Guided tissue regeneration in Class II furcation defects using a resorbable polylactic acid barrier.

PURPOSE: To evaluate, clinically, the use of guided tissue regeneration (GTR) with a resorbable polylactic acid membrane (Atrisorb) in the treatment of Class II furcation defects in humans. METHODS: The study included 10 patients with two comparable Class II furcations in lower molars. The lesions were randomly assigned to one of the treatments: GTR with an Atrisorb resorbable membrane (test) or open flap debridement (control). The following clinical parameters were recorded before the surgeries (baseline) and after a healing period of 6 months: relative vertical clinical attachment level, re

Guided tissue regeneration with bioabsorbable and expanded polytetrafluoroethylene barrier membranes in the treatment of naturally occurring periodontal defects in dogs.

The purpose of the present study was to evaluate the healing events and compare the effects of guided tissue regeneration (GTR) using 3 different membranes: a weakly cross-linked bioabsorbable bovine collagen membrane; a strongly cross-linked bioabsorbable bovine collagen membrane; and a non-resorbable ePTFE membrane. Twenty adult female beagle dogs with naturally occurring periodontitis were subjected to periodontal probing and scaling. In both right and left mandibular quadrants, mucoperiosteal flaps were raised, and after debridement, the roots of experimental premolar teeth received apical

Ten-year longitudinal study of periodontal attachment loss in healthy adults.

Despite much research, it is still difficult to make generalizations concerning aging and periodontal diseases. This study examined 95 healthy men and women (aged 29 to 76 years at initial visit) from the oral physiology component of the Baltimore Longitudinal Study of Aging over a 10-year period. Periodontal measurements were taken at two visits, and recession, pocket depth, and level of attachment determined. The change of level of attachment and attachment loss was assessed as a longitudinal measure of disease progression. Overall there were only slight changes in periodontal measurements o

Amoxicillin and metronidazole as an adjunct to full-mouth scaling and root planing of chronic periodontitis.

BACKGROUND: It has been suggested that scaling and root planing of all pockets within a few hours and chlorhexidine treatments (full-mouth disinfection) may reduce the need for supplementary therapies. The aim of this study was to evaluate the clinical benefit of amoxicillin and metronidazole administered immediately after completion of full-mouth periodontal debridement in patients with chronic periodontitis. METHODS: This was a single-center, double-masked, placebo-controlled, randomized longitudinal study of 6 months' duration. Fifty-one patients received full-mouth periodontal debridement

Mandibular incisor extraction: A treatment alternative for large maxillary midline diastema.

Maxillary midline diastema is a common aesthetic concern of dental patients. Various treatment modalities have been employed to close diastemas. This case report describes an unusual orthodontic treatment approach for a 25-year-old African American female patient with a large maxillary midline diastema of 5mm, bialveolar dental protrusion and unilateral Class III malocclusion. The treatment included one mandibular incisor extraction, followed by retraction of the incisors. At the end of the 16-month active treatment period, favourable aesthetic and occlusal outcomes were attained. Closure of m

Implant placement post extraction in esthetic single tooth sites: when immediate, when early, when late?

Implant placement in post-extraction sites of single teeth in the esthetic zone has been a topic of great interest in the field of implant dentistry since 1990. Triggered by the development of guided bone regeneration, the concept of immediate implant placement became quite popular in the 1990s. In the past 12 years, however, the dental community has begun to focus increasingly on the esthetic outcomes of post-extraction implant placement and several studies indicated a significant risk for the development of mucosal recessions with immediate implants. Parallel with this, significant progress

Esthetic treatment of peri-implant soft tissue defects: a case report of a modified surgical-prosthetic approach.

A major esthetic concern is soft tissue defects around implant restorations, which often result in an extra long prosthetic crown. This report describes a modified prosthetic-surgical approach to the treatment of peri-implant horizontal and vertical soft tissue defects in an esthetically demanding patient. One month before surgery, the implant crown restoration was removed, the preexisting implant abutment was reduced, and a short provisional crown, at the level of the homologous contralateral incisor, was applied. A bilaminar technique, consisting of an envelope coronally advanced flap coveri

Histology of connective tissue graft. A case report.

BACKGROUND: Few investigations can be found in the literature on the histological nature of the attachment of connective tissue grafts to root surfaces previously exposed by recession. METHODS: In this case report, a 24-year-old patient was treated with a connective tissue graft combined with a partial-thickness coronally positioned flap for root coverage of Class I Miller recessions at the maxillary right and left canines and first premolars. The treated sites exhibited 83% and 100% root coverage on the right and left sides, respectively. Twelve months later, the case required extraction of

Dynamics of mucosal dimensions after root coverage with a bioresorbable membrane.

BACKGROUND: So far, the clinical effects of the placement of a resorbable membrane for guided tissue regeneration have not been studied in humans in great detail. The dynamics of the resorptive processes, in particular, appear to be rather speculative. In the present longitudinal study, specific alterations of the dimensions of the dentogingival mucosa were explored after surgical root coverage by using a bioresorbable membrane and a coronally-repositioned flap. METHODS: The study population consisted of 14 patients with a total of 31 predominantly shallow, Miller class I, II or III recession

Conservative approach to posttraumatic treatment of maxillary anterior teeth: a clinical report.

This clinical report emphasizes that comprehensive treatment planning, whether simple or complex, requires interdisciplinary knowledge. Conservative treatment was implemented for this patient, a 29-year-old woman with a posttraumatic anterior open occlusal relationship, crowding, and a shifted midline. The treatment plan involved selective occlusal grinding, adjunctive orthodontics, periodontal surgery, and cosmetic dentistry and proved to be highly successful.

Periodontal repair in dogs: effect of wound stabilization on healing.

This study evaluated a biodegradable polylactic acid matrix as a wound stabilizing implant in reconstructive periodontal surgery. Supra-alveolar circumferential periodontal defects, 5 to 6 mm large, were surgically created around the mandibular premolars in 7 beagle dogs. The root surfaces in left and right jaw quadrants were treated with either heparin or saline. In this model, root surface treatment with heparin compromises periodontal repair and results in a long junctional epithelium and a reduced connective tissue repair to the root surface, whereas saline treatment results in almost comp
