Experimental study of the effect of prostaglandin administration on tooth movement--with particular emphasis on the relationship to the method of PGE1 administration.

Recently, the mechanism of action of prostaglandins in promoting bone resorption has received wide attention. Prostaglandins are used in orthodontics to shorten the period of tooth movement. PGE1 was administered locally (5 micrograms/kg every 12 hours) and systemically (7.5 ng/kg/min) to rats to study the difference in the efficacy of the two methods of administration in accelerating bone resorption. The Waldo method was used on 72 Wistar rats (8 weeks old) for 5 days to induce mesial movement of the first molar. The body weight of the rats was measured every 12 hours from the start of the ex

A quantitative assessment of respiratory patterns and their effects on dentofacial development.

The purpose of this study was to assess the effects of quantitatively determined breathing patterns on dentofacial development in growing children. Forty-nine subjects ranging in age from 10 to 16 years participated in the breathing pattern assessment portion of this project. Oral, nasal, and total airflow were measured at separate times by means of a head-out body plethysmograph technique and the values were compared with the subjects' and parents' subjective perceptions of their breathing modes. These breathing pattern measurements also were compared to nasal airway resistance and nasal powe

Effect of surgical reduction of the tongue on oral stereognosis, oral motor ability, and the rest position of the tongue and mandible.

The oral ability to recognize forms and oral motor ability were studied by means of two specific tests in 27 subjects, 10 to 23 years of age, before and after tongue reduction because of macroglossia. Recordings were made before and 6 and 12 months after the operation. At the same time the natural position of the head and of the cervical column, the craniocervical relation, the position of the tongue and the hyoid bone, and the rest position of the mandible were studied with profile roentgen-cephalometry. The surgical reduction of the tongue had a minor influence on the subject's performance i

CT density of the TMJ disk: correlation with histologic observations of hyalinization, metaplastic cartilage, and calcification in autopsy specimens.

The attenuation number of the TMJ disk was measured in direct sagittal computerized tomograms (CTs) of eight fresh TMJ autopsy specimens. The CT attenuation number was correlated to histologic observations of hyalinization, calcification, and cartilaginous metaplasia of the disk. Correlations were found between high CT attenuation numbers and the presence of hyalinization, calcification, and foci of cartilaginous metaplasia in the disks. The results suggest that the high CT attenuation number of a disk might be associated with hyalinization, calcification, and metaplastic cartilage formation.

A cephalometric analysis of patients with coronoid process enlargement and locking.

Two patient groups with radiographically verified coronoid process locking were cephalometrically examined. In eight patients the locking was of congenital origin, and in eight patients the locking had developed as a result of internal derangement of the temporomandibular joint. When compared with that in a control group, the height of the coronoid process was statistically significantly greater in both patient groups, but there was no difference on condylar height between any of the groups. Neither were there any differences between groups regarding the upper face height, which, if present, c

Legal aspects of orthodontic practice: risk management concepts. Diagnosis, root resorption, and progress monitoring.

In this and succeeding issues of the American Journal of Orthodontics and Dentofacial Orthopedics, factual risk management scenarios will be presented. These scenarios are based on composites of actual court cases that have been tried to verdict or decision. Valuable risk management lessons may be learned from careful analysis of the course of the events described. Please be advised that the standard of care determined in any case is specific for that jurisdiction and that set of facts as established by expert testimony for the prevailing party.

A study of the relationship between incisor intrusion and root shortening.

Apical root shortening is one of the most common complications of orthodontic treatment. Force magnitude has been suggested as an important factor. Studies on the occurrence of root resorption show equivocal results. The aim of the present study was to evaluate the relationship between intrusion with low forces (25 gm) using utility arches in the bioprogressive technique and root shortening. Age, sex, facial type, treatment time, extraction versus nonextraction therapy, width of the symphysis, and the angle of the incisors to skeletal reference planes also were studied for their relationship t

The effect of cortisone on the eruption rate of root-resected incisors in the rat.

It is generally thought that the periodontal ligament plays a key--perhaps even exclusive--role in tooth eruption. The present study was designed to test the hypothesis that the fundus also may contribute to the process. To this end the effect of cortisone on the eruption rate of root-resected mandibular incisors was examined in male albino rats. Half of the animals received daily injections of cortisone; the other half were given normal saline. Impeded and unimpeded eruption rates were measured at 3-day intervals and tested for between- and among-means differences by repeated-measures analysi

Effects of a fixed magnetic appliance on the dentofacial complex.

The purpose of the study was to design and evaluate the effects of a fixed magnetic appliance that hinged the mandible open and exerted an intrusive force on the teeth. Ten patients between the ages of 8 years and 10 years 6 months, with Class II, Division 1 malocclusion associated with mandibular retrusion and increased lower facial height, were treated with this appliance. The length of treatment was 4 months, after which the appliance was removed and the patients were followed up for 4 months. Ten children with similar age, sex, and dentofacial characteristics acted as controls and did not

Clinical experience with third molar orthodontics.

An orthodontic uprighting technique for effective treatment of impacted third molars, exposed or unexposed, is described. When used with conventional fixed appliances, this technique presents a viable option to extraction or prediction of eruption. Several applications illustrate that, with proper intervention, this uprighting technique is simple and effective in positioning malposed third molars.

A cephalometric assessment of children with fetal alcohol syndrome.

Lateral cephalometric radiographs were quantitatively assessed in a series of 15 American black children with fetal alcohol syndrome (FAS). Although none was profoundly affected, FAS had been diagnosed in all the children at birth. Comparisons with age-, sex-, and race-matched controls disclosed a triad of facial profile differences: (1) frontal bossing, (2) palatal plane tipped up in the front with proclined upper incisors and a sharp nasolabial angle acquired from digit habits, and (3) above-average length of the mandibular corpus. Collectively these generate the perception of midface hypopl

Partitioning of ventilation between nose and mouth: the role of nasal resistance.

We have examined the relationship between nasal resistance (Rna) and the distribution of ventilation between the nose and mouth in 10 normal breathing children and 15 children who met clinical criteria of mouth breathing. We studied Rna by posterior rhinometry. We used a face mask divided into separate oral and nasal chambers to measure oral and nasal components of ventilation. Each chamber of the mask was connected to a separate pneumotachograph. We measured oral and nasal tidal volumes (VTna) by integration of the oral and nasal flow, and calculated the total tidal volume (VTtot) by summing

The effect of age on tooth movement and mineral density in the alveolar tissues of the rat.

A comparison of tooth movement cycles and changes in alveolar tissue mineral densities was made between young (21 to 28 days old) and adult (90 to 100 days old) rats. An initial 60-g mesial tipping force was applied to the maxillary first molars; tooth movement was estimated by measuring the opening between first and second molars, and tissue mineral density by sample ash weight per cubic centimeter. A characteristic three-part tooth movement cycle was found in both groups of rats. This cycle consists of an early "instantaneous" movement that is a function of the viscoelastic properties of the

Effect of orthodontic force on periodontal tissue metabolism. A histologic and biochemical study in normal and hypocalcemic young rats.

The relationship between force and degradation activity after application of orthodontic force is still obscure. Of particular clinical interest are the etiologic factors behind excessive root resorptions appearing in connection with orthodontic movement of teeth that has been proposed to be influenced by systemic factors regulating the tissue-degrading activity in periodontal tissues. Thus, the aim was, by histologic and new biochemical methods, to investigate the effect of orthodontic forces on the periodontal tissues in the normal and the hypocalcemic situation with secondary hyperparathyro

New attachment through periodontal treatment and orthodontic intrusion.

The present study was performed to investigate the tissue reaction related to orthodontic intrusion of teeth with a reduced periodontium and further to evaluate the influence of oral hygiene on this reaction. In each of five Macaca fascicularis monkeys, periodontal tissue breakdown was induced around the premolars and the upper incisors by placing orthodontic elastic ligatures around the teeth. The breakdown procedure was continued until a minimum of four pockets could be measured on probing. Following removal of the elastics, a flap operation was performed. The pocket epithelium and granulati

Changes in the molar relationship between the deciduous and permanent dentitions: a longitudinal study.

The purpose of this study was to describe the changes in the molar relationship from the deciduous dentition to the permanent dentition in 121 subjects from the Iowa Longitudinal Growth Study. In addition, an attempt was made to determine the association between the various dentofacial variables and the changes in the molar relationship in 55 persons (33 male and 22 female subjects) with normal occlusion. All subjects were evaluated at three stages of dental development: stage I, completion of the deciduous dentition (means age = 4.94 years); stage II, when permanent first molars initially eru

The effect of load duration on long-term recovery of the eruptive function in the rat incisor.

The purpose of this study was to determine the long-term recovery of the normal rate of eruption of the rat incisor following application of loads of similar magnitude but of varying duration. The daily rate of eruption of the continuously growing tooth served as indicator of the functional recovery of its periodontium. The left mandibular incisor was shortened out of occlusion in 34 experimental and 16 control rats. The experimental animals were divided into groups A, B, and C, and continuous loads (19.47 g +/- 0.42 g) were applied to the shortened incisors by means of closed-coil springs for

Resorption of maxillary lateral incisors caused by ectopic eruption of the canines. A clinical and radiographic analysis of predisposing factors.

Factors predisposing to resorption of adjacent permanent lateral incisors caused by ectopic eruption of maxillary canines were evaluated. The subjects consisted of two groups: one with 40 lateral incisors with resorption caused by ectopic eruption and a control group of 118 ectopic eruption cases with no lateral incisor resorption. The mean age of the children in the two groups differed by only 0.7 of a year and ranged from 10.0 to 15.0 years, covering the normal eruption period of the maxillary canine. Resorption of lateral incisors was three times as common in girls as in boys. The resorptio

Substitution of a transposed premolar for a congenitally absent lateral incisor.

The purpose of this report is to review the orthodontic treatment of a patient with bilateral transposition of maxillary canines and first premolars in addition to congenital absence of a maxillary lateral incisor. This unique combination of factors led to substitution of the maxillary left first premolar for the left lateral incisor. The occlusal fit, gingival form, and esthetic appearance of premolar/lateral incisor substitution are discussed.
