The high incidence of peptic ulcers and abdominal pain from other causes in patients with primary hyperparathyroidism has been recognized for many years. This paper reports 20 patients with hyperparathyroidism: 12 with peptic ulcers and 8 with abdominal pain from other causes. Only in a minority of these was the diagnosis of hypercalcaemia made as a result of a deliberately sought assay; in most the plasma calcium had been measured as part of a 'screening' procedure with a multichannel analyser. In almost all of the patients who had parathyroid surgery the abdominal symptoms were completely re…

Drug damage to the stomach provides a model for study of the development of peptic ulcer, gastritis, and duodenitis in man. Aspirin damage is the best understood. Pathophysiologically, aspirin alters the gastric mucosal barrier to hydrogen ions and lowers gastric potential difference. Ultrastructurally, aspirin damage to surface epithelial cells leads to microerosions. Macroscopically, acute hemorrhage and erosions are seen in both the stomach and duodenum by endoscopy after ingestion of a large dose of aspirin. Patients with chronic rheumatic diseases taking aspirin have a 50% incidence of ga…

Differences in distribution of age, hemoglobin values, location and number of ulcers, activities of gastric secretion, associated conditions, treatment and prognoses between bleeding and non-bleeding gastric ulcer patients were studied. The ages of bleeding patients were distributed more widely than those of non-bleeding patients and the number of patients over 70 years of age was greater in bleeding patients than in non-bleeding patients. Associated conditions of bleeding patients were more serious than those of non-bleeding patients and some bleeding patients showed coagulation disorders. Fi…

Ulcer disease is the product of different pathogenetic mechanisms which destroy the balance between protection and aggregation in the human organism. Investigation of such errors of physiologic balance in the human organism is in full swing, and has provided the theoretical elements for appropriate therapy. Therapy for ulcer disease must take into consideration the patient's personality as well as the surrounding conditions. Though duodenal and gastric ulcer show many different characteristics, therapy does not greatly differ. Food and antacids neutralize hydrochloric acid already produced and…

This very rare affection of unknown origin, achieves a macroscopic and microscopic aspect similar to that described by Cruveillhier at the level of the stomach. The authors present a case of ulcer located on the transverse colon, at the free margin, with stenosing evolution and coexisting with duodenal ulceration of chronic character. The patient also had portal hypertension in the third stage and arteriopathy that had been diagnosed previously.

In 1971 to 1977, 34 patients were operated for gastric ulcer at Zaria in the Savannah region of Northern Nigeria. The most frequent operation (35%) was for perforation. The frequency of perforation is similar to Ibadan's but smaller than Dakar's. Compared with Ibadan and Burundi, there are fewer women, more Type I ulcers and more frequent haemorrhages. There were three giant lesser curvature ulcers. Only a few combined duodenal and gastric ulcers were seen suggesting that gastric stasis is not the immediate factor in the production of gastric ulcer in this region.

In a 42-year-old man with bilateral severe Mooren's ulcers, results of immunologic studies were normal, with the exception of an in vitro blastogenic response of the patient's lymphocytes to corneal stroma. The histologic studies revealed vast numbers of vacuolated neutrophils with intracytoplasmic phagosomes in and about the ulcerating cornea. Although the progression of ulceration was initially unaltered by surgical and pharmacologic trials, systemic immunosuppression with methotrexate may have prevented ulceration of the lamellar tectonic graft.

Seven patients with the clinical diagnosis of Mooren's ulcer underwent macrophage migration inhibition factor testing for evidence of cellular immunity to corneal antigen. Six of seven patients demonstrated positive macrophage migration inhibition in response to corneal antigen, thus suggesting that cell-mediated immunity may play a role in the cause of Mooren's ulcer. One of these patients had a negative macrophage migration inhibition factor when the disease was inactive, but developed a positive response in association with a homograft rejection episode.
