This communication is designed to overview the literature publications concerning sore throat and chronic tonsillitis that appeared during the period from 2007 to 2011. It is shown that the prevalence of chronic tonsillar pathology tends to grow progressively over time and is presently 1.5-1.8 times higher than it was in the 1970s. To evaluate prevalence, main causes and social factors of chronic tonsillitis (CT), we examined 321 families 335 children (age 1-15 years) from which had diagnosis of CT. The examination detected CT in the other 114 children from these families. Families of 120 healthy children matched for age served control. Family tonsillitis was detected in 171 (53.3%) families of the study group and 15 (17.5%) families of the control group (p < 0.05). We describe three homosexual men presenting to the ear, nose and throat clinic with severe tonsillitis. All were subsequently diagnosed with secondary syphilis of the tonsil. Syphilis should be considered as a diagnosis in high risk patients presenting with tonsillar lesions, even in the absence of other classical features of the disease.

Acute tonsillitis is a common condition and usually runs a benign course. However life-threatening complications do still occur, even in this postantibiotic era. After oral administration of 300 mg Sobelin, the efficacy and pharmacological kinetics of clindamycin (CAS 18323-44-9) were analysed in 35 patients with recurrent tonsillitis. Minimal inhibitor concentration (MIC) and minimal bactericidal concentration (MBC) have been evaluated for 120 strains. MIC and MBC for 91.7% of the strains amounted to 0.25 micrograms/ml. A total of 193 patients with streptococcal pharyngitis/tonsillitis received 500 mg dirithromycin once daily and 196 patients 250 mg erythromycin four times daily for ten days in a double-blind, parallel-group multicentre study. A quantitative analysis of adenoid and tonsillar specimens from 460 patients was performed. The patients underwent tonsillectomy or adenotonsillectomy for either recurrent tonsillitis or obstructive tonsillar hypertrophy. Histologic preparations revealed Actinomycetes as statistically more prevalent in the obstructive population. The presence of Actinomycetes in the adenotonsillar core of this diagnostic group may indicate an etiologic role for this organism in tonsillar and adenoidal hypertrophy.
