Impact of Alternative Payment Models on Quality of Care in Federally Qualified Health Centers From 2014 to 2024: A 2-stage Difference-in-Differences Analysis.

BACKGROUND: Federally Qualified Health Centers (FQHCs) are essential safety-net providers, but traditional payment mechanisms may focus on volume over quality-of-care. Alternative payment models (APMs) have been introduced to enhance value-based purchasing. It is hypothesized that APMs would improve flexibility and incentivize quality of care, but evidence remains limited, especially for maternal and child health and cardiovascular conditions. OBJECTIVES: We aim to evaluate the impact of APMs on quality-of-care in FQHCs in the United States. METHODS: We used a 2-stage difference-in-differenc

Care Offered by an Information-Rich Pediatric Acute Illness Connected Care Model.

BACKGROUND AND OBJECTIVES: Prevailing regulatory and financing issues constrain dissemination of connected care despite evidence supporting acceptability, effectiveness, and efficiency. In this analysis we describe care provided over a 12-year period by Health-e-Access, an evidence-based, information-rich, connected care model designed to serve children with acute illness. We demonstrate the broad clinical capacity of this care model and key components imparting this capacity. MATERIALS AND METHODS: Since 2001, Health-e-Access has been used in childcare, elementary schools, neighborhood after

Psychometric Evaluation of the EVCA in Digital Health Settings Using Rasch Analysis Among Patients with Chronic Diseases.

With the expansion of internet-based health care, value co-creation behaviors of patients with chronic diseases have become increasingly important. However, appropriate instruments for assessing such behaviors among community-dwelling patients in digital health contexts remain limited. This study aimed to psychometrically evaluate the effort in value cocreation activities (EVCA) questionnaire using the Rasch model to establish a valid and reliable measure of patient engagement in value cocreation in digital health. A Chinese version of the EVCA questionnaire, developed following standard proce

Development and validation of a 5-year mortality prediction model using regularized regression and Medicare data.

PURPOSE: De-implementation of low-value services among patients with limited life expectancy is challenging. Robust mortality prediction models using routinely collected health care data can enhance health care stakeholders' ability to identify populations with limited life expectancy. We developed and validated a claims-based prediction model for 5-year mortality using regularized regression methods. METHODS: Medicare beneficiaries age 66 or older with an office visit and at least 12 months of pre-visit continuous Medicare A/B enrollment were identified in 2008. Five-year mortality was asses

Evaluation of the HEDIS measure of behavioral health care quality. Health Plan Employer Data and Information Set.

OBJECTIVE: The Health Plan Employer Data and Information Set (HEDIS) is the most widely used "report card" system comparing health care plans across different dimensions of performance. HEDIS uses only one measure of the quality of behavioral health care-the rate of follow-up after hospitalization for major affective disorder. This study used data from a national Veterans Affairs database to evaluate the generalizability of the HEDIS behavioral health quality measure. METHODS: Using administrative data from a nationwide sample of 114 VA hospitals, the HEDIS (version 2.5) quality measure was c

Shared Decision-Making and Cardiovascular Health: A Scientific Statement From the American Heart Association.

Shared decision-making is increasingly embraced in health care and recommended in cardiovascular guidelines. Patient involvement in health care decisions, patient-clinician communication, and models of patient-centered care are critical to improve health outcomes and to promote equity, but formal models and evaluation in cardiovascular care are nascent. Shared decision-making promotes equity by involving clinicians and patients, sharing the best available evidence, and recognizing the needs, values, and experiences of individuals and their families when faced with the task of making decisions.

An extraction-free one-pot assay for rapid detection of Klebsiella pneumoniae by combining RPA and CRISPR/Cas12a.

Klebsiella pneumoniae poses a significant threat to global public health. Traditional clinical diagnostic methods, such as bacterial culture and microscopic identification, are not suitable for point-of-care testing. In response, based on the suboptimal protospacer adjacent motifs, this study develops an extraction-free one-pot assay, named EXORCA (EXtraction-free One-pot RPA-CRISPR/Cas12a assay), designed for the immediate, sensitive and efficient detection of K. pneumoniae. The EXORCA assay can be completed within approximately 30 min at a constant temperature and allows for the visualizatio

Father's involvement in child feeding and associated factors among fathers having children aged 6-24 months in Jinka town, Ethiopia, 2024.

Child feeding practices are crucial for child health, growth, and development. Fathers have a vital role in optimal child-feeding practices, while traditional cultural norms dictate that men are responsible for family finances and leadership, and women are responsible for child care. Hence, this study aimed to assess father involvement in child feeding and associated factors among fathers having children aged 6-24 months in Jinka Town. A community-based cross-sectional study was conducted in Jinka town from January 1 to February 15, 2024, on 415 randomly selected study participants. Pre-tested

Values Considerations in Telemedicine: Pause Before Shifting.

As a contingency standard of care, telemedicine use surged during the COVID-19 pandemic. The Medicare telehealth flexibilities introduced during the COVID-19 pandemic expired in September 2025. Any ongoing sustained pivot to telemedicine warrants purposeful attentiveness to ethical considerations and not just technology use as an end unto itself. Telemedicine has the potential to complement face-to-face care practices and enhance clinical interactions when its use is based on shared values. Values such as access, equity, justice, compassion, autonomy, and dignity warrant thoughtful use of tele

The Value of Social Determinants of Health Needs Assessments in Primary Care.

Social determinants of health (SDOH) such as income, housing, food security, education, transportation, and healthcare access, exert a far greater influence on health outcomes compared with traditional clinical care alone. This policy commentary argues for the routine integration of SDOH needs assessments into primary care to enable more comprehensive, equitable, and effective patient care. By screening for social drivers, providers can identify non-medical obstacles to treatment adherence, connect patients with community resources, and develop realistic care plans tailored to individual circu

Patient-Level Modeling Approach Using Discrete-Event Simulation: A Cost-Effectiveness Study of Current Treatment Guidelines for Women with Postmenopausal Osteoporosis.

Decision tree and Markov models have been the most commonly used modeling methods in health economic evaluations. Both methods are known for their limitations. Discrete-event simulation (DES), an event-driven model in continuous time at the patient level, is a relatively new method in health economic evaluations that addresses some limitations of the common modeling techniques. Specifically, with the advent of personalized medicine, conventional methods for value assessment that are based on population-level measures might not be appropriate. The best treatment would depend on patient characte
