The goal of the current study was to analyze patients treated with the Gamma nail, and to describe techniques that prevent commonly reported complications. One hundred patients with closed peritrochanteric femur fractures were treated by one surgeon using the Gamma nail. Ninety patients met the minimum 6 month followup requirement. Eighty-eight of the 90 fractures (98%) healed after the index procedure. There were nine (10%) complications (nine patients) with four patients (4%) requiring surgical intervention. One patient required total hip arthroplasty because of nonunion, and one patient req

Treatment of actual or impending pathologic fractures of the femur provides the senior author with some of the most rewarding surgical interventions of his practice. The patients' survival outlook is not changed, but their quality of life is enhanced significantly. Most health care providers usually provide the metastatic cancer patient only temporary symptomatic relief, at best, and often at the expense of continued pain, suffering, or sickness, such as is seen with chemotherapy-associated morbidity. Patients with metastatic bone disease are usually incredibly grateful for the restoration of

In the first part of the study we examined the accuracy of femoral anteversion (AV) determined by fluoroscopy, simulating a method used in closed intramedullary femoral nailing. Twenty cadaveric femora were used. The condyles and posterior trochanteric area were resting on a horizontal table. The proximal femur was imaged using a C-arm image intensifier with horizontal X-ray beam at an angle of 30, 45, or 60 degrees to the long axis of the femoral shaft. To evaluate the reliability of the measurements, the real AV angle was determined by radiography. A small difference (mean 2 degrees ) was fo

Intercalary replacement is a valuable option in the treatment of diaphysary long bone metastases. In certain cases methylmethacrylate bone cement may be used as a cheap and readily available alternative to modular or custom-made prostheses. Stabilization is best performed by locked intramedullary nailing. By cutting open the cylindric part of a large syringe and placing it around the bone ends after resection a mould is created where liquid bone cement can be filled in. Thus spilling of liquid methacrylate into the adjacent soft tissues is prevented. After curing this results in a smooth round

We investigated whether primary (< 24 hours) intramedullary stabilization of femoral shaft fractures in multiple trauma patients with severe thoracic injury might be associated with an increased incidence of adult respiratory distress syndrome (ARDS). A total of 766 patients with multiple trauma admitted to Hannover Medical School between January 1, 1982, and December 31, 1991, were investigated retrospectively. Of these, 106 patients met the inclusion criteria: Injury Severity Score > 18, femoral midshaft fracture treated by intramedullary nailing, primary admission or referral within 8 hours

With regard for the principles that govern the correct application of intramedullary osteosynthesis in the long bones of the lower limb, the authors also support its use in the humerus. After an exhaustive survey of the specific problems at this site, a critical review of 130 cases observed over the last 15 years is reported. Extremely favourable results emerged in the treatment of the humerus with this means of synthesis, thanks to its surgical, mechanical and biological advantages.

A modified Zickel supracondylar rod was used in a prospective study for the fixation of 31 fractures of the proximal humerus in a patient population an average of 55 years of age. The follow-up period averaged 18 months and the overall Neer score at the time of final follow-up examination was 83. Six two-part fractures, ten three-part fractures and fracture dislocations, seven four-part fractures and dislocations, four pathologic fractures, and four humeral shaft fractures were treated with the device. The Neer score did not vary significantly among the groups, although the group with four-par

Sixty-eight patients with fractured femurs were studied who, in terms of personal characteristics, associated injuries and type of fracture, could have been treated by intramedullary nailing or traction followed by the application of either a spica cast or fracture brace. Twenty-eight patients actually underwent intramedullary nailing, while 18 were treated with a spica cast and 22 with a fracture brace. Multiple outcome factors were studied and, based upon every comparison, intramedullary nailing was found superior. Among those patients treated by non-operative techniques, fracture bracing pe

PURPOSE: Posttraumatic bone defects in the setting of severe open injuries of the lower extremity present a significant challenge for orthopaedic trauma surgeons. The induced membrane technique, also known as the Masquelet technique, has been shown to be generally successful in achieving bony union. This video demonstrates the use of the Masquelet technique for a large (18 cm) femoral defect.
METHODS: The Masquelet technique is a 2-stage process. The first stage involves debridement of all devitalized tissue, using open reduction and internal fixation, and placement of a cement spacer with or

BACKGROUND: It is known that posterior malleolar fracture is often associated with tibial diaphyseal fractures. However, in literature there are very few studies on tibial shaft fractures with respect to posterior malleolus fragment. We hypothesized that the incidence of posterior malleolar fracture is higher than in previous studies.
METHODS: A total of 74 closed tibial shaft fractures were treated with intramedullary nailing in Turku University Central Hospital between January 1994 and December 1998. There were 55 men and 19 women, with a mean age of 39 years (range, 16 to 58 years). Two ca

Popliteal cysts may occur with diseases of the knee and may cause several complications. We report about a patient with a popliteal cyst, that contained calcified concrements after a femur fracture and a complex knee injury and that we found to be a rare cause of persisting knee pain and effusions. The etiology of the concrements will be discussed, that we think are more likely to be posttraumatic loose joint bodies than traumatically induced chondromatosis.

BACKGROUND: Olecranon osteotomy (OO) is commonly utilized to improve exposure when treating intra-articular distal humeral fractures. A chevron-shaped osteotomy facilitates reduction and increases surface area for healing1. Following distal humeral fracture reduction and fixation, the OO fragment is fixed with a precontoured plate. The OO technique yields comparable outcomes to alternative techniques1,2.
DESCRIPTION: The technique is performed as follows. (1) Imaging is reviewed and preoperative planning is performed. (2) The patient is positioned in the lateral decubitus position with the op

Introduction: Tibial shaft fractures, particularly those classified as AO 42-C3, represent a significant orthopedic challenge due to their high risk of delayed union or nonunion. Intramedullary nailing is a widely used treatment, though optimizing biological healing remains essential. Platelet-rich plasma (PRP), containing abundant growth factors, has been suggested as a therapeutic aid to enhance fracture healing. Methods: A randomized controlled trial was conducted, including 32 patients diagnosed with acute AO 42-C3 pure diaphyseal tibial fractures. Patients were randomized into two groups:

Proximal fractures of femur represent an important cause of hospitalization, morbidity and mortality for elderly patients. Femoral intramedullary nailing is an option to achieve a stable osteosynthesis but the breakage is a rare complication. The aim was to determinate the prevalence, causes, management and functional outcomes of this patients in our hands. Retrospective review of 1246 proximal femur fractures operated between 2003 and 2012. We analyse epidemiological data, fracture type and the quality of closed reduction and fixation by preoperative and postoperative radiology. Type of salva

A spontaneous subcapital femoral neck fracture is an uncommon complication of a healed intertrochanteric hip fracture. To determine the etiology of this complication, 274 patients who had been treated for intertrochanteric hip fractures were followed up over an 8-year period from June 1988 to June 1996. We found 7 fractures of the subcapital femoral neck without a history of a trauma. The remaining 267 patients were defined as the control group. All seven patients with fractures of the subcapital femoral neck were women, and their average age at the time of compression hip screw (CHS) fixation

BACKGROUND: The influence of bone mineral density (BMD) on hidden blood loss (HBL) in older patients with unstable intertrochanteric fractures treated with intramedullary nailing (IMN) is underexplored. The current retrospective cohort study aimed to evaluate the relationship between BMD and perioperative HBL in these cases.
MATERIALS AND METHODS: A total of 115 patients aged > 60 years undergoing IMN between January 2022 and December 2024 were included. Data included demographics, BMD T score, preoperative hemoglobin (Hb), medical history, and HBL(calculated using the Nadler formula). Spearm

BACKGROUND: Trochanteric femoral fractures are associated with high morbidity and mortality with a substantial proportion of patients presenting with ongoing antithrombotic therapy (ATT). Evidence regarding the impact of ATT on surgical outcomes and complication rates in this population remains limited. The purpose of this study was to evaluate revision rates, infection risk, surgical timing, and mortality in patients with trochanteric fractures receiving ATT. METHODS: We retrospectively analyzed 656 patients who underwent cephalomedullary nailing for trochanteric femoral fractures between Jan

Hip fracture prevention approaches like prophylactic augmentation devices have been proposed to strengthen the femur and prevent hip fracture in a fall scenario. The aim of this study was to validate the finite element model (FEM) of specimens augmented by prophylactic intramedullary nailing in a simulated sideways fall impact against ex vivo experimental data. A dynamic inertia-driven sideways fall simulator was used to test six cadaveric specimens (3 females, 3 males, age 63-83 years) prophylactically implanted with an intramedullary nailing system used to augment the femur. Impact force mea

PURPOSE: The retrograde femoral nailing advanced (RFNA) system (DePuy synthes) is a commonly used implant for the fixation of low distal femur and periprosthetic fractures. There is concern that the rate of distal interlock screw back-out may be higher for the RFNA compared to other nails (ON). The purpose of this study was to evaluate the incidence of interlock screw back-out and associated screw removal for RFNA versus ON, along with associated risk factors.
METHODS: A retrospective comparative study of patients who underwent retrograde nailing for a distal femur fracture at an academic lev
