Prenatal vitamin B12 status and cognitive functioning in children at 4 years of age: The ECLIPSES Study.

Maternal vitamin B12 deficiency has been associated with disturbed cognitive functioning in offspring at different ages during childhood. However, this association has not been explored in pre-school-age children. The objective of this study was to examine the association between maternal vitamin B12 levels at the beginning and end of pregnancy and cognitive functioning in their children at 4 years of age. This longitudinal prospective study included a subsample of pregnant women and their children aged 4 years (n = 249) who participated in the ECLIPSES Study conducted in the province of Tarra

Maternal vitamin B12 status in early pregnancy and its association with birth outcomes in Canadian mother-newborn Dyads.

Vitamin B12 (B12) is a co-enzyme essential for fetal growth and development. Lower maternal B12 status has been associated with preterm birth (<37 gestational weeks) and low birth weight (<2500 g), which are linked to morbidity and mortality across the lifespan. In Canada, 17-25 % of women in early pregnancy had a serum total B12 concentration <148 pmol/l and maternal total B12 concentration decreased throughout pregnancy. This study aimed to determine the association between maternal B12 status and birth outcomes in Canadian mother-newborn dyads. A secondary analysis of 709 mother-newborn dya

Utilization of B12 for the treatment of chronic migraine.

Chronic migraine is a particular classification of a headache that is typically unilateral and pulsatile and lasts for at least 3 months. Owing to its high prevalence and detrimental impact on personal, social, and economic aspects of patient lives, much desire has gone into fully understanding the pathogenesis of migraine, and to search for therapeutic agents. In addition to current therapeutics such as triptans, ergotamine, and monoclonal antibodies targeting calcitonin gene-related peptide receptors, vitamin B12 has been investigated for its possible use as a prophylactic agent for migraine

Hypotension: an unusual presentation of vitamin B12 deficiency, with complete recovery following cyanocobalamin therapy.

Vitamin B12 deficiency and its sequelae are well described and reported, especially in vegetarians. However, its association with haemodynamic instability is not well identified. We report a case of a young man, previously healthy, presenting with fever, hypotension requiring vasopressors and pancytopenia. Extensive workup was unrevealing for possible infective, inflammatory or endocrine causes except for vitamin B12 deficiency. Fever and haematological parameters stabilised after adequate supplementation of cyanocobalamin (vitamin B12).

Oral vitamin B12 versus intramuscular vitamin B12 for vitamin B12 deficiency.

BACKGROUND: Vitamin B12 deficiency is common, and the incidence increases with age. Most people with vitamin B12 deficiency are treated in primary care with intramuscular (IM) vitamin B12. Doctors may not be prescribing oral vitamin B12 formulations because they may be unaware of this option or have concerns regarding its effectiveness. OBJECTIVES: To assess the effects of oral vitamin B12 versus intramuscular vitamin B12 for vitamin B12 deficiency. SEARCH METHODS: We searched CENTRAL, MEDLINE, Embase, and LILACS, as well as the WHO ICTRP and ClinicalTrials.gov. The latest search date was 17

Cobalamin and folate deficiency in the elderly.

Elderly persons are more likely to have low values for serum and erythrocyte folate, and for serum cobalamin. Many of those with low vitamin levels have biochemical abnormalities consistent with true deficiency, including increased formiminoglutamic acid excretion, abnormal marrow deoxyuridine suppression, and raised serum levels of methylmalonic acid and homocysteine. Therapy with the appropriate vitamin reverses the biochemical defect. Despite this, the clinical consequences for most elderly persons are remarkably few. True megaloblastic anaemia is rare, and the small number of therapeutic t

Comparison of diagnostic algorithms used in guidelines on nutritional anaemias in adults.

BACKGROUND: Anaemia is highly prevalent and commonly caused by nutritional deficiencies. Guidelines often include an algorithm to find the underlying cause. Here, we compared diagnostic algorithms and suggested laboratory tests for anaemia in clinical practice guidelines in countries with similar healthcare systems, focusing on iron, vitamin B12 and folate deficiency in the general adult population. METHODS: We searched for diagnostic guidelines on anaemia in the Trip, Guidelines International Network and country specific databases. To be selected, the guidelines had to include diagnostic cri

A short review of malabsorption and anemia.

Anemia is a frequent finding in most diseases which cause malabsorption. The most frequent etiology is the combination of iron and vitamin B12 deficiency. Celiac disease is frequently diagnosed in patients referred for evaluation of iron deficiency anemia (IDA), being reported in 1.8%-14.6% of patients. Therefore, duodenal biopsies should be taken during endoscopy if no obvious cause of iron deficiency (ID) can be found. Cobalamin deficiency occurs frequently among elderly patients, but it is often unrecognized because the clinical manifestations are subtle; it is caused primarily by food-coba

Methylmalonic Acid in Skeletal Aging and Osteoporosis: Linking Vitamin B12 Metabolism to Mitochondrial Dysfunction.

Methylmalonic acid (MMA) is a functional marker of vitamin B12 insufficiency and an emerging metabolic factor associated with skeletal aging and osteoporosis. This review synthesizes epidemiological, experimental, and mechanistic evidence linking the vitamin B12-MMA axis to impaired bone homeostasis. Population studies associate elevated circulating MMA with lower bone mineral density, higher odds of osteoporosis, and lower knee extensor strength, while disturbed vitamin B12 metabolism has also been linked to altered bone turnover. Experimental MMA exposure and models of methylmalonyl-CoA muta

Unwarranted variation in total B12 test thresholds for the diagnosis of vitamin B12 deficiency across the UK and the validation of a method-specific total B12 reference interval for the Beckman Coulter DxI platform to support implementation of NICE NG239.

BACKGROUND: Prompt diagnosis and treatment of vitamin B12 deficiency prevents haematological, neurological, psychological and psychiatric disturbances. During November 2024, reference interval data used for the clinical interpretation of total B12 assays was collated from participants of the UK NEQAS for Haematinic Assays Scheme. From these data we characterised the geographical variation in diagnostic thresholds across the United Kingdom to identify regions with poor adherence to National Institute for Health and Care Excellence (NICE) guidance (NG239). METHODS: Applied diagnostic thresholds

A Framework to Guide Defining an Upper Threshold of Crystalline Vitamin B12 in Foods and Food Supplements.

PURPOSE OF REVIEW: To define an intake threshold of vitamin B12 from food supplements that is sufficient to maintain normal body functions, but it does not cause pharmacological effects. RECENT FINDINGS: We used studies on the amount of B12 absorbed following oral B12 application and non-comparative case-series studies to synthesize evidence on pharmacological effects of oral B12 (between < 10 µg and 3000 µg) in people with manifested deficiency. There is a dose-dependent intestinal absorption of B12 and in the same time effects on body metabolism and functions. Food supplements providing ≤ 2

A Rare Case of Severe Pernicious Anemia with Neuropsychiatric Implications.

BACKGROUND Pernicious anemia (PA) is caused by a deficiency in intrinsic factor (IF), which is necessary for vitamin B12 (cobalamin) absorption, resulting in vitamin B12 deficiency and subsequent megaloblastic anemia. Diagnosis of PA relies on the detection of circulating antibodies to IF. In addition to anemia, patients can develop neuropsychiatric conditions, such as subacute combined degeneration of the spinal cord or psychosis. We present the case of a patient with a history of schizophrenia who exhibited unusually severe manifestations of pernicious anemia, including life-threatening anem

Impact of metformin treatment on cobalamin status in persons with type 2 diabetes.

Over the last decades, low vitamin B12 status has been reported in individuals with type 2 diabetes mellitus (T2DM). Metformin, the first-line therapy for lowering blood glucose, is the main driving factor behind this association. Although the relationship between vitamin B12 deficiency and metformin is well established, results of studies on the exact effect of the dose and duration of the therapy remain inconsistent. Additionally, a lack of consensus on the definition of vitamin B12 deficiency adds to the conflicting literature. The objectives of this review were to analyze and synthesize th

Subacute Combined Degeneration of the Spinal Cord Induced by Nitrous Oxide Abuse: A Rare Patient Presentation to a Spine Surgery Clinic: Illustrative Case.

Subacute combined degeneration (SCD) of the spinal cord is a disease involving the lateral and posterior columns of the spinal cord that can manifest in patients with vitamin B12 deficiency. Nitrous oxide (N2O)-induced SCD of the spinal cord is a result of N2O interfering with the metabolism of vitamin B12 and results in nervous system demyelination. This is an infrequent complication of N2O anesthesia; however, cases are rising with recreational N2O use. This case report describes a patient with SCD of the spinal cord induced by recreational N2O abuse. The patient presented to a spine surgery
