Targeting a uniquely nonspecific prenyl synthase with bisphosphonates to combat cryptosporidiosis.

Cryptosporidiosis is a neglected disease without a wholly effective drug. We present a study demonstrating nitrogen-containing bisphosphonates (N-BPs) to be capable of inhibiting Cryptosporidium parvum at low micromolar concentrations in infected MDCK cells. Predictably, the mechanism of action is based on inhibition of biosynthesis of isoprenoids but the target enzyme is unexpectedly a distinctive C. parvum enzyme dubbed nonspecific polyprenyl pyrophosphate synthase (CpNPPPS). This enzyme produces various isoprenoid products larger than FPP and is inhibited by N-BPs at subnanomolar concentrat

Bone density and bone turnover markers in patients with epilepsy on chronic antiepileptic drug therapy.

In this comparative, cross-sectional study, we evaluated 55 patients with epilepsy on chronic use of antiepileptic drugs (AED); [(38 females and 17 males, 35 +/- 6 years (25 to 47)] and compared to 24 healthy subjects (17 females/7 males). Laboratorial evaluation of bone and mineral metabolism including measurements of bone specific alkaline phosphatase (BALP) and carboxyterminal telopeptide of type I collagen (CTX-I) were performed. Bone mineral density (BMD) was measured by DXA. BALP and CTX-I levels did not differ significantly between the groups. CTX-I levels were significantly higher in p

Computerized tomographic findings in bisphosphonate-associated osteonecrosis of the jaw in patients with cancer.

OBJECTIVES: To study bisphosphonate-associated osteonecrosis of the jaw with respect to radiographic, demographic, and clinical features. STUDY DESIGN: Thirty-two patients with painful exposed bone in the jaws subsequent to treatment with bisphosphonates for various neoplastic diseases were examined with both dental panoramic radiograph and multislice spiral computerized tomography (CT). Each case was assessed for the following radiologic signs: structural alteration of trabecular bone, cortical bone erosion, osteosclerosis, small sequestrum, extensive sequestrum, and presence of periosteal n

The treatment of bisphosphonate-associated osteonecrosis of the jaws with bone resection and autologous platelet-derived growth factors.

BACKGROUND: Bisphosphonates administered intravenously are used to treat patients with cancer who have hypercalcemia associated with malignant disease, multiple myeloma or metastatic tumors (breast, lung, prostate) in the bones. Bisphosphonates are bone resorption inhibitors and have been associated with osteonecrosis of the jaws. In this article, the authors provide an alternative treatment modality for refractory bisphosphonate-associated osteonecrosis (BON). CASE DESCRIPTION: The authors treated 12 patients with refractory BON and a history of long-term bisphosphonate therapy. Each patient

Drug-induced side effects affecting the gastrointestinal tract.

With the ever-growing armamentarium of pharmacological agents, the gastrointestinal drug-induced side effects of dyspepsia, nausea, vomiting, diarrhoea and constipation are increasingly seen. They are often self-limiting and without serious sequelae, but of greater concern is drug-induced mucosal ulceration that can manifest as gastrointestinal haemorrhage, stricture and perforation. These complications are mainly attributable to NSAIDs and aspirin, which can injure the mucosa anywhere along the gastrointestinal tract. These iatrogenic serious side effects can be reduced with co-prescription o

Targeting factors involved in bone remodeling as treatment strategies in prostate cancer bone metastasis.

Prostate cancer is the most commonly diagnosed cancer in men within the western world and the third leading cause of cancer-related deaths. Even if the cancer is considered localized to the prostate, there is a 15% to 20% incidence of subsequent metastatic disease. Prostate cancer has a very high proclivity for metastasizing to bone, with approximately 90% of men with advanced disease having skeletal lesions. The prostate cancer metastases are characteristically osteoblastic, with extensive new bone deposition, unlike other tumors that metastasize to bone and cause an osteolytic response refle

Osteonecrosis of the jaws in patients treated with bisphosphonates - histomorphologic analysis in comparison with infected osteoradionecrosis.

BACKGROUND: Patients treated with bisphosphonates because of bone metastases have been shown to develop osteonecrosis of the jaws. In the present study, we examined the histologic findings of these cases. As similarities between this disorder and infected osteoradionecrosis (IORN) are described, both lesions were compared. METHODS: We investigated eight patients with bisphosphonate treatment and osteonecrosis (four female, four male; median age: 65.6 years; cancer: multiple myeloma in five patients, breast cancer in three patients; mandibular involvement in five patients, maxillar involvement

Oral pamidronate prevents high-dose glucocorticoid-induced lumbar spine bone loss in premenopausal connective tissue disease (mainly lupus) patients.

Glucocorticoid (GC)-induced osteoporosis contributes to chronic damage in patients suffering from connective tissue diseases (CTD) such as systemic lupus erythematosus (SLE). In this study, performed in an highly selected cohort of premenopausal female CTD (mostly lupus) patients, given high-dose GC therapy for severe disease, we show that lumbar spine bone loss can be averted by treatment with oral disodium pamidronate combined with calcium salts and vitamin D3 supplements and not by calcium salts and vitamin D3 supplements alone. We stress the need for optimal GC-induced bone loss prevention

Symptomatic hypocalcaemia and renal impairment associated with bisphosphonate treatment in patients with multiple myeloma.

We report three cases of severe hypocalcaemia associated with i.v. bisphosphonate treatment in patients with multiple myeloma. All patients had symptomatic hypocalcaemia, including a tonic-clonic seizure and tachyarrhythmia in one case. Two cases were associated with the development of acute renal failure, whereas the third patient had pre-existing renal impairment. We recommend that bisphosphonates be used with caution in patients with myeloma and renal impairment, that vitamin D deficiency be corrected prior to treatment (to reduce the risk of hypocalcaemia) and that serum calcium and renal

Oral ibandronate: a less frequently administered therapeutic option for postmenopausal osteoporosis.

Osteoporosis is a severe condition, associated with significant disability as a result of fragility fractures and increased mortality. Oral bisphosphonates effectively reduce the risk of osteoporotic fracture and are generally well tolerated. Unfortunately, patient outcomes are often compromised by suboptimal therapeutic adherence. In other disease areas, reduced dosing frequency has been shown to improve therapeutic adherence. A positive impact for adherence has been observed with a reduction in the bisphosphonate dosing frequency from daily to weekly. However, overall adherence remains subop

What is osteoporosis?

Osteoporosis is a very common disorder, which results in an increase in fracture risk. The annual cost attributable to hip, vertebral, and wrist fractures in England and Wales is pound 1.7 billion. Significant mortality and morbidity are associated with osteoporotic fractures. The method that is most widely used for the diagnosis of osteoporosis is dual energy x-ray absorptiometry. The aim of prevention and treatment of osteoporosis is to prevent the occurrence of future fractures. Lifestyle changes should be encouraged in high risk patients. Pharmacological treatments include the bisphosphona

Effects of treatment with ibandronate on bone mass, architecture, biomechanical properties, and bone concentration of ibandronate in ovariectomized aged rats.

OBJECTIVE: To investigate the effects of treatment with ibandronate, a highly potent nitrogen-containing bisphosphonate, on bone loss, bone quality, biomechanical properties, and bone concentrations in aged ovariectomized rats. METHODS: Eight-month-old female Wistar rats were ovariectomized (Ovx) or sham-operated. Treatment was started 10 weeks following Ovx with subcutaneous ibandronate in doses of 0.2, 1.0, 5.0, or 25 micro g/kg/day for 12 mo. Additional groups received 25 or 125 micro g/kg intermittently every 25 days, resulting in the same total dose as compared to 1.0 or 5.0 micro g/kg/d

Cyclic administration of pamidronate in children with severe osteogenesis imperfecta.

BACKGROUND: Severe osteogenesis imperfecta is a disorder characterized by osteopenia, frequent fractures, progressive deformity, loss of mobility, and chronic bone pain. There is no effective therapy for the disorder. We assessed the effects of treatment with a bisphosphonate on bone resorption. METHODS: In an uncontrolled observational study involving 30 children who were 3 to 16 years old and had severe osteogenesis imperfecta, we administered pamidronate intravenously (mean [+/-SD] dose, 6.8+/-1.1 mg per kilogram of body weight per year) at 4-to-6-month intervals for 1.3 to 5.0 years. Mineralization disturbances of the developing rat molar induced by mono- and bisphosphonates.

Morphological changes have been reported previously in the developing dental tissues of the rat molar following administration of mono- or bisphosphonates. However, few studies have been published on the relationship between these and the possible mineralization disturbances. Two monophosphonates, phosphonoformic and phosphonoacetic acids, and two bisphosphonates, dichloromethylene bisphosphonate (Cl2MBP) and 1-hydroxyethylidene-1, 1-bisphosphonate (HEBP) in a dosage of 10-30 mg P/kg b.w. were injected in 5 day old rats and the induced changes analyzed by microradiographic and histologic exami

Scintigraphic findings in progressive diaphyseal dysplasia.

A 14-yr-old white male with a severe form of progressive diaphyseal dysplasia (Engelmann-Camurati disease) was referred to our institution for evaluation of splenomegaly, which is not usually associated with the disease. Our studies included bone-marrow, and liver-spleen scans. These scintigraphic findings, along with the probable cause for splenomegaly, are discussed.

Postoperative bisphosphonate therapy is associated with lower prosthesis-related complications after total or reverse shoulder arthroplasty in osteoporotic patients: a TriNetX-based cohort study.

OBJECTIVES: Osteoporosis is a prevalent comorbidity in patients undergoing total or reverse shoulder arthroplasty and is associated with higher rates of periprosthetic fracture, implant loosening, and revision surgery. While bisphosphonates are commonly used to treat osteoporosis, prior research has primarily examined preoperative use. This study aimed to evaluate the association between postoperative bisphosphonate initiation and implant-related complications, particularly revision surgery, in osteoporotic patients undergoing shoulder arthroplasty. DESIGN: We used the TriNetX U.S. Collaborat

Comparison of bone mineral density changes between denosumab and bisphosphonates in tenofovir-exposed chronic hepatitis B patients with osteoporosis.

UNLABELLED: This study compared denosumab and bisphosphonates for osteoporosis in tenofovir-exposed chronic hepatitis B patients. Both denosumab and bisphosphonates increased bone mineral density (BMD) at year 1. However, denosumab resulted in a greater BMD improvement and a more pronounced reduction in procollagen type 1 N-terminal peptide (P1NP) compared to bisphosphonate. These findings suggest that while both drugs have beneficial effects, denosumab may be the more effective therapeutic option in this population. BACKGROUND: Tenofovir disoproxil fumarate (TDF), a first-line antiviral for
