisplatin and gemcitabine plus bevacizuma
