Clinical characteristics of probable vestibular migraine.

BackgroundThe International Classification of Vestibular Disorders defines probable vestibular migraine (PVM) as an episodic vestibular disorder with either a history of migraine (criterion B) or migraine features during vestibular episodes (criterion C).ObjectiveThe purpose of this study is to investigate the clinical characteristics of PVM, focusing on items in the diagnostic criteria.MethodsWe retrospectively reviewed the medical records of 30 consecutive PVM patients diagnosed according to the Barany classification. The PVM patients were divided into two groups: those who fulfilled the cri

cVEMP and VAT for the diagnosis of vestibular migraine.

BACKGROUND: Although the diagnostic criteria of vestibular migraine (VM) have already been defined, various clinical manifestations of VM and the lack of pathognomonic biomarker result in high rate of misdiagnosis and mismanagement. A timely and accurate diagnosis tool for the evaluation of VM is highly needed. OBJECTIVE: The current study aims to investigate the potential feasibility of cervical vestibular evoked myogenic potential (cVEMP) and vestibular autorotation test (VAT) as a diagnosis tool for VM. METHODS: A total of 211 subjects were recruited into the current study with all subjec

[Dizziness from a neuro-otological viewpoint].

The symptom of vertigo can be due to many different causes. Differential diagnosis will be discussed primarily from a neuro-otologic point of view. Vertigo can be thought of as a subjective disturbance of the integration of different sensory inputs. The history and subjective characterisation of vertigo often provide enough information for initial differential diagnosis and recommendation for a specific evaluation. The evaluation includes simple tests of posture and gait, tests of ocular motility, and examination of nystagmus. Instability and nystagmus towards a specific direction point to a v

[Effect of inner ear immune response on vestibular function in guinea pigs].

We examined vestibular dysfunction and histological damage caused by direct antigen challenge to the endolymphatic sac in guinea pigs. We observed spontaneous nystagmus every eight hours and performed caloric testing every week following endolymphatic sac secondary KLH challenge. Spontaneous nystagmus was seen in 12 of 18 animals, and nystagmus in all directed toward the unchallenged ear (paralytic). The caloric response time courses were classified into two types, which were irreversible type and reversible type after endolymphatic sac KLH challenge. The immune injury of animals with irrevers

Clinical application of dynamic posturography for evaluating sensory integration and vestibular dysfunction.

The assessment of sensory organization in balance control by dynamic posturography is unique compared with clinical maneuvers and other vestibular or neurologic testing. It was designed to isolate the relative contributions of visual, somatosensory, and vestibular feedback, and thus to assess the overall function of the balance control system in adaptively selecting between available sensory information for orientation cues to serve as a reference to earth vertical. Rather than serving as a test for specific pathologic conditions, the utility of posturography is in screening for balance disord

[Diagnostic and therapeutic problems in dissociated forms of Menière's disease].

The authors report the cases of 5 subjects who presented with incapacitating vertigo which was attributed to a dissociated form of Meniere's disease. They stress the difficulty, in these circumstances, in making a precise diagnosis which can only in fact be confirmed with the benefit of a longer follow up. They stress the value of systematically combining frequency analysis with measurement of maximal slow phase velocity during vestibular testing. This allowed better precision in terms of topographical diagnosis. All subjects underwent vestibular neurotomy via the retro-sigmoid approach. The p

[Vestibular neuronitis and its differential diagnosis (author's transl)].

The study gives a survey of the different terminologies, opinions regarding etiology, and differential diagnoses. In accordance with animal experiments, and our own experience with 71 patients, an activating therapy, including exercises, is recommended instead of bed-rest. Under special circumstances diagnosis may be difficult. For instance, a centrally reduced thermal reactivity of the healthy labyrinth may mimic a bilateral canal paresis. The vestibular type of Menière's disease, disseminated encephalomyelitis, herpes zoster oticus, or the complication of a chronic otitis media cannot always

Usher syndrome: an otoneurologic study.

Usher syndrome is an autosomal recessive disorder characterized by severe hearing loss or deafness and retinitis pigmentosa. Eleven families with 25 affected members were studied. The test battery included genetic studies, clinical examination, audiological, ophthalmologic, and otoneurological tests, and magnetic resonance imaging. Sixteen affected persons had profound hearing loss or were considered anacusic, with absent bilateral vestibular responses. These patients had varying degrees of retinitis pigmentosa. These 16 patients were considered to have type I Usher syndrome. Nine persons were

Neurological and vestibular findings in three cases of Multiple Sclerosis.

Multiple sclerosis (MS) is a chronic and inflammatory autoimmune disease that affects the central nervous system (CNS). Dysfunction of body balance is also a common symptom and may be related to neurological injuries resulting from this disease. The aim of this study was to characterize the neurological and vestibular findings of three clinical cases diagnosed with MS. Data on the neurological evaluation and the magnetic resonance imaging of the skull were collected from the medical records. The patients responded to an initial interview and underwent clinical assessment of body balance and Vi

Audio-vestibular findings in patients with vestibular neuritis.

19 patients with the clinical diagnosis vestibular neuritis, i.e., cases with unilaterally ENG verified abolished or reduced caloric reactions but without symptoms of central nervous involvement or hearing impairment, participated in the study. In addition to ENG examinations and Békésy audiometry all patients were submitted to stapedius reflex tests and auditory brainstem response examinations (ABR) for signs of subclinical auditory nerve engagement. In all but one of the 19 patients the caloric reactions improved; 8 of the 13 patients available for tests after one year recovered completely.

Separating stroke and acute unilateral vestibulopathy using history, examination and vestibular tests: a machine learning approach.

BACKGROUND: Acute unilateral vestibulopathy (AUVP) and posterior circulation stroke (PCS) are the two common causes of the acute vestibular syndrome. We developed and evaluated machine learning models to differentiate AUVP and PCS using combinations of patient history, examination, and vestibular tests. METHODS: We recruited 294 patients presenting to the Emergency Room (ER) with acute vestibular syndrome (AUVP: n = 163, PCS: n = 131). Data from history, bedside examination and vestibular tests (video-nystagmography (VNG), video head-impulse test (VHIT), vestibular-evoked myogenic potentials

Function tests of the otolith or statolith system.

PURPOSE OF REVIEW: This review aims to provide an overview of recent advances in tests to evaluate otolith function over the last 2 years. RECENT FINDINGS: Over the last 2 years, many papers have focused on the application of the vestibular evoked myogenic potentials (VEMP). Several aspects are under survey: a search for optimal stimuli, search for normative data, search for which labyrinthine function losses and what kind of pathologies induce abnormal VEMPs. The review shows that some fundamental problems still have to be solved to improve reproducibility and to increase sensitivity. Other

Neuro-otology.

New techniques for the assessment of vestibulospinal, neck-afferent, and viscerograviceptive function may show future clinical applications for a broader assessment of balance function in neuro-otological patients. Magnetic resonance imaging studies revealed that many intralabyrinthine disorders (labyrinthitis, hemorrhage, schwannomas) can be adequately visualized. Perceptual and oculomotor clinical studies improved our understanding of vestibular disordered function in the roll (coronal) plane. Therapeutic studies highlight the importance of a team approach, with individually tailored program

Vestibular hyperreactivity in patients with idiopathic spasmodic torticollis.

Ocular motor tests performed on 14 patients with idiopathic spasmodic torticollis were normal. The vestibulo-ocular reflex tested in eight patients showed a significant high level in seven. It is suggested that this phenomenon is secondary rather than the cause of spasmodic torticollis.
