Asymptomatic occlusive retinal vasculitis in newly diagnosed active tuberculosis.

INTRODUCTION: Worldwide, tuberculosis is the leading cause of death from an infectious disease. Ocular involvement can cause significant and permanent vision loss. Ocular manifestations of tuberculosis often present with visual symptoms. Asymptomatic ocular tuberculosis is uncommon and yet can have serious consequences if missed. CASE REPORT: An immunocompetent 26-year-old Filipino man living in regional Australia who was diagnosed with active pulmonary tuberculosis and started on antitubercular therapy. He was referred to an ophthalmologist for baseline ethambutol screening to exclude pre-ex

Imaging in Tubercular Choroiditis: Current Concepts.

PURPOSE: Ocular tuberculosis has protean clinical manifestations. Because of its varied clinical presentation, multimodal imaging is very important to characterize the disease activity, presence of inflammation, determining therapeutic response, and detection of complications. METHODS: Narrative review. RESULTS: In this review, various imaging modalities employed in the management of ocular tuberculosis including fluorescein angiography (FA), indocyanine green angiography (ICGA), optical coherence tomography (OCT), and optical coherence tomography angiography (OCTA) have been reviewed. Not o

[Pseudotumorous choroid granuloma in miliary tuberculosis].

Recently, tuberculosis re-emerged as a serious public health problem, in industrialized countries, while remaining it remains a major cause of mortality and morbidity in third world countries. Intraocular localization is rare. The most common manifestation of ocular tuberculosis in patients with miliary is Bouchut tubercules. An atypical feature of ocular tuberculosis is discussed: a pseudotumoral choroid granuloma. The behavior of the lesion and its response to antituberculous therapy was documented clinically by fundus photography on fluorescein angiography.

Ocular sarcoidosis and tuberculous lymphadenopathy: coincidence or real association.

Tuberculosis and sarcoidosis share similarity in histopathologic findings and clinically occur in association with each other occasionally. Tuberculosis should always be ruled out before the diagnosis of sarcoidosis. But, the diagnosis is often complicated, especially in extrapulmonary cases. Here we present a case of bilateral vitreous hemorrhage with uveitis. Ocular sarcoidosis was initially diagnosed based on the characteristic ocular findings, negative results on chest radiography, tuberculosis culture, and polymerase chain reaction of aqueous, as well as simultaneous presence of panda and

Sudden monocular vision decrease caused by isolated choroidal tuberculoma during anti-tuberculosis treatment: a case report.

Mycobacterium tuberculosis primarily causes pulmonary diseases; however, it can also affect extrapulmonary organs, including the eyes. The choroid, as the intraocular tissue with the richest blood supply, is one of the most susceptible structures to intraocular tuberculosis. Abnormal ocular symptoms that occur during systemic anti-tuberculosis treatment (ATT) can easily be misdiagnosed as drug side effects or treatment failure. Abruptly discontinuing a specific medication could lead to the recurrence of extraocular tuberculosis and irreversible vision loss. This case study focuses on a young f

Treatment of neuro-ophthalmologic manifestations of tuberculosis.

In the absence of obvious pulmonary or disseminated tuberculosis, ocular and central nervous system (CNS) tuberculosis may represent a significant diagnostic challenge. Refinements in polymerase chain reaction techniques and neuroimaging have strengthened the battery of tests used to diagnose CNS and ocular tuberculosis, yet in many cases, the diagnosis remains one of exclusion; it may ultimately be determined through exacerbation by anti-inflammatory therapy with subsequent improvement by antitubercular medication treatment. Because of emerging drug resistance, at least a two-drug regimen is …

Progressive Necrotizing Ocular Surface Disease, Despite Immunosuppression.

A 60-year-old man was referred with 2 years of left conjunctivitis that persisted despite antibiotic and systemic corticosteroid therapy. The severity and extent of surface disease had increased slowly after prior dacryocystorhinostomy. Subsequent conjunctival biopsies demonstrated granulomas with patchy necrosis but no evidence of malignancy or organisms; systemic screening for sarcoidosis and tuberculosis was negative. A markedly thickened and inflamed left ocular surface was present on referral, with areas of conjunctival ischemia and patchy white slough on the tarsi. Further ocular surface

Risk of tuberculosis transmission by tissue transplantation: a quantitative risk assessment.

Two recent tuberculosis outbreaks in the United States implicated a contaminated bone graft product containing living cells. Evidence-based donor selection criteria may help prevent future tragic events like these. However, the development of such criteria must be informed by estimates of the risk of tuberculosis transmission by tissue transplantation, which are currently lacking. Therefore, we aimed to estimate the risk of tuberculosis transmission through tissue transplantation in Québec, Canada. We developed a deterministic model to assess the risk of tuberculosis transmission through the t
