AI without reach: The last mile problem in preventive public health.

OBJECTIVES: Examine the "last mile problem" in artificial intelligence (AI) applications for preventive public health. STUDY DESIGN: Conceptual short communication introducing a five-stage pathway to examine where AI-enabled prevention breaks down. METHODS: This draws on emerging literature in AI, health services research, and implementation science to distinguish between predictive capacity and real-world impact. A five-stage pathway model of prediction, translation, delivery, uptake, and sustainment is introduced to characterize where breakdowns occur between identifying risk and changing

A Sobol-Driven Multi-Objective Whale Migration Algorithm for Engineering Optimization.

Multi-objective optimization plays an important role in modern design and complex engineering applications. However, achieving an effective balance between the convergence and diversity of Pareto-optimal solutions remains challenging. This paper proposes a Sobol-driven Multi-objective Whale Migration Algorithm (SMOWMA), which extends the Whale Migration Algorithm within a non-dominated sorting and elite-selection framework. A maximin scrambled Sobol initialization scheme is first employed to improve the distribution of the initial population. An archive-guided adaptive Student-t flight mechani

Langevin and Fokker-Planck analyses for diffusion-mediated passing of circular and discorectangular species in two-dimensional channels.

The propensity for pairs of diffusing species to pass each other within narrow channels or pores is of basic interest as a first-passage-type problem. It is also of relevance for solution-phase transport in nanoporous materials, and in particular for catalytic conversion reactions where high yield requires that product species can efficiently pass reactant species to exit the pores. We analyze a two-dimensional model with nonoverlapping circular and discorectangular species confined to a rectangular channel, and where passing is mediated by Brownian dynamics in an implicit solvent. For narrowe

Social Cognitive Deficits and Altered Multibrain Dynamics During Problem Solving in Heroin Abstainers: A Functional Near-Infrared Spectroscopy Hyperscanning Study.

BACKGROUND: Despite extensive research on the neurobiology of addiction, little is known about how repeated drug use and withdrawal are related to social functioning impairments in humans, a highly social species. This obscures the broader societal impact of drug addiction and limits treatment efficacy. This study examined social cognitive impairment and its multibrain neural underpinnings during socially interactive problem solving in heroin use disorder (HUD) and further explored their co-occurrence with protracted withdrawal symptoms. METHODS: Thirty-eight HUD pairs and 30 healthy control

A descriptive study of gambling practices and problem gambling among internal migrants in Muse, Northern Shan, Myanmar.

Due to rapid economic growth in China, many gambling sites have emerged along the neighboring countries, including Myanmar, which place migrants at risk of developing gambling problems. This study aimed at identifying: i) types of gambling, ii) gambling involvement and intensity, and iii) the associations with problem gambling. A community-based cross-sectional study was conducted in Muse, Myanmar. A convenience sample of 273 internal migrants (18-49 years) filled a self-administered questionnaire. Chi-square tests and multiple logistic regression explored associations between gambling practic

Family communication with teens at clinical high-risk for psychosis or bipolar disorder.

Previous research has found that family problem-solving interactions are more constructive and less contentious when there is a family member with bipolar disorder compared with schizophrenia. The present study extended this research by examining whether family problem-solving interactions differ between clinical high-risk (CHR) stages of each illness. Trained coders applied a behavioral coding system (O'Brien et al., 2014) to problem-solving interactions of parents and their adolescent child, conducted just prior to beginning a randomized trial of family-focused therapy. The CHR for psychosis

Gambling goals predict chasing behavior during slot machine play.

AIMS: The purpose of this study was to test the effect of gambling goals (i.e., gambling achievement-orientation) on chasing behavior (i.e., decision to chase, chasing spins) over and above known antecedents (e.g., problem gambling severity, winning money motivations, approach/avoidance motivation). METHODS: Young adult gamblers (N=121) were provided $20 and invited to use those funds on a slot machine situated in an immersive virtual reality casino. Unbeknownst to participants, outcomes were manipulated such that a nominal amount of money was either won or lost (depending on experimental con

Varieties of Angry Experience: Interpersonal Construct Validity of the Displaced Aggression Questionnaire and Its Alteration Via Partialing.

The Displaced Aggression Questionnaire (DAQ) assesses the trait tendency to aggress against innocent targets, following unrelated provocation. In a primary aim, interpersonal concepts and methods and other measures were used to evaluate the construct validity of DAQ subscales (i.e., angry rumination, revenge planning, behavioral displaced aggression). A second aim evaluated the extent to which partialing altered interpersonal and other correlates of the DAQ subscales. A representative online sample of 999 U.S. adults completed the DAQ, interpersonal circumplex measures of trait social behavior

Association between Self-Perception of Chewing, Chewing Behavior, and the Presence of Gastrointestinal Symptoms in Candidates for Bariatric Surgery.

Given the changes in the digestive tract post-bariatric surgery, adapting to a new pattern of eating behavior becomes crucial, with special attention to the specifics of chewing mechanics. This study aimed to investigate the association between self-perception of chewing, chewing behavior, and the presence of gastrointestinal symptoms in preoperative patients undergoing bariatric surgery. Sixty adult candidates for bariatric surgery at a public hospital in Belém (Brazil) were analyzed. Participants predominantly exhibited unilateral chewing patterns (91.6%), a fast chewing rhythm (73.3%), a la

A pilot study of the association between sleep disturbance in children with liver transplants and parent and family health-related quality of life.

OBJECTIVE: To investigate the relationship between sleep disturbance in children with liver transplants and parent and family health-related quality of life (HRQOL). METHOD: 47 parents of children with liver transplants completed measures of child sleep and family HRQOL. Relationships between sleep and HRQOL and differences in HRQOL between groups with scores above and below the cutoff on a pediatric sleep measure were examined. RESULTS: Parents endorsed higher rates of sleep-related breathing disorder (SRBD) and restless legs syndrome and periodic limb movements during sleep (RLS/PLMS) and

Developmental and contextual considerations for adrenal and gonadal hormone functioning during adolescence: Implications for adolescent mental health.

Substantial research has implicated the hypothalamic-pituitary-adrenal (HPA) and hypothalamic-pituitary-gonadal (HPG) axes independently in adolescent mental health problems, though this literature remains largely inconclusive. Given the cross-talk between the HPA and HPG axes and their increased activation in adolescence, a dual-axis approach that examines both axes simultaneously is proposed to predict the emergence and persistence of adolescent mental health problems. After briefly orienting readers to HPA and HPG axis functioning, we review the literature examining associations between hor

Individual differences in the development of self-regulation during pre-adolescence: connections to context and adjustment.

Difficulties with self-regulation are implicated in the development of emotional and behavioral problems during adolescence. Although children's ability to regulate their behaviors continues to improve throughout childhood and adolescence, it remains unclear how contextual risk factors might influence this development during the transition to adolescence, or how variation in the development of self-regulation predicts adjustment. Using a community sample of 214 8-12 year-olds (T1 M = 9.5, SD = 1.01), we examined growth trajectories of effortful control and impulsivity over three years and test

Neuropsychological findings in childhood narcolepsy.

Narcolepsy with cataplexy is a severely disabling disorder very often arising in childhood. Data on neuropsychological impairment in children are scant. We administered standardized neuropsychological tests to 13 children with narcolepsy with cataplexy. Overall, our patients displayed multiple patterns of cognitive and behavioral dysfunction, and often academic failure (7 cases out of 13). All children had a normal full intelligence quotient (IQ), but 3 patients presented a significantly higher and 2 a significantly lower Verbal IQ compared to Performance IQ, respectively. Mean sleep latency w

Validity of the Externalizing Spectrum Inventory in a criminal offender sample: relations with disinhibitory psychopathology, personality, and psychopathic features.

The Externalizing Spectrum Inventory (ESI; Krueger, Markon, Patrick, Benning, & Kramer, 2007) provides a self-report based method for indexing a range of correlated problem behaviors and traits in the domain of deficient impulse control. The ESI organizes lower order behaviors and traits of this kind around higher order factors encompassing general disinhibitory proneness, callous-aggression, and substance abuse. In the current study, we used data from a male prisoner sample (N = 235) to evaluate the validity of ESI total and factor scores in relation to external criterion measures consisting …

[Gastrointestinal abnormalities in children with autism].

The autistic spectrum disorder (ASD) is a neurodevelopmental disorder characterized by socially aloof behavior and impairment of language and social interaction. This paper is a review of literature on gastrointestinal problems in children with ASD. Gastrointestinal symptoms are described in 9-54% of autistic children, among which most common are: constipation, diarrhea and abdominal distension. The gastro-intestinal abnormalities reported in autism include: inflammation (esophagitis, gastritis, duodenitis, enterocolitis) with or without autoimmunity, lymphoid nodular hyperplasia, increased in

Differences in endocrine parameters and psychopathology in girls with premature adrenarche versus on-time adrenarche.

Girls with premature adrenarche (PA) are at risk for multiple problems related to exaggerated androgen synthesis. Whether PA carries a risk of psychopathology remains unknown. This study examined group differences in: (a) anthropometric and endocrine parameters, and (b) mood and behavior problems, in 6-8 year-old girls with PA (n = 40) compared to on-time adrenarche girls (n = 36). PA girls were taller (p < or =0.05) and heavier (p < or =0.01) than the on-time adrenarche girls but body mass index showed no difference. PA girls had significantly (p <0.05) higher adrenal androgen and testosteron

Impulsivity in women with eating disorders: problem of response inhibition, planning, or attention?

OBJECTIVE: Impulsivity is generally believed to be more characteristic of individuals with bulimic than with restrictive eating disorders (EDs). However, studies have not exhaustively explored the association between EDs and various component dimensions of the impulsivity construct. METHOD: We conducted a multidimensional assessment of impulsivity in 84 women with bulimia nervosa (BN), 37 with anorexia nervosa (AN: 19 restricters and 18 bingers-purgers), and 61 normal-control participants. To assess multiple components of impulsivity, participants completed a battery of self-report questionna

Adolescents at risk for drug and alcohol abuse.

Although all children have the potential for becoming destructively involved with psychoactive drugs, there is considerable evidence that youngsters with particular kinds of psychologic and family problems are at high risk for chemical dependency. These include youngsters with developmental deficiencies that interfere with their capacity to master the environment. Children with a strong family history of alcoholism or drug abuse also seem to be at high risk. Obviously, there is some overlap between these two groups, not only because parental drug abuse and alcoholism may damage the fetus, but

Exposure to Family Violence and Internalizing and Externalizing Problems Among Spanish Adolescents.

Exposure to intimate partner violence (IPV) and child maltreatment may have devastating consequences on children's development. The aim of this research was to examine the predictive associations between exposure to violence at home (witnessing violence against the mother and/or direct victimization by the parents) and adolescent internalizing and externalizing problems. A total of 613 Spanish adolescents (13-18 years) took part in this study. Results indicate that psychological victimization by the parents predicted an increase in anxious/depressive symptoms, aggressive and rule-breaking beha
