Case Report: Successful treatment of steroid- and ruxolitinib-refractory gastrointestinal acute graft-versus-Host disease with anti-thymocyte globulin.

Acute graft-versus-host disease (aGVHD), particularly with gastrointestinal (GI) involvement, remains a life-threatening complication after allogeneic hematopoietic stem cell transplantation (HSCT). Despite corticosteroids and ruxolitinib as first- and second-line therapies, up to 50% of patients develop refractory disease, with limited evidence guiding third-line interventions. Anti-thymocyte globulin (ATG), historically used in conditioning regimens, has shown variable efficacy in steroid-refractory aGVHD, but its role in patients previously exposed to ATG prophylaxis remains underexplored.

The search to understand the development of the chicken immune system: Differences in expression of MHC class I loci and waves of thymocytes as evolutionary relics?

Chickens are renowned as a model for embryogenesis but have also been responsible for crucial advances in virology, cancer research and immu … thymic emigrants are also found for chickens, and reasoning by analogy, it may be that the waves of thymocytes and the expression of class I molecules during ontogeny of chickens are evolutionary relics. As well as scientific interest in the ontogeny of MHC class I expression and appearance of peripheral T cells, there are potential practical implications, g …

Anti-Thymocyte Globulin Treatment Augments 1,25-Dihydroxyvitamin D3 Serum Levels in Patients Undergoing Hematopoietic Stem Cell Transplantation.

Application of anti-thymocyte globulin (ATG) is a widely used strategy for the prevention of graft-versus-host disease (GvHD). As vitamin D3 serum levels are also discussed to affect hematopoietic stem cell transplantation (HSCT) outcome and GvHD development, we analysed a possible interplay between ATG treatment and serum levels of 25-hydroxyvitamin D3 and 1,25-dihydroxyvitamin D3 in 4 HSCT cohorts with different vitamin D3 supplementation. ATG is significantly associated with higher serum level of 1,25-dihydroxyvitamin D3 around HSCT (day -2 to 7, peri-transplant), however only in patients w

LCK-Mediated RIPK3 Activation Controls Double-Positive Thymocyte Proliferation and Restrains Thymic Lymphoma by Regulating the PP2A-ERK Axis.

Receptor-interacting protein kinase 3 (RIPK3) is the primary regulator of necroptotic cell death. RIPK3 expression is often silenced in various cancer cells, which suggests that it may have tumor suppressor properties. However, the exact mechanism by which RIPK3 negatively regulates cancer development and progression remains unclear. This report indicates that RIPK3 acts as a potent regulator of the homeostatic proliferation of CD4+ CD8+ double-positive (DP) thymocytes. Abnormal proliferation of RIPK3-deficient DP thymocytes occurs independently of the well-known role for RIPK3 in necroptosis …

Cytomegalovirus in renal transplant recipients from living donors with and without valganciclovir prophylaxis and with immunosuppression based on anti-thymocyte globulin or basiliximab.

BACKGROUND: In our population, anti-thymocyte globulin (ATG) of 1 mg/Kg/day for 4 days is used; which permits not using valgancyclovir (VGC) prophylaxis in some renal transplant recipients (RTR) with moderate risk (R+), to reduce costs. This study aimed to determine the incidence and risk of developing cytomegalovirus (CMV), with or without prophylaxis, when exposed to low doses of ATG or basiliximab (BSL). PATIENTS AND METHODS: A retrospective cohort included 265 RTR with follow-up of 12 months. Prophylaxis was used in R-/D+ and some R+. Tacrolimus (TAC), mycophenolate mofetil, and prednison

Long-term follow-up study of porcine anti-human thymocyte immunoglobulin therapy combined with cyclosporine for severe aplastic anemia.

Immunosuppressive therapy with antithymocyte immunoglobulin (ATG) and cyclosporine A is the first treatment option for severe aplastic anemia (SAA) patients without transplantation. Horse ATG is not marketed in China. Because the price of porcine ATG (pATG) is only about one-third of the price of rabbit ATG (rATG), long-term follow-up studies of pATG's efficacy will help provide valuable insights into the treatment of SAA. Retrospective studies were performed to analyze the clinical information of 102 SAA patients treated with pATG and cyclosporine A from 1999 to 2014 in Peking Union Medical C

ATM deficiency impairs thymocyte maturation because of defective resolution of T cell receptor alpha locus coding end breaks.

The ATM (ataxia telangiectasia mutated) protein plays a central role in sensing and responding to DNA double-strand breaks. Lymphoid cells are unique in undergoing physiologic double-strand breaks in the processes of Ig class switch recombination and T or B cell receptor V(D)J recombination, and a role for ATM in these processes has been suggested by clinical observations in ataxia telangiectasia patients as well as in engineered mice with mutations in the Atm gene. We demonstrate here a defect in thymocyte maturation in ATM-deficient mice that is associated with decreased efficiency in V-J re

Increase of Thy-1 antigen on the thymocytes accompanied with their augmented adhesion capacity to thymic epithelial cells in the mice infected with Listeria monocytogenes.

It becomes increasingly clear that adhesion systems such as CD2/LFA-3 (lymphocyte function-associated antigen-3), LFA-1/ICAM-1 (intercellular adhesion molecule-1) and Thy-1/putative Thy-1 ligand participate in the association between murine thymocytes and thymic epithelial cells. In the present study, thymocytes showed an increase in surface Thy-1 levels in mice infected with Listeria monocytogenes, but no significant changes in the levels of CD2 or LFA-1. No alteration was found either in the ratio of CD3high/CD3low/CD3- or in that of CD4/CD8 subsets in these thymocytes compared with uninfect…

Erythrocyte Integrity, Thymocyte Volume Regulation, and Functional State of the Heart during Ischemia/Reperfusion in Rats with Type 2 Diabetes Mellitus.

In the streptozotocin-nicotinamide model of type 2 diabetes mellitus (T2DM), erythrocytes exhibit increased resistance to osmotic and colloid-osmotic lysis. For thymocytes, cell volume regulation in T2DM was found to be significantly less effective than in the control. Using the Langendorff isolated heart retrograde perfusion model, we demonstrated that reperfusion after 30-min global ischemia leads to a dramatic decrease in HR, a decrease in left-ventricular systolic pressure, left-ventricular developed pressure, and the rate of left-ventricular pressure rise and fall. While the general patte…

Early versus standard anti-thymocyte globulin administration in haploidentical transplantation using post-transplant cyclophosphamide: a pilot comparative study.

Combining rabbit anti-thymocyte globulin (ATG) with post-transplant cyclophosphamide (PTCy) reduces graft-versus-host disease (GVHD) after haploidentical peripheral blood stem cell transplantation (haplo-PBSCT) but may increase cytomegalovirus (CMV) reactivation due to intensified immunosuppression. Because ATG exposure at the time of graft infusion is influenced by administration timing, we evaluated whether earlier ATG delivery modifies CMV outcomes while maintaining comparable observed transplant outcomes in this small pilot cohort. In this single-center, non-randomized pilot study, 36 adul

Absolute CD3+ T-Cell Counts After Anti-Thymocyte Globulin: Impact on Graft and Infectious Outcomes in Pediatric Kidney Recipients.

BACKGROUND: Rabbit anti-thymocyte globulin (r-ATG) is a thymocyte depleting agent for the induction of immunosuppression in kidney transplant recipients (KTRs). There is little data comparing a level of CD3+ T-cell suppression for optimal graft outcomes with minimized infectious risk. METHODS: KTRs from January 2020 to January 2024 were included if they met these criteria: (1) < 21 years of age at transplantation, (2) no previous receipt of a solid organ or hematopoietic stem cell transplant, (3) completion of a r-ATG-based induction protocol, (4) had absolute CD3+ T-cell counts quantified at

Single-cell multiomic analysis of thymocyte development reveals drivers of CD4+ T cell and CD8+ T cell lineage commitment.

The development of CD4+ T cells and CD8+ T cells in the thymus is critical to adaptive immunity and is widely studied as a model of lineage commitment. Recognition of self-peptide major histocompatibility complex (MHC) class I or II by the T cell antigen receptor (TCR) determines the CD8+ or CD4+ T cell lineage choice, respectively, but how distinct TCR signals drive transcriptional programs of lineage commitment remains largely unknown. Here we applied CITE-seq to measure RNA and surface proteins in thymocytes from wild-type and T cell lineage-restricted mice to generate a comprehensive timel…

Clinical outcomes with low dose anti-thymocyte globulin in patients undergoing matched unrelated donor allogeneic hematopoietic cell transplantation.

Anti-thymocyte globulin (ATG) has been associated with decreased rates graft versus host disease (GVHD) but with a potential risk of increasing risk of infection and relapse. We retrospectively studied the impact of single dose low dose (2.5 mg/kg) ATG in patients undergoing allogenic hematopoietic cell transplantation (HCT) from 8/8 matched unrelated donors (MUD). Of the total 209 patients identified, 129 received ATG. At baseline, the ATG group had more intermediate and high disease risk index (DRI) (64.6% vs. 54.3%) (28.3% vs. 23.7%) p < .001, respectively, and who received reduced intensit

The impact of anti-thymocyte globulin on the outcomes of Patients with AML with or without measurable residual disease at the time of allogeneic hematopoietic cell transplantation.

Measurable residual disease (MRD) status pre-allogeneic hematopoietic cell transplantation (allo-HCT) has been shown to predict transplant outcomes. We investigated the effect of Anti-Thymocyte Globulin (ATG) on acute myelogenous leukemia (AML) relapse by pretransplant MRD status. AML patients undergoing allo-HCT in first complete remission from either a matched sibling or unrelated donor during the 2006-2017 period were selected. Outcomes of 1509 patients (MRD+, n = 426) were studied. ATG was used in 561 (52%) and 239 (58%) patients within the MRD- and MRD+ cohorts, respectively. In MRD- pati
