Perioperative conditions affect long-term hypertrophic scar formation.

Corticosteroids are widely used as treatment for excessive scarring by intralesional injection with variable success rates. It is conceivable that systemically administered corticosteroids affect a wider range of inflammatory processes that influence wound healing and may be more successful in preventing hypertrophic scar formation. To study this presumption, we have used a standardized model of presternal scars caused by cardiothoracic surgery through a median sternotomy incision. During cardiac surgery with cardiopulmonary bypass, 1 mg/kg dexamethasone was administered preoperatively, and 0.

Primary sternal closure with titanium plate fixation: plastic surgery effecting a paradigm shift.

BACKGROUND: Postoperative mediastinitis is a serious and potentially lethal complication from cardiac surgery. Although postoperative mediastinitis cannot be reliably predicted, a number of preoperative and intraoperative risk factors have been defined by previous work. The authors now present their cumulative experience with primary sternal fixation of high-risk patients as one preventative measure. METHODS: A retrospective review from July of 2000 to October of 2006 was performed on 750 patients who had at least three established risk factors for postoperative mediastinitis and received pri

One-stage repair of aortic coarctation and ascending aortic aneurysm by extra-anatomic graft.

The best surgical approach for patients presenting with coarctation of the aorta and additional surgical cardiovascular disorders is uncertain. We describe the case of a young man with an aneurysm of the ascending aorta and a bicuspid aortic valve with a moderate insufficiency associated with a coarctation of the aorta. The patient underwent a single-stage procedure where the ascending aorta was replaced, the aortic valve repaired and the coarctation bypassed with an extra-anatomic graft. In our opinion, ascending-to-descending extra-anatomic graft is a good solution to treat these complicated

Clinical and radiologic evolution of synovitis, acne, pustulosis, hyperostosis, and osteitis syndrome: a single center study of a cohort of 71 subjects.

OBJECTIVE: To assess the basic features and outcomes of synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome. METHODS: We identified all patients seen in our unit between 1990 and 2008 diagnosed according to the proposed inclusion criteria with SAPHO syndrome, who had a followup of at least 2 years. RESULTS: Seventy-one patients (48 women, 23 men) with SAPHO syndrome were identified. The median disease duration at the end of followup was 10 years (interquartile range [IQR] 7-15 years), and the median followup duration was 11 years (IQR 6-11.5 years). Six patients were dia

Intraoperative retrograde type I aortic dissection in a patient with chronic type IIIb dissecting aneurysm.

Iatrogenic acute aortic dissection of the ascending aorta during cardiac surgery is a rare but potentially fatal complication. We describe the emergency repair of iatrogenic acute aortic dissection of the ascending aorta during distal arch replacement in a patient with a chronic type IIIb dissecting aneurysm. We scheduled distal arch and descending aortic aneurysm repair through a left anterolateral thoracotomy with a femoro-femoral bypass. While trimming the proximal suture line, retrograde aortic dissection occurred from the cross-clamped site to the aortic root. Transesophageal echocardiogr

A developmental toxicity-screening test of valerian.

Valerian (Valeriana Officinallis) is a perennial plant used as a mild sedative for anxiety and for insomnia. It is also used in the treatment of gastrointestinal cramps and as a diuretic. It is traditionally contraindicated in pregnancy; however, there are no studies to warrant this warning. This study was performed to provide some information to fill this knowledge gap. Female rats were orally dosed with a valerian extract in 45% ethanol (supplied by MediHerb) daily on either gestation days (GD) 1-8 or 8-15. On GD 20, rats were sacrificed and fetuses, placentae and ovaries collected. The fetu

Postoperative effects of opioid analgesics administered via continuous perfusion and patient controlled analgesia after open heart surgery.

Critical care nurses and physicians are familiar with the principles of patient controlled analgesia and the opioid analgesics' regimens and observations necessary for pain control in the postoperative cardiac surgical patients. The objective of the study was to compare the effects of morphine, fentanyl, meperidine, remifentanil and tramadol which were administered by patient controlled analgesia and continuous intravenous infusion combination on the various parameters. This study was designed as prospective randomised trial. Fifty patients undergone open heart surgery with sternotomy were ent

Surgical patch closure of atrial septal defects.

BACKGROUND: Development of nonsurgical techniques for closure of atrial septal defects (ASD) has prompted reevaluation of current surgical outcomes with an emphasis on less invasive methods. METHODS: This retrospective review is based on a single surgeon's experience between July 1, 1988 and December 21, 2002 with 176 consecutive adult (n = 47) and pediatric (n = 129) surgeries, in which ASD was the primary anatomical diagnosis to ascertain current optimal methods and outcomes expected for surgical closure. Patch closure with pericardium was used in all cases. Surgical methods encompassed thr

Pectoralis muscle flap reconstruction of pediatric sternal wound infections.

Sternal wound infection (SWI) is a life-threatening complication in both the pediatric and adult population. The morbidity and mortality of SWIs have decreased with the use of muscle flap reconstruction of the chest wall. Although the pectoralis muscle flap is the most frequently used flap in adults for reconstruction after SWI, its use in children has not been well described. To review current experience with the use of the pectoralis muscle flap in a pediatric population, a retrospective review of 1,200 consecutive median sternotomies occurring at the Children's Hospital of Pittsburgh betwee

Traumatic scapulothoracic dissociation.

Scapulothoracic dissociation is an infrequent injury with a potentially devastating outcome. The diagnosis is based on clinical and radiographic findings of forequarter disruption. These include massive soft tissue swelling of the shoulder, displacement of the scapula and neurovascular injuries (brachial plexus, subclavian artery and osseous-ligamentous injuries). The mechanism of injury appears to be the delivery of severe rotational force sheering the shoulder girdle from its chest wall attachments around the scapula, shoulder joint and at the clavicle. Early recognition of the entity and ag

Spinal disorders at the cervicothoracic junction.

STUDY DESIGN: This study reviewed 36 retrospective patients who underwent surgeries for rare cervico-thoracic junctional problems. OBJECTIVES: The authors review cervico-thoracic junctional disorders and study diagnostic methods, surgical approaches, surgical outcomes, and associated complications. SUMMARY OF BACKGROUND DATA: The literature is sparse on cervico-thoracic junctional problems. This paper is the largest series to date on this subject. METHODS: Thirty-six patients who underwent surgeries for spinal problems at the cervico-thoracic region (C7-T3) were reviewed. These included 18

Mid-mediastinal parathyroid lesions: preoperative localization and surgical approach in two cases.

Although hyperfunctioning mediastinal parathyroid lesions that require median sternotomy or thoracotomy for removal are occasionally present, the majority are located in the anterior mediastinum closely associated with the thymus. Only eight cases of ectopic hyperfunctioning parathyroid tumors in the middle mediastinum have been reported. We experienced two cases of either persistent or recurrent hyperparathyroidism in which abnormal parathyroid tissue was located in the aorticopulmonary window. One of the patients had a parathyroid adenoma and the other had metastatic lesions of parathyroid c

Perichondritis (Tietze's syndrome) at the xiphisternal joint: a mimic of severe disease.

Tietze's syndrome (peristernal chondritis or perichondritis) is a self-limited, painful inflammatory condition of the costochondral, sternoclavicular, manubriosternal or xiphisternal junctions. It is benign, and affects all ages, sexes and races equally. The principal manifestations are pain and, frequently, a mass most commonly at the costochondral junctions. In 1974, C.J. described the first cases to be reported at the xiphisternal junction. We describe here 24 cases at that site. These patients' presenting symptoms suggested cardiac, pulmonary, intraabdominal and other severe disease proces

Sternal and costochondral infections following open-heart surgery. A review of 2,594 cases.

From a series of 2,594 patients undergoing open-heart surgery, 39 had sternal or costochondral infections. Most of these infections were associated with a number of predisposing factors: prolonged perfusion time, excessive postoperative bleeding, depressed cardiac output in the postoperative period, and a history of re-exploration for the control of hemorrhage. One important factor was the use of bilateral internal mammary artery implants. The prognosis for patients with sternal wound infections appears related to the length of time required for institution of treatment and the adequacy of ini

Clinical outcome of juvenile myasthenia gravis after extended transsternal thymectomy in a chinese cohort.

BACKGROUND: The role of surgical treatment for juvenile myasthenia gravis (MG) remains unclear. Here, we performed a retrospective study to evaluate the predictors of clinical outcome of juvenile MG treated with extended transsternal thymectomy. METHODS: A total of 141 consecutive juvenile MG patients underwent extended transsternal thymectomy at an academic hospital over a 20-year period were reviewed. Thymectomy was performed in patients resistant to pyridostigmine therapy, with generalized symptoms or ocular MG with partial response to pyridostigmine for more than 2 years. Variables potent

A simple technique for pectus bar removal using a modified Nuss procedure.

BACKGROUND: Although the Nuss procedure has been widely accepted as the standard procedure for the repair of pectus excavatum in children, adolescents, and even adults, few reports have documented the bar removal procedure as a whole. In this study, we retrospectively evaluated the safety and efficacy of a modified Nuss bar removal procedure. METHODS: A total of 186 patients undergoing bar removal after the Nuss procedure were included in this study. All cases had unilateral incision (metallic stabilizers were used on one side in all patients). Patients were laid down in the supine position a
