Optic disc vasculitis is an idiopathic, usually benign condition in young healthy adults. Hayreh suggested that the condition is to be divided into two types: type 1, with optic disc edema and a benign course as the dominant features and type 2, a condition resembling a central retinal vein (CRV) occlusion with a variable outcome depending on the extent and the site of the lesion. Differential diagnosis with other causes of one-sided papilledema (type 1) and other vascular conditions (type 2) is very difficult but important. We suggest performing extensive ophthalmological and neurological exa…

Although uncommon, a wide variety of ocular manifestations can be seen in polyarteritis nodosa. These occur as a result of the arteritis or secondary to the associated renal induced hypertension. A case of biopsy documented polyarteritis nodosa is reported in which the patient presented with bilateral iritis, vitritis, and a retinal vasculitis involving both the retinal arteries and veins, a feature not described previously. Patients with this potentially fatal disorder may initially present with ocular involvement; thus ophthalmologists should be familiar with the clinical features of this di…

Fifty-two plaque or lesion areas were examined from 25 cases of multiple sclerosis. Twenty-four of these showed acute features, whereas the rest were more chronic in nature. The acute lesions showed lymphocytic infiltration (79%), fibrinous exudation (63%), lymphocytic meningitis (50%) and venulitis (58%). Of the chronic lesions, there were only 21% with lymphocytic infiltration, 11% with fibrinous exudates, none with meningitis, 29% with organising endovenulitis, 36% with fibrosed vein walls. The finding of a fibrinous inflammatory exudate in the acute lesion is a new observation in multiple

Cutaneous vasculitis may present as an urticaria-like eruption in association with viral illness, systemic lupus erythematosus, Sjögren's syndrome, or serum sickness. Often, however, urticarial vasculitis presents as an immune complex-mediated disease without other associated connective-tissue disease or associated infection. Many forms of therapy have been tried, including antihistamines, prednisone, and immunosuppressive drugs without success in every patient. Because colchicine had been effective in some patients with non-urticarial cutaneous vasculitis, we decided to use it in two patients…

Leg ulcerations associated with livedoid vasculitis may undergo a benign or very serious course depending on the presence of underlying disease. The disease has fairly distinctive clinical and histopathologic features, and it should be considered in cases of ulcerations refractory to conventional treatment. Most of these cases may eventually heal after a long course of intermittent eruptions and ulcerations. However, in rare instances, the loss of limbs may be unavoidable.

A 48-year-old woman with the classical features of longstanding rheumatoid arthritis acutely developed a diffuse cerebral disease. Angiograms showed extensive cerebral vasculitis, and CT scan demonstrated cerebral oedema and haemorrhage. Her clinical state, EEG and CT scan rapidly returned to normal following treatment with corticosteroids.

Systemic vasculitis is known to affect the gastrointestinal tract but the nature of the complication is poorly characterized. Out of 65 patients with systemic vasculitis, the majority of whom had renal disease, the intestine was found to be affected in 18. These comprised four of eight patients with polyarteritis nodosa, nine of seventeen with microscopic polyarteritis, four of thirty-six with Wegener's granulomatosis and one of four with Churg-Strauss syndrome. The features included abdominal pain (85 per cent), diarrhoea (50 per cent), gut haemorrhage (44 per cent) and abnormal liver functio…

A 38 year old female, with chronic uncontrolled coeliac disease, presented with the rare complication of cutaneous leucocytoclastic vasculitis. Detailed study failed to identify any cause for the vasculitis, other than the underlying coeliac disease. Haematuria and proteinuria with mesangial nephritis were also demonstrated on renal biopsy with electron microscopic study. It is speculated that exogenous or endogenous antigens permeated the abnormal small bowel mucosa leading to formation of circulating immune complexes. Subsequent tissue deposition of these complexes then resulted in vasculiti…

Plasma exchange (PE) was used to treat 9 patients with rheumatoid vasculitis. The immediate development of new lesions was arrested in 7 patients. Two patients continued to develop new lesions. One responded to additional cyclophosphamide plus methyl prednisolone but the other died within days from severe systemic vasculitis. Prolonged PE alone, or PE followed by cyclophosphamide plus methyl prednisolone was associated with healing of existing lesions without relapse. Short term PE, alone or followed by azathioprine, failed to heal existing lesions and relapses occurred within 3 months. IgG rh…

Vasculitis is an uncommon, extremely debilitating complication of severe, chronic rheumatoid arthritis. High levels of circulating immune complexes along with vascular deposition of immunoglobulins and complement indicate an immune complex mediated disease. No pharmacologic therapy has been completely effective and the incidence of mortality is exceptionally high. This study shows clinical improvement with combination of plasma exchange and cytoxan therapy. We report an association in the conspicuous decline of immune complexes with objective clinical remission in 2 patients who underwent comb…
