Management of complex extremity injuries: tourniquets, compartment syndrome detection, fasciotomy, and amputation care.

Historically, complex extremity injuries, otherwise known as mangled extremities, have been difficult management problems. This is especially true in multiply-injured patients where many priorities exist and where amputation is considered a failure of limb salvage. Over the past decade, advances in the total management of complex extremity injuries, from the placement of life-saving and limb-saving tourniquets in the prehospital setting to the advancement of prosthetics and rehabilitation months to years later, have resulted in superb functional results regardless of whether limb salvage or am

Injury risks in frontal crashes by delta V and body region with focus on head injuries in low-speed collisions.

PURPOSE: This study investigated injury risks in frontal crashes by belt use and crash severity (delta V) with a focus on studying whether there is a pattern to the crashes causing serious head injuries in low-speed frontal collisions. METHODS: 1996-2007 National Automotive Sample System-Crashworthiness Data System (NASS-CDS) was analyzed for frontal crashes involving front-outboard occupants. Light vehicles were included with model year 1997+. Injuries of maximum severity MAIS 0-6 and fatalities were determined by crash severity and belt. Body region injury (AIS 0-6) was also determined. When and how to image a suspected broken rib.

Radiographic confirmation of rib fracture is often clinically irrelevant in patients who appear to have no complications or associated injuries. However, it is essential in a number of cases, especially when the clinical presentation and history raise suspicion of complications such as organ damage, or if the patient has other risk factors or conditions for which the precise information would help in management decisions. The authors discuss when to order imaging and which imaging test to order.

Solid viscus injury predicts major hollow viscus injury in blunt abdominal trauma.

BACKGROUND: As nonoperative management of blunt abdominal trauma has become more popular, reliable models for predicting the likelihood of concomitant hollow viscus injury in the hemodynamically stable patient with a solid viscus injury are increasingly important. METHODS: The Pennsylvania Trauma Systems Foundation registry was reviewed for the period from January 1992 to December 1995 for all adult (age > 12 years) patients with blunt trauma and an Abbreviated Injury Scale (AIS) score > or = 2 for a solid viscus (kidney, liver, pancreas, spleen). Patients with an initial systolic blood press

Femoral catheters and deep venous thrombosis: a prospective evaluation with venous duplex sonography.

Femoral vein cannulation (FVC) with 8.5F Swan-Ganz catheter introducers allows expeditious intravenous access and rapid volume infusion; however, the incidence of associated iliofemoral deep venous thrombosis (DVT) is unknown. Trauma patients (n = 76) requiring unilateral FVC were prospectively entered into a study where they were resuscitated via FVC and serially evaluated with bilateral venous duplex sonography (VDS). The incidence of DVT in catheterized femoral veins was compared with that in uncatheterized femoral veins. Catheters were removed promptly and VDS was performed within 24 hours

Shock and Trauma Risk Index Nomogram (STRIN): A bedside physiology-based tool for early-high-acuity care triage in major trauma.

BACKGROUND: Major trauma is a leading cause of mortality and permanent disability, especially in younger adults. Early identification of high-risk patients is critical for appropriate care. Tools like the Shock Index (SI) or New Trauma Score (NTS) enable rapid bedside assessment but show variable performance across populations. OBJECTIVE: To develop and internally test the Shock Trauma Risk Index Nomogram (STRIN), integrating Shock Index (SI), Glasgow Coma Scale (GCS), oxygen saturation (SpO₂), and mean arterial pressure (MAP) for early risk stratification of time-critical resource use in maj

Forensic life-threat assessments using trauma scoring in single stabs to the trunk.

INTRODUCTION: Forensic assessments of whether an injury would have been life-threatening often rely on empirical knowledge rather than evidence-based criteria. To address this gap, we applied the validated trauma scoring system New Injury Severity Score (NISS) to estimate mortality risk along the natural course of single stab injuries to the trunk. METHODS AND MATERIALS: We included 408 survivors and 139 fatalities with single stab injuries to the trunk, using data from registries maintained by the Swedish National Board of Forensic Medicine. Organ and vessel damage in survivors, stratified by

Identifying the reciprocal causation between hit-and-run behavior and crash injury severity.

OBJECTIVE: Hit-and-run behavior is believed to exacerbate the injury severity of traffic crashes due to the delayed emergency response for the victims. However, several previous studies indicated the opposite finding that hit-and-run crashes were associated with less severe injuries. The relevant studies mainly identified the statistical associations between hit-and-run behavior and injury severity without revealing causation between them. To this end, the study aims to explore the reciprocal causation between the two variables. METHOD: The two-stage probit model with endogenous regressors is

Peak Power: A Severity Measure for Head Acceleration Events Associated with Suspected Concussions.

OBJECTIVES: In elite rugby union, suspected concussions lead to immediate removal from play for either permanent exclusion or a temporary 12-min assessment as part of the Head Injury Assessment 1 (HIA1) protocol. The study aims to retrospectively identify a head acceleration event (HAE) severity measure associated with HIA1 removals in elite rugby union using instrumented mouthguards (iMGs). METHODS: HAEs were recorded from 215 men and 325 women, with 30 and 28 HIA1 removals from men and women, respectively. Logistical regression was calculated to identify whether peak power, maximum principa

BPBSAM: Body part-specific burn severity assessment model.

BACKGROUND AND OBJECTIVE: Burns are a serious health problem leading to several thousand deaths annually, and despite the growth of science and technology, automated burns diagnosis still remains a major challenge. Researchers have been exploring visual images-based automated approaches for burn diagnosis. Noting that the impact of a burn on a particular body part can be related to the skin thickness factor, we propose a deep convolutional neural network based body part-specific burns severity assessment model (BPBSAM). METHOD: Considering skin anatomy, BPBSAM estimates burn severity using bo

Targeted isometric force impulses in patients with traumatic brain injury reveal delayed motor programming and change of strategy.

The capability of quickly (as soon as possible) producing fast uncorrected and accurate isometric force impulses was examined to assess the motor efficiency of patients with moderate to severe traumatic brain injury (TBI) and good motor recovery at a clinical evaluation. Twenty male right-handed patients with moderate to severe TBI and 24 age-matched healthy male right-handed controls participated in the study. The experimental task required subjects to aim brief and uncorrected isometric force impulses to targets visually presented along with subjects' force displays. Both TBI patients and co

Evaluation of Microvascular Perfusion and Resuscitation after Severe Injury.

Achieving adequate perfusion is a key goal of treatment in severe trauma; however, tissue perfusion has classically been measured by indirect means. Direct visualization of capillary flow has been applied in sepsis, but application of this technology to the trauma population has been limited. The purpose of this investigation was to compare the efficacy of standard indirect measures of perfusion to direct imaging of the sublingual microcirculatory flow during trauma resuscitation. Patients with injury severity scores >15 were serially examined using a handheld sidestream dark-field video micro

Pancreatic trauma.

INTRODUCTION: Pancreatic trauma occurs in approximately 4% of all patients sustaining abdominal injuries. The pancreas has an intimate relationship with the major upper abdominal vessels, and there is significant morbidity and mortality associated with severe pancreatic injury. Immediate resuscitation and investigations are essential to delineate the nature of the injury, and to plan further management. If main pancreatic duct injuries are identified, specialised input from a tertiary hepatopancreaticobiliary (HPB) team is advised. METHODS: A comprehensive online literature search was perform
