The Association of Hearing Loss With Active Music Enjoyment in Hearing Aid Users.

OBJECTIVE: Hearing aids (HAs) are designed for speech rather than music listening. The impact of HAs on music enjoyment is poorly studied. We examine the effect of HAs on active music enjoyment in individuals with varying levels of hearing loss (HL). STUDY DESIGN: Cross-sectional study. SETTING: Tertiary medical center and community. METHODS: Adult (≥18 years) bilateral HA users and normal hearing (NH) controls actively listened to musical stimuli and rated their enjoyment across 3 measures (pleasantness, musicality, naturalness) with and without HAs using a visual analog scale. Multivariab

The outcomes of bilateral bone conduction hearing devices (BCHD) implantation in the treatment of hearing loss: A systematic review.

OBJECTIVES: Bone Conduction Hearing Devices (BCHDs) are a means of improving hearing ability in those that do not receive full benefit from conventional hearing aids. In 2016, the NHS started commissioning bilateral BCHDs. This review aims to evaluate the current literature and identify areas for further research. METHODS: This review was conducted in line with the PRISMA guidelines and registered on the PROSPERO database (registration CRD42020191956). MEDLINE, CINAHL and The Cochrane Library were systematically searched. The full text of relevant titles and abstracts was then reviewed before

The posterior ligament of the incus ("white dot"): A reliable surgical landmark for the facial recess.

OBJECTIVE: There is a void in the literature describing reliable surgical landmarks that aid in the dissection of the facial recess in the absence of skeletonizing the mastoid segment of the facial nerve. The posterior ligament of the incus is a readily distinguishable "white dot" along the incus buttress that has been used to guide dissection in a safe and efficient manner. The goal of our study is to describe a surgical approach that utilizes this surgical landmark to drill the facial recess and to take anatomical measurements demonstrating the safety and reliability of this approach. Bilateral Deafness as a Complication of the Vaccination-A Case Report.

The vaccination has much profit for an infectious disease. On the other hand, there is little frequency, side effects may appear. It includes severe complication. We reported the case that resulted in bilateral acute profound hearing loss after mumps alone and measles and rubella (MR) vaccination. The case was a 5 years old girl. She inoculated mumps alone and MR vaccine. After 18days later, both sides profound hearing loss occurred in her. The hearing loss was not improved by the intravenous feeding of the steroid. Three months later, cochlea implantation was carried out to her right ear. She

Chronology of labyrinthitis ossificans induced by Streptococcus pneumoniae meningitis.

OBJECTIVE: Labyrinthitis ossificans consists of novel osteogenesis that fills the normally patent cochlear and vestibular lumen as an end-stage sequelae to various pathologies. This study was designed to establish the sequence of events and chronology of the osteoneogenesis and calcification. STUDY DESIGN: A prospective randomized double-blind study. METHODS: By using serial application of different colored fluorochromes, which deposit in newly forming bone, the timing of bone deposition and bone remodeling can be established. Labyrinthitis ossificans was induced in six groups (n = 5) of ger

AUDITORY NERVE: ELECTRICAL STIMULATION IN MAN.

Auditory perceptions produced in a person deaf to acoustic stimulation were studied by electrically exciting the auditory nerve through permanently implanted electrodes. Pulsed current as small as 1 microampere peak-to-peak could be perceived. Pitch, as reported by the subject, varied with electrode selection, current amplitude, and pulse repetition rate from about 70 to at least 300 pulses per second. Loudness increased with amplitude and duration of pulse stimuli, and to a lesser extent with repetition rate. The total range in amplitude of the stimulus, from threshold to an uncomfortable lou

Intracochlear PLGA implants for simultaneous controlled release of multiple drugs.

Reliable drug delivery to the inner ear is still an unmet clinical need, due to the blood cochlear barrier, which effectively protects this highly sensitive organ. To overcome this hurdle, miniaturized, biodegradable implants were prepared (0.3 mm in diameter), which can be administered into the cochlea via a tiny hole drilled into the round window. They were based on poly(lactic-co-glycolic acid) (PLGA) and prepared by hot melt extrusion. Three drugs were incorporated: (i) 20.0% dexamethasone with anti-inflammatory, anti-fibrotic and protective activity for hair cells (being critical for hear

The feasibility of endoscopic transcanal approach for insertion of various cochlear electrodes: a pilot study.

To determine the feasibility of inserting various types of electrode arrays using an endoscopic transcanal approach into the cochlea via the round window membrane (RWM). All the procedures were performed by the first author and started with a cortical mastoidectomy. A six o'clock vertical incision was made in the meatal skin, and a posterior tympano-meatal flap was elevated transmeatally to expose the middle ear cavity using a rigid 0° endoscope (diameter 3 mm, length 14 cm). The chorda tympani nerve (CTN) and body of the incus were exposed. The RWM was incised, and the electrodes were passed

Self-Adjustment of Upper Electrical Stimulation Levels in CI Programming and the Effect on Auditory Functioning.

OBJECTIVES: With current cochlear implants (CIs), CI recipients achieve good speech perception in quiet surroundings. However, in acoustically complex, real-life environments, speech comprehension remains difficult and sound quality often remains poor. It is, therefore, a challenge to program CIs for such environments in a clinic. The CI manufacturer Cochlear Ltd. recently introduced a remote control that enables CI recipients to alter the upper stimulation levels of their user programs themselves. In this concept, called remote assistant fitting (RAF), bass and treble controls can be adjusted

Auditory velocity discrimination in the horizontal plane at very high velocities.

We determined velocity discrimination thresholds and Weber fractions for sounds revolving around the listener at very high velocities. Sounds used were a broadband white noise and two harmonic sounds with fundamental frequencies of 330 Hz and 1760 Hz. Experiment 1 used velocities ranging between 288°/s and 720°/s in an acoustically treated room and Experiment 2 used velocities between 288°/s and 576°/s in a highly reverberant hall. A third experiment addressed potential confounds in the first two experiments. The results show that people can reliably discriminate velocity at very high velociti

The effect of percutaneous and transcutaneous BAHA on hearing and subjective auditory abilities: A comparative study.

Hearing loss significantly affects communication, social interactions, and the overall quality of life. The bone-anchored hearing aid (BAHA) is an implantable system that bypasses the outer and middle ear to directly stimulate the cochlea through bone conduction. This study aimed to compare hearing performance and subjective auditory ability improvements between transcutaneous and percutaneous BAHA devices using audiological assessments and Speech, Spatial and Qualities of Hearing Scale. This cross-sectional prospective study included 29 participants aged 14 to 69 years who had used BAHA for a

Tinnitus treatment with precise and optimal electric stimulation: opportunities and challenges.

PURPOSE OF REVIEW: Electric stimulation is a potent means of neuromodulation that has been used to restore hearing and minimize tremor, but its application on tinnitus symptoms has been limited. We examine recent evidence to identify the knowledge gaps in the use of electric stimulation for tinnitus treatment. RECENT FINDINGS: Recent studies using electric stimulation to suppress tinnitus in humans are categorized according to their points of attacks. First, noninvasive, direct current stimulation uses an active electrode in the ear canal, tympanic membrane, or temporal scalp. Second, inner e

Feedback analysis in percutaneous bone-conduction device and bone-conduction implant on a dry cranium.

HYPOTHESIS: The bone-conduction implant (BCI) can use a higher gain setting without having feedback problems compared with a percutaneous bone-conduction device (PBCD). BACKGROUND: The conventional PBCD, today, is a common treatment for patients with conductive hearing loss and single-sided deafness. However, there are minor drawbacks reported related to the percutaneous implant and specifically poor high-frequency gain. The BCI system is designed as an alternative to the percutaneous system because it leaves the skin intact and is less prone to fall into feedback oscillations, thus allowing

Gene therapy for hereditary deafness: progress, achievements and future challenges.

BACKGROUND: Sensorineural hearing loss is the most common sensory disorder, and more than 50% of congenital cases are genetic. For decades, clinical management has relied largely on prosthetic devices such as hearing aids and cochlear implants, which improve audibility but do not restore native cochlear biology. AIMS: To review how advanced biological therapies-especially inner-ear gene therapy-have evolved from animal proof-of-concept to first-in-human trials. To summarize the progression of therapeutic strategies (gene replacement, antisense oligonucleotides, genome/RNA editing) and the tec

Neurotrophin gene therapy to promote survival of spiral ganglion neurons after deafness.

Hearing impairment is a major health and economic concern worldwide. Currently, the cochlear implant (CI) is the standard of care for remediation of severe to profound hearing loss, and in general, contemporary CIs are highly successful. But there is great variability in outcomes among individuals, especially in children, with many CI users deriving much less or even marginal benefit. Much of this variability is related to differences in auditory nerve survival, and there has been substantial interest in recent years in exploring potential therapies to improve survival of the cochlear spiral g

Acoustically and electrically evoked contralateral suppression of otoacoustic emissions in guinea pigs.

It is generally accepted that stimulation of the efferent auditory system results in changes of cochlear activity. A simple method of activating the olivocochlear pathway by contralateral electrical stimulation of the round window (ES-RW) was used in this study with the aim of comparing the efficacy of acoustically and/or electrically evoked contralateral suppression. The suppression of transient evoked otoacoustic emissions (TEOAEs) and distortion product otoacoustic emissions (DPOAEs) was elicited by contralateral acoustic stimulation (AS) (61 dB SPL continuous white noise), and/or by electr
