Optical coherence tomography as a marker of vision in children with optic pathway gliomas.

PURPOSE: Optic pathway gliomas (OPG) represent an important cause of visual loss in pediatric population. The indication of treatment is based on clinical or neuroimaging progression. Visual acuity loss is the most important symptom of disease progression, but children with OPG are frequently unable to complete the testing of visual function. Optical coherence tomography (OCT) was suggested as an objective tool for visual assessment. A literature review was performed in order to determine the role of retinal OCT as a surrogate marker of vision in children with OPG. METHODS: The search was per

Development of a Fiber-Optic Optical Coherence Tomography Probe for Intraocular Use.

PURPOSE: To evaluate the performance of a newly developed 23-G optical coherence tomography (OCT) probe in animal and human eyes. METHODS: The probe is a side-imaging OCT device with a scanning beam set 43° to the optical axis and a working distance of 1.5 to 2.0 mm. The performance of the OCT probe was tested during vitrectomy in porcine cadaver eyes and rabbit eyes in situ. Optical coherence tomography images of a normal retina, retinal break, optic disc, pars plicata of the ciliary body, and intraoperative surgical manipulations were recorded. The probe was also tested in a pilot study of

Transsynaptic retinal degeneration in optic neuropathies: optical coherence tomography study.

PURPOSE: Recently demonstrated neuronal loss in the inner nuclear layer of the retina in multiple sclerosis (MS) and glaucoma raises the question of a primary (possibly immune-mediated) or secondary (transsynaptic) mechanism of retinal damage in these diseases. In the present study we used optical coherence tomography to investigate retrograde retinal transsynaptic degeneration in patients with long-standing and severe loss of ganglion cells due to optic neuropathy. METHODS: Fifteen eyes of glaucoma patients with visual field defect limited to upper hemifield and 15 eyes of MS patients with p

Optical coherence tomography in the diagnosis and management of optic neuritis and multiple sclerosis.

Optic neuritis (ON) is an inflammatory optic nerve injury, which is strongly associated with multiple sclerosis (MS). Optical coherence tomography (OCT) has the potential to provide a reliable means of capturing axonal deficits, which can be paired to tests of visual function to provide a structural-functional paradigm of brain injury. In this respect, the eye provides a unique view into the effects of central nervous system inflammation, which may enhance the understanding of disease mechanisms that contribute to neurological disability in MS. This review addresses the published experience wi

[The use of optical coherence tomography in hereditary optic neuropathies: description of a family].

CASE REPORT: This report describes two siblings, a woman aged 44 years and her brother aged 29 years, who both complained of visual loss in both eyes. The woman had bilateral optic nerve (ON) temporal pallor and severe reduction of ON fibre layer thickness in this area. Both she and her brother had cecocentral defects in perimetry and color vision deficiency with a marked tritanopia deficit. DISCUSSION: ON pallor, limited to the temporal region, is the basic criterion for the diagnosis of dominant optic atrophy. Optical coherence tomography can be a useful technique in defining the diagnosis

Optical coherence tomography imaging of the optic nerve head pre optic and post optic nerve sheath fenestration.

Idiopathic intracranial hypertension (IIH) is a disorder of unknown aetiology, and causes elevated intracranial pressures. This is often associated with papilloedema with subsequent degrees of optic atrophy if the intracranial pressures are not controlled in a timely manner. Optical coherence tomography imaging is widely recognised for its use in the monitoring of optic nerves in glaucoma, and this report is the first to describe its use to monitor the optic nerve head pre optic and post optic nerve sheath fenestration.

[Optical coherence tomography angiography (OCT-A) and the (non-glaucomatous) optic nerve].

Optical coherence tomography angiography (OCTA) is a non-invasive, reliable, repeatable exam. As opposed to fluorescein angiography, OCTA allows analysis of the capillary bed without interference from diffusion phenomena. It is now widely used for both the diagnosis and ongoing care of retinal and choroidal diseases. However, its use is still limited for evaluation of the optic nerve. Recently, new data quantification software can add value to macular and papillary vascularization images. This review details OCTA results in the most important optic nerve conditions, whether acquired or congeni

Findings of optical coherence tomography angiography of nasal optic disc hypoplasia.

PURPOSE: This study aimed to present a case of a woman diagnosed with congenital nasal optic disc hypoplasia (NOH) in the right eye. OBSERVATIONS: A woman in her 20s presented with a small optic disc, irregular optic disc margins with a double-ring sign, and wedge-shaped defects temporally extending from Mariotte's blind spot in the right eye. The radial peripapillary capillary (RPC) density around the optic nerve disc was examined using optical coherence tomography angiography (OCTA). The best-corrected visual acuity was 1.2 OU, and intraocular pressure was 12 mmHg OU. Optical coherence tomo

Optical coherence tomography angiography for the measurement of optic disc: Macular relationship.

INTRODUCTION: Fundus photography is the gold standard for assessing ocular torsion over the last 30 years. However, it is not a precise and reproducible tool during clinical practice. Optical coherence tomography angiography is characterized by precise identification of the macula and the optic disc, and it could be an effective method to easily calculate the angle of ocular torsion, compared to fundus photography. The aim of this study was to show whether any difference in the measurement and the accuracy of the angle of torsion between the head of the optic nerve and the fovea was present.

Optical Coherence Tomography Angiography Findings in Optic Nerve Sheath Meningioma.

A 46-year-old female with a history of 5 months of progressive painless visual loss in the left eye was found to have an optic nerve sheath meningioma. Optical coherence tomography angiography showed a reduction in the superficial capillary plexus density consistent with her visual field defect and peripapillary retinal nerve fiber layer thinning. Moreover, abnormalities in the choriocapillaris were found in the affected eye compared to the fellow eye. Possible explanations for these findings are discussed. Further studies and a consistent number of cases are needed to correctly assess the imp

Optical Coherence Tomography for the Detection of Remote Optic Neuritis in Multiple Sclerosis.

BACKGROUND AND PURPOSE: Evidence of remote optic neuritis is often used to support a diagnosis of multiple sclerosis (MS). Optical coherence tomography (OCT) can provide qualitative and quantitative data on the retina, where the effects of optic neuritis can be seen. Our aim was to determine whether there is a quantitative difference in retinal structures in eyes with optic neuritis compared with those without, and which measures were best able to discriminate eyes with a history of optic neuritis from nonoptic neuritis eyes in MS patients. METHODS: We performed a prospective cohort study of

Macular optical coherence tomography in patients with unilateral optic nerve hypoplasia.

PURPOSE: To characterize the extent and location of macular thinning in patients with unilateral optic nerve hypoplasia (ONH) as compared to the contralateral normal eye. METHODS: The medical records of patients with unilateral ONH who underwent spectral domain optical coherence tomography (SD-OCT) of the macula were retrospectively reviewed. SD-OCT scans were manually segmented by 3 observers in 3 macular regions (superior, central, inferior). Boundaries identified included the inner limiting membrane, the junction between the inner nuclear layer and outer plexiform layer, and the neural ret

Quantifying axonal loss after optic neuritis with optical coherence tomography.

OBJECTIVE: To determine to what degree changes in retinal nerve fiber layer (RNFL) thickness after optic neuritis (ON) correlate with either visual recovery or impairment. METHODS: ON can cause visible defects within the RNFL, which can be quantified using optical coherence tomography (OCT). It may be possible to predict visual recovery by measuring RNFL loss after ON. Fifty-four patients underwent repeated evaluations with optical coherence tomography and standardized ophthalmic testing after ON. Regression analyses were used to determine the relationship between RNFL thickness and visual fu

Apical optic nerve compression of dysthyroid optic neuropathy on computed tomography.

Computed assisted tomography (CT) of seven cases of dysthyroid optic neuropathy showed moderate to severe thickening of the extraocular muscles at the orbital apex, white CT of two control cases of dysthyroid orbitopathy without neuropathy showed minimal or fusiform thickening. The consequences include probable apical compression with optic nerve neuropathy either by direct pressure on the nerve or its blood supply. The management of these cases is either by oral administration of corticosteroids in high dosage with gradual tapering or radiation therapy to the orbital apex (approximately 3,000
