Massive calcium pyrophosphate dihydrate crystal deposition disease: a cause of pain of the temporomandibular joint.

Calcium pyrophosphate dihydrate deposition (CPDD) disease is a disorder that occasionally affects the temporomandibular joint (TMJ) and temporal bone, causing pain (66.6% of cases), swelling (50%), trismus (36.8%), and hearing loss (22.2%). Diagnosis of CPDD is challenging because clinical symptoms and imaging features are not characteristic and may mimic a chondrosarcoma. When the diagnosis of CPDD of the TMJ is under consideration, conventional radiographs of the wrist or the knee may contribute to the final diagnosis. Imaging features of CPDD are discussed with a review of the literature.

Self-inflicted traumatic macroglossia.

Macroglossia is a potentially life-threatening problem. The multiple aetiologies responsible for this condition include cysts, tumours, amyloidosis, angioedema, and infections. Although injuries to the oral cavity are common, self-inflicted traumatic macroglossia is not common. We report a case of a comatose patient with limb contractures and severe trismus who developed entrapment of the tongue with progressive macroglossia and near auto-amputation of the tongue. The importance of early recognition and treatment of tongue entrapment in comatose patients is emphasised since this is a potential…

Evaluation of the orbital floor in zygoma fractures.

Displaced fractures of the zygoma require reduction to prevent malunion. The indications for concomitant orbital floor exploration are controversial. In a retrospective review, the preoperative clinical findings, roentgenographic findings, and mode of injury in 106 patients with zygoma fractures who had their orbital floors explored was compared with surgical findings. The finding of preoperative diplopia was correlated with a surgically significant floor defect. The finding of trismus correlated with the absence of a significant defect. No other clinical or roentgenographic finding was correl…

The effects of Kinesio tapes on facial swelling following bimaxillary orthognathic surgery in the supraclavicular region.

BACKGROUND: Several osteotomies are required for orthognathic surgery to reposition the jaws correctly. This study aimed to evaluate whether Kinesiotaping can reduce swelling, pain, and trismus following orthognathic surgery of the facial skull. MATERIALS AND METHODS: The present study consists of two phases. In the split-mouth phase, 16 skeletal class III patients underwent Bimax Orthognathic surgery, and Kinesiological tape (KT) was applied on one half of the face. In the prospective case-control phase, 30 patients were divided into two groups. Kinesio tape was applied on both sides of the …

Actinomycotic osteomyelitis with proliferative periostitis arising in the mandibular ramus: an unusual case with spontaneous bone regeneration after coronoidectomy.

Actinomycotic osteomyelitis is an aggressive and persistent disease capable of invading and destroying bone, and chronic osteomyelitis with proliferative periostitis represents new bone formation with periosteal reaction. We report a rare case of actinomycotic osteomyelitis with proliferative periostitis arising in the mandibular ramus and spontaneous bone regeneration after coronoidectomy. A 14-year-old girl was referred for swelling in the right parotid-masseteric region and severe trismus. Contrast-enhanced CT revealed that heterogenous enhancement of the right masseter muscle, and a reacti…

An Unusual Presentation of Nasopharyngeal Carcinoma as Lemierre Syndrome.

BACKGROUND Clinical presentation of nasopharyngeal carcinoma (NPC) is correlated with the extent of primary and nodal disease. Hence, depending on the anatomical structures affected, the clinical presentation varies accordingly, ranging from non-specific symptoms of epistaxis, unilateral nasal obstruction, and auditory complaints, to cranial nerve palsies. Nodal metastasis in the neck is a frequent clinical finding in nasopharyngeal carcinoma. CASE REPORT A female was admitted to the hospital because of fever and trismus with painful swelling in the right neck. Computed tomography (CT) reveale…

Late complication of cosmetic injectable treatments presenting to oral and maxillofacial surgeons: soft tissue filler granulomas.

Hospital-based clinicians have seen a progressive increase in complications following cosmetic treatments. A challenging late complication of soft tissue filler treatments is granuloma formation. Oral and maxillofacial surgeons have a key role in the discussion around this delayed-onset complication. We present a case of trismus arising from intramasseteric granulomas following soft-tissue filler injections. Collaboration between surgeons and aesthetic medicine clinicians is crucial to preventing and managing complications from treatments which remain largely unregulated.

Dystonic reactions in children caused by metoclopramide.

Five cases of alarming dystonic reactions are reported in children receiving a normal dosage of metoclopramide, a new anti-emetic drug. The symptomatology consisted mainly of neck pain, torticollis, retrocollis, oculogyric crisis, and trismus, and patients were admitted with presumed diagnoses of encephalitis, meningitis, and tetanus. After discontinuing the drug, all symptoms disappeared completely. We believe that metoclopramide should be used with caution in paediatrics and that all doctors should be familiar with its possible bizarre side-effects.

Zygomaticomaxillary Fractures.

Fractures of the zygomaticomaxillary complex and zygomatic arch are common athletic injuries. Fracture displacement can lead to midfacial retrusion and widening, causing noticeable deformity. Associated signs and symptoms include hypoesthesia of the infraorbital nerve distribution, trismus, and subjective malocclusion. Operative treatment is indicated in cases of significant displacement or functional disturbance. The approach and details of osteosynthesis are catered to the specific characteristics of the fracture. Technology, such as virtual surgical planning, intraoperative navigation, and …

Grisel syndrome, acute otitis media, and temporo-mandibular reactive arthritis: A rare association.

We present a case report of a four-year-old boy with torcicollis and trismus after acute otitis media. Grisel Syndrome diagnosis in association with temporo-mandibular reactive arthritis was admitted, leading to early conservative treatment. GS should be suspected in a child presenting with torticollis after an upper respiratory tract infection or an ENT surgical procedure. The association with temporo-mandibular reactive findings is somehow rarer but not impossible, due to the close vascular communication between retropharyngeal and pterigoid spaces.

Radiotherapy and the oral environment the effects of radiotherapy on the hard and soft tissues of the mouth and its management.

Radiation therapy plays an important role in the care of patients with head and neck cancer. When the oral cavity and the salivary glands are exposed to high doses of radiation, there can be dramatic effects on the patient's oral health. The clinical consequences of radiation can include mucositis, hyposalivation, taste loss, osteoradionecrosis, radiation caries and trismus. This paper looks at the available literature regarding the effects of radiotherapy on the oral environment and outlines practical clinical approaches to prevent or reduce the adverse side effects of treatment.
