Enterobius vermicularis: a possible cause of symptoms resembling appendicitis.

The role of Enterobius vermicularis in appendicitis has been disputed. The aims of this retrospective study were to determine the prevalence … explanations for abdominal pain. In an analysis of the subgroup of 147 patients who had incidental appendectomy at the time of laparotomy for other reasons, only one had E. vermicularis. It is concluded that E. vermicularis occurs more frequently in uninflamed appendices. It may be a cause of symptoms resembling acute appendicitis although the mechanism for t …

Colitis in Behçet's disease: characteristics on double-contrast barium enema examination in 20 patients.

We performed a retrospective review of double-contrast barium enema examinations of 20 patients with clinically proven Behçet's colitis. Mai … al deformity. The appendix was visualized in only three (20%) of the 15 patients with no history of appendectomy. On follow-up study of 15 cases, the ulcers disappeared or decreased in size in 13 cases (86%) and the mucosal deformity was not improved in all cases. On the basis of our results, we believe that the characteristic findings of colitis in Behçet's d …

Cyclic neutropenia with colonic perforation and nonhealing colocutaneous fistula.

Cyclic neutropenia is an hereditary disorder of white blood cells, characterized by profound neutropenic episodes approximately every three weeks. Septic complications are usually limited to cutaneous and oropharyngeal infections. A 4-year-old boy with known cyclic neutropenia was in shock with neutropenia, clostridial septicemia, and right lower quadrant peritonitis when he was admitted. At the time of laparotomy, inflammation of the cecum, with no gross perforation, was found; no resection or appendectomy was done. He subsequently developed a right lower quadrant abscess that was drained, re…

Environmental factors and the risk for childhood pedestrian-motor vehicle collision occurrence.

In King County, Washington, during 1985-1986, 98 children aged less than 15 years involved in pedestrian-motor vehicle collisions resulting in death or injuries severe enough to require hospitalization were identified through the Trauma Registry at the Regional Trauma Center and the Medical Examiner's Office. Two age- and sex-matched controls were selected for each case, one through random digit dialing and the other from children undergoing appendectomy. Various environmental and traffic characteristics were ascertained from visits to the neighborhoods of all subjects. Children living in mult…

Small bowel obstruction and its management.

We present a retrospective analysis of 105 instances of small bowel obstruction (SBO) in 80 patients admitted to our hospital over a ten year period. Adhesions accounted for 73% of the cases and secondary involvement by malignancy for 13%. Appendectomy, colorectal and other pelvic procedures were the most frequent surgical antecedents responsible for the adhesions. In the 86% of cases with a temperature over 100 degrees F there was significant morbidity, mortality and/or strangulation, and this sign also foretold a prolonged hospital stay. Leukocytosis, when present along with abdominal tender…

Incidence and frequency or complications and management of Meckel's diverticulum.

During the last 27 years, 84 patients with Meckel's diverticulum (MD) have undergone resection at our institution (incidental findings, n = 50; MD with complications, n = 34). The most frequent complications of MD were obstruction and diverticulitis. Analysis of records of patients revealed that MD is a rare intraoperative finding. The calculated incidence was 2.4 per cent, and the ratio of diverticulectomy to appendectomy was 1:55. A preoperative diagnosis of MD was rare (4 per cent). Only in those patients with hemorrhage were diagnostic procedures (angiography, scintigraphy and roentgenogra…

Pregnancy-associated ectopic decidua.

Extrauterine formation of decidua of stromal cells has been well described, particularly in the cervix and ovary. Sporadic reports have documented decidua formation of peritoneal surfaces and lymph nodes as well as ovarian ectopic decidua in pregnant patients. The apparent hormonal mechanism of this phenomenon suggests a relationship to endometriosis. Between 1983 and 1986, tissue from 10 pregnant patients (acquired from nine at cesarean section and from one at appendectomy) demonstrated submesothelial decidua formation in the form of microscopic nodules and diffuse cell arrangements in an ede…

[Incidence of intestinal disease due to Yersinia enterocolitica (author's transl)].

Enteropathogenic bacteria have been identified in 413 of 7054 patients (5.9%) with intestinal disease who were examined at the Institute of Hygiene and Microbiology, University of Würzburg (South Germany), during the period November 1975 to November 1977. Salmonella was most frequently isolated (304 cases = 4.3%), followed by Yersinia enterocolitica (102 cases = 1.5%). Cases of shigellosis (7 cases) or infections with so-called enteropathogenic serogroups of Escherichia coli (20 cases) were rarely observed. Disease due to Yersinia enterocolitica occurred in 57 male and 45 female patients. Sixt

Bladder sarcoma treated with radical cystectomy and adjuvant epirubicin and ifosfamide chemotherapy: a case report.

Bladder sarcoma is a rare mesenchymal tumor of the bladder, accounting for approximately 0.3% of all bladder tumors. Due to its low incidenc … ent underwent laparoscopic radical cystectomy with pelvic lymph node dissection, ileal conduit, and appendectomy. Postoperative pathological examination, combined with immunohistochemical results, was considered to be a small round cell undifferentiated sarcoma. Primary Intracranial Cholesteatoma Presenting as a Cerebellar Mass Without Otologic Symptoms.

Intracranial cholesteatoma (epidermoid tumor) is an uncommon benign lesion that may demonstrate locally aggressive behavior, resulting in bone erosion and significant mass effect. Posterior fossa involvement presenting as a cerebellar syndrome in the absence of otologic symptoms is exceptionally rare and may closely mimic primary intracranial neoplasms, posing a diagnostic challenge. We report the case of a 29-year-old male who developed progressive vertigo, gait instability, and projectile vomiting one month after an appendectomy performed under spinal anesthesia. Neurological examination rev…

Management of Acute Cholecystitis in a Patient With Eisenmenger Syndrome and Abdominal Heterotaxy: A Case Report.

BACKGROUND Eisenmenger syndrome presents a unique challenge for the acute care surgeon. Even routine operations such as laparoscopic appendectomy or cholecystectomy become challenging due to the cardiopulmonary physiologic changes and anatomic anomalies associated with Eisenmenger syndrome. The care of these patients can be further complicated by the severity of disease, surgical complexity, and the abnormal anatomy associated with the syndrome. CASE REPORT A 35-year-old patient with Eisenmenger syndrome and abdominal heterotaxy presented with acute cholecystitis. She underwent percutaneous ch…

The patient underwent an uneventful laparoscopic appendectomy using a standard three-port technique without modification. We conclude that a high index of suspicion for variant positions of the appendix is required. Conventional laparoscopic port placement remains effective even in anatomical anomalies. Famili …
