Typical ultrasonic images of normal testes, hydrocele and cysts of the epididymis, tumors and inflammatory diseases of the testis and epididymis, and injuries of the scrotal organs obtained in the study of 137 patients are presented. The method is simple, harmless and informative. Grey scale scanning was carried out using a water bath, which gives more complete data on the structure of the testis and its epididymis as compared to routine B-mode scanning. Ultrasonography is a reliable aid to the urologist in the diagnosis of scrotal swellings.

Transducer side lobes consist of multiple low-intensity sound beams located outside of the main ultrasound beam. Although the echoes generated by side lobes originate outside of the main beam, they appear as specular or diffuse artifactual echoes within the beam. They are visible with both static and real-time equipment. Thickening of the bladder wall and dilatation of the prostatic urethra can be shown by ultrasound in infants with posterior urethral valves. This can be helpful in the diagnosis of hydroureter. Marked hypertrophy of the bladder muscle can appear as solid echogenic mass and simulate a pelvic tumor. Ultrasonic findings in 5 infants with posterior urethral valves are reviewed.

Forty-six consecutive studies of carotid bifurcations were done with a Hokanson Ultrasonic Arteriograph before contrast arteriography. Based on the Doppler signal and the oscilloscopic image, each internal carotid artery was sorted into one of four categories as an estimation of the percent diameter reduction: less than or equal to 10 percent, 10 percent to 49 percent, 50 percent to 99 percent, and total occlusion. The skin thickness in a group of patients with scleredema was measured using ultrasonic techniques and then compared with a group of normal individuals. The mean dermal thickness in scleredema was 7.7 mm, compared to 3.3 mm in the normal control group. Ultrasonic evaluation of skin thickness may be of value in following the natural history or response to therapy of certain diseases that affect dermal thickness.

This study presents the results of abdominal ultrasonic scanning in 108 patients attending a tropical referral hospital. Clinical diagnoses included hepatocellular carcinoma, metastatic liver disease, amoebic liver abscess, hydatid disease, obstructive jaundice, hepatosplenomegaly of uncertain aetiology and renal cysts and tumours. Blunt abdominal trauma comprises a major portion of surgical emergencies. These patients require imaging modalities that are rapid and accurate. Four cases of intramural exogastric lesions studied by ultrasound are presented. In all four patients, a large mass was identified in the left mid- and upper abdomen with a strip of solid tissue extending toward the left hemidiaphragm anteriorly, adjacent to the left lobe of the liver. This finding, best demonstrated on longitudinal scanning, suggested that the mass originated in the stomach rather than in the spleen or splenic flexure of the colon, which are posterior in location. Two of the lesions were leiomyoblastomas and two leiomyosarcomas.

Gartner's duct cysts will usually be incidental findings during pelvic sonography. Of developmental origin, they may present anywhere along the lateral aspect of the female genital tract. When the cysts are of parovarian origin, they will mimic other fluid-filled adnexal masses, and no specific diagnosis can be made. When alongside the vagina or cervix, however, their ultrasonographic appearance is probably characteristic.

During the last five years we have applied diagnostic ultrasound to more than 350 girls under the age of 15 for suspected obstetric and gynecologic disorders. More than 60% of these patients were pregnant and were studied before elective abortion. Many other children were studied for amenorrhea or for the evaluation of a pelvic mass. Ultrasound scans are presented in 2 cases of bilateral congenital hydroceles. In both cases, large scrotal sacs were identified and seen to contain prominent fluid collections. In one case, the testes were identified within the fluid. No testes were seen in the other patient, who subsequently proved to have bilateral undescended testicles.

The unique ability of ultrasound to detect soft tissue interfaces makes it a prime tool for at least one major diagnostic approach to soft tissue lesions. Two patients are reported with obstructive jaundice in pregnancy. In one of these ultrasonic scanning confirmed a diagnosis of choledochal cyst, and in the other made this diagnosis. Ultrasonic scanning is suggested as the preliminary radiological examination for obstructive jaundice in pregnancy. Any patient with a clinical diagnosis of choledochal cysts should have an ultrasonic scan.

Twenty patients with pelvic abscesses diagnosed either at surgery or from typical clinical findings underwent sonography. Seventeen of the twenty patients had inflammatory processes of gynecological origin. In the main, two patterns were observed. The more common pattern was a small, round adnexal or cul de sac mass with a slightly irregular border. Ultrasonic examination of the pancreas is rendered difficult by the echogenic characteristics of the organ, by its depth, by the overlying gas, and by bony structures and anatomic variations. The reintroduction of gray-scale imaging promises to simplify the technique and expand its usefulness. The 75Se-/-selenomethionine scan is a reliable test when performed after effective pancreatic stimulation with a scintillation camera that permits the angulation required to separate pancreas from liver. Gallium-67-citrate may be important for both mapping inflammatory processes and imaging some tumors.
