BACKGROUND: About 40 000 cases of extensively drug-resistant tuberculosis emerge worldwide annually, but the predictors of extensive drug resistance are unclear.
OBJECTIVE: To identify risk factors for extensively drug-resistant tuberculosis and multidrug-resistant but non-extensively drug-resistant tuberculosis in patients with culture-confirmed pulmonary tuberculosis.
DESIGN: Cross-sectional, countrywide study.
SETTING: Estonia, a country with 1 of the world's highest rates of extensively drug-resistant and multidrug-resistant tuberculosis.
PATIENTS: All patients with culture-confirmed p

BACKGROUND: This study aimed to assess the utility of sputum examinations and chest radiographs (CXRs) in patients with extrapulmonary tuberculosis (XPTB) to detect pulmonary involvement of tuberculosis (TB).
METHODS: We studied 72 XPTB patients who were managed through the TB Program, King County, WA, from January 2003 through November 2004.
RESULTS: The two most common sites of XPTB were the lymph nodes (36 [50%]) and pleura (12 [17%]). Thirty-five of 72 XPTB patients (49%) had abnormal CXR findings. Sputum was not obtained from 15 patients despite sputum induction. Of the 57 patients from

OBJECTIVE: To evaluate diagnostic differences and associations between pleural tuberculosis (PLTB), and pulmonary tuberculosis alone (PTB), as well as, pleural and pulmonary co-infection (PLPTB) and PTB.
METHODS: Data from a 6-year, population-based study in Houston, Texas, were analyzed by logistic regression modeling. Study personnel interviewed 2226 tuberculosis cases for TB risk assessment and reviewed medical records. Mycobacterium tuberculosis isolates from available culture-positive patients underwent genotyping.
RESULTS: Among the categories analyzed [PTB, n=2070; PLTB, n=95; and PLP

The purpose of this study was to demonstrate the computed tomography (CT) features of pulmonary tuberculosis in patients with acute clinical manifestations. The emergent CT images of 46 non-AIDS patients with acute clinical presentations of pulmonary tuberculosis (TB) were analyzed. The distribution and characterization of the pulmonary and pleural lesions were specifically focused. Multifocal airspace consolidation in multiple lobes was most commonly seen on the CT in 32 of 46 patients (70%), followed by cavitary pulmonary nodular shadows (50%). Other CT manifestations included pleural effusi

The X-ray pattern of disseminated pulmonary tuberculosis is considered in terms of examination findings in 98 patients. Under the present-day conditions, the X-ray pattern of disseminated tuberculosis is more commonly characterized by a combination of focal shadows of varying diameters with a prevalence of foci of this or that size. Symmetrical areas of infiltration (14.3%) and relatively symmetrical systems of decay cavities (6%) were a major abnormal shadow. A total of 76% had a destructive process. There was a predominance of generalized processes with medium and large cavities.

A patient given intravesical BCG immunotherapy developed miliary pulmonary tuberculosis. After resection of a superficial bladder tumor, the patient was given weekly intravesical BCG infusions. After the 4th session, the patient developed fever (40 degrees C), shivers, dry cough, profuse sweating, and weight loss. Initially, the chest x-ray was normal. The patient was given isoniazid (5 mg/kg) in a single-drug regimen. Rapid degradation of the general status led to a new chest x-ray, 10 days later, which demonstrated a reticulonodular syndrome. High-resolution thoracic CT confirmed the diagnos

Adenosine deaminase (ADA) activity has been helpful for the diagnosis of tuberculous pleurisy. However, there are few studies about the role of ADA in the diagnosis and follow-up of pulmonary tuberculosis. In our study, serum ADA activity was determined in order to investigate the role of the enzyme in the diagnosis of pulmonary tuberculosis and monitoring the efficiency of therapy. The ADA activity was (mean +/- SD) 21.77 +/- 8.51 U/L in pulmonary tuberculosis patients (n= 44), 6.24 +/- 3.25 U/L in old tuberculosis patients (n= 24), 8.58 +/- 4.38 U/L in healthy control subjects (n= 20), where

Six patients with extra-pulmonary tuberculosis who presented within a 2-year period to the Paediatric Department of the Faculty of Medicine, Galle, Sri Lanka, are presented. In spite of the BCG vaccination being given to all of them at birth, five patients subsequently developed miliary tuberculosis and one patient bone tuberculosis. The likely reasons for failure of BCG vaccination to prevent extra-pulmonary complications developing are discussed.

We studied 40 patients with pulmonary tuberculosis. All were positive for acid-fast bacilli (AFB) in the sputum. Their mean age was 30 yrs (range 10-50 yrs) and the duration of illness was 26.3 +/- 2.3 months. Radiologically minimal, moderately advanced and far advanced lesions were present in 7 (17.5%), 9 (22.5%) and 23 (57.5%) patients respectively. One patient with endobronchial lesion had no radiological evidence of pulmonary tuberculosis. Clinically, 14 patients (35%) had one or the other features of adrenocortical insufficiency. Postural hypotension was the commonest feature and was pres

A 68-yr-old man was admitted to the hospital with severe Legionnaires' disease, which was serologically confirmed by a 6-fold increase in antibody titers to Legionella pneumophila, serogroup 1. The patient improved dramatically with intravenously administered erythromycin therapy but was discharged from the hospital with residual debility and pulmonary complaints later recognized as being caused by active pulmonary tuberculosis. The diagnostic and therapeutic confusion engendered by the concurrence of these pulmonary infections is discussed.
