The pressure of the ulnar nerve at the level of the elbow is a pathological situation which causes serious problems of use of the suffering upper limb to its patient. In this study a reference is made to the anatomy of the ulnar nerve, to the reasons and to the main anatomic points of pressure of the ulnar nerve on the elbow, to the clinical indications and to the subjective symptoms of the patient. The state of art according to the diagnosis and methods of treatment is reported. The Authors particularly outline on the surgical technique used in order to solve this particular problem and to th

This paper describes four cases of ulnar tunnel compression syndrome caused by the accessory abductor digiti minimi muscle and stresses on the observation that wrist trauma could be an important precipitating factor. Possible pathophysiological explanations of this phenomenon are offered and the anatomy of the accessory abductor digiti minimi muscle is reviewed.

Cubital tunnel syndrome (CuTS) is the 2nd most common compressive neuropathy. To improve both diagnosis and the selection of patients for surgery, there is a pressing need to develop a reliable and objective test of ulnar nerve 'health'. Diffusion tensor imaging (DTI) characterises tissue microstructure and may identify differences in the normal ulnar from those affected by CuTS. The aim of this study was to compare the DTI metrics from the ulnar nerves of healthy (asymptomatic) adults and patients with CuTS awaiting surgery. DTI was acquired at 3.0 T using single-shot echo-planar imaging (55

PURPOSES: (1) To revisit the anatomical boundaries of the canal, its contents and its two channels, (2) to describe the anatomical variations of the canal's borders and the variations of its contents, and (3) to discuss the clinical relevance of the Guyon's canal syndrome.
METHODS: Two hundred and fifty MR wrists examinations were reviewed. MR spin echo T1-weighted axial slices were used to analyze the Guyon's canal. The anatomical boundaries, the cross-sectional area and length of the canal were calculated. The anatomical variations of the canal's walls and contents and their prevalence were

Ulnar nerve entrapment is one of the most common entrapment neuropathies in the upper limb. The most frequent location of this syndrome is behind the elbow. The clinical picture is associated with the localization of the entrapment but usually consists of an altered sensation at the fourth and fifth digits and a weakness of the intrinsic muscles of the palm. The most constructive tool in making the diagnosis and in assessing the treatment's efficacy is the physical examination. Treatment alternatives depend on entrapment location. Conservative treatment options such as rest, a change in the wo

Published correlations between electrophysiological and surgical findings are relatively rare in cases of ulnar nerve compression at the wrist, compared to the more common compression of the ulnar nerve at the elbow. We describe a patient who presented with clinical and electrodiagnostic findings of a pure motor ulnar neuropathy involving the territory of the deep branch. Surgical exploration revealed that a ganglion cyst caused compression of the deep ulnar motor branch at Guyon's canal. This case illustrates the usefulness of electrodiagnostic studies in the localization of nerve entrapment

Heterotopic ossification after head injury may occur in the elbow joint. Rarely does this lead to entrapment of the ulnar nerve. We describe the case of a 20-year-old patient who developed heterotopic ossification 6 weeks after a traumatic brain injury. She subsequently developed bilateral ulnar nerve palsy which was confirmed by electrodiagnostic studies and treated by transposition of the ulnar nerve.

Ulnar nerve entrapment at the wrist (UNW) is uncommon and often difficult to localize electrophysiologically. The difference between the motor latencies to the median-innervated second lumbrical (2L) and ulnar-innervated palmar interosseous (Pl) (Diff 2L-Pl) has been shown to be of localizing value in patients with median neuropathy at the wrist. In the last year, we evaluated 2 patients with clinically definite ulnar neuropathy at the wrist. We performed motor studies to the 2L-Pl on the 2 patients and 12 disease controls with ulnar neuropathy at the elbow as follows: Using the same electrode

Some of the experimental, pathologic, and clinical features of extremity compression are reviewed with special reference to vulnerability of peripheral nerves to compressive forces at specific anatomic locations. Attention is directed toward syndromes resulting from mechanical factors, and it should be pointed out that a nerve can be compressed at more than one level. Localized peripheral nerve compression is common in clinical medicine; carpal tunnel syndrome is the most common example. Physicians who maintain a high index of suspicion to the signs and symptoms of peripheral nerve compression

We report herein a 35-year-old man who suffered from pain at his left elbow and numbness in his left hand. Electromyographic studies demonstrated a localized nerve conduction block in the left elbow region. Ultrasonographic evaluation revealed enlargement of the ulnar nerve at the level of the medial epicondyle as well as bilateral anconeus epitrochlearis muscles, one of which was hypertrophic, causing the ulnar neuropathy at the symptomatic site. We emphasize with this case report the complementary role of ultrasound in peripheral nerve pathologies, as it confirmed the entrapment and determin

Muscle anomalies are common in the upper extremity but are usually only discovered at cadaveric dissection. Anomalous muscles associated with compression neuropathies are reviewed. The clinical significance of anomalies is recognition of the associated compression patterns that occur. Surgical excision is employed as part of standard decompressive procedures. Four illustrative cases of anomalies discovered at surgical exploration are presented.

A patient is presented who had recurrent carpal tunnel syndrome symptoms in his left hand 1 year after having undergone release of the transverse carpal ligament. On exploration, this was found to be due to an aneurysm of a median artery and possibly scarring due to this compression around the branches of the bifurcated median nerve. This represents the first case in the literature to comment on (1) the absence of bilaterality of the anatomic finding and (2) carpal tunnel syndrome relative to median artery aneurysm. With this in mind, a plea is made for careful exploration of the carpal tunnel

Guyon's canal, a fibro-osseous anatomical tunnel, sometimes harbors underlying pathologies such as a ganglion cyst, leading to symptoms including dysesthesia, paresthesia, and motor weakness in the ulnar nerve distribution. Surgical excision is often the recommended approach to restore normal hand function. Ulnar nerve palsy caused by mechanical compression can benefit from surgical decompression; however, a timely diagnosis is often difficult. In this report, we describe the successful tumor excision and nerve decompression of a ganglion cyst in Guyon's canal. The observed intraoperative find

Elbow dislocations associated with a medial epicondyle fracture and ulnar nerve palsy are uncommon injuries. We present the case of an 11-year-old girl with an elbow dislocation treated by closed manual reduction. The medial epicondyle fracture was missed initially until she developed an ulnar nerve palsy 2 months later. Intraoperatively we discovered the bony epicondyle piercing the joint capsule and compressing the ulnar nerve. Removal of the bony fragment relieved her symptoms and she returned to normal activities at 1 year follow-up. We would like to highlight this rare occurrence and pres

Spasmodic torticollis is thought to be a central nervous system disorder with no peripheral nervous system component. Several consecutive patients with spastic torticollis were found to have ulnar nerve compromise at the elbow on the side ipsilateral to the direction of chin turning. This article describes three such patients in detail. It is postulated that the cause is the stereotypical posture assumed by these patients in using their hand to maintain a neutral head position. The chronic leaning and elbow flexion place the ulnar nerve at risk for the development of pressure neuropathy.

Cubital tunnel syndrome, the second-most common peripheral compression neuropathy, is associated with dynamic pressure in the cubital tunnel with the elbow flexion position. Medial elbow ganglion originated from the ulnohumeral joint causing cubital tunnel syndrome has been reported. This report describes the case of a 48-year-old man who developed numbness in his left ring and little finger after prolonged motorbike touring. He gradually showed decreased hand grip strength with medial elbow joint pain. Ultrasonography and magnetic resonance images revealed small occult ganglion at the medial
