OBJECTIVE: Venoarterial extracorporeal membrane oxygenation (ECMO) is a salvage therapy in patients with severe cardiopulmonary failure. Owing to the large size of the cannulas inserted via the femoral vessels (≤24-F) required for adequate oxygenation, this procedure could result in significant limb ischemic complications (10%-70%). This study evaluates the results of a distal limb perfusion arterial protocol designed to reduce associated complications.
METHODS: We conducted a retrospective institutional review board-approved review of consecutive patients requiring ECMO via femoral cannulati

Obstructive sleep apnea syndrome (OSA) and restless legs syndrome (RLS) with periodic limb movement during sleep (PLMS) are two sleep disorders characterized by repetitive respiratory or movement events associated with cortical arousals. We compared the cerebral hemodynamic changes linked to periodic apneas/hypopneas with arousals (AHA) in four OSA-patients with periodic limb movements (PLMA) with arousals in four patients with RLS-PLMS using near-infrared spectroscopy (NIRS). AHA induced homogenous pattern of periodic fluctuations in oxygenated (HbO2) and deoxygenated (HHb) hemoglobin, i.e.,

BACKGROUND: Monitoring of the microcirculation may add additional information in terms of improving the adequacy of hemodialysis (HD) for patients. Withdrawal of liquid and complement activation during a HD session reduces the external pressure on the microcirculation and leads to an increased dilatation of the peripheral capillaries. The purposes of this study were to assess the effect of a single HD or hemodiafiltration session on the thenar microcirculation in patients with end-stage renal disease (ESRD) with or without diabetes, investigate the possible relationship between changes in the

AIMS: Preterm infants can be inadvertently exposed to high tidal volumes (V(T)) in the delivery room, causing lung inflammation and injury, but little is known about their effects on the brain. The aim of this study was to compare an initial 15 min of high V(T) resuscitation strategy to a less injurious resuscitation strategy on cerebral haemodynamics, inflammation and injury.
METHODS: Preterm lambs at 126 d gestation were surgically instrumented prior to receiving resuscitation with either: 1) High V(T) targeting 10-12 mL/kg for the first 15 min (n = 6) or 2) a protective resuscitation strat

PURPOSE: The endovascular repair (EVAR) of an abdominal aortic aneurysm (AAA) with a bilateral common iliac artery aneurysm (CIAA) often requires exclusion of the bilateral hypogastric artery (HA), which can be associated with pelvic ischemic complications such as erectile dysfunction and buttock claudication. This study assessed the effect of HA bypass on improving pelvic circulation.
METHODS: Five patients who underwent endovascular repair with HA bypass for an AAA with bilateral CIAA were evaluated. In all patients, the patency of the inferior mesenteric artery and bilateral HAs arteries w

The survival of infants with congenital heart disease has improved dramatically. However, the incidence of neurological injury in infants surviving cardiac surgery remains considerable. These neurological sequelae are attributable at least in part to hypoxia-ischemia/reperfusion, which inevitably accompanies infant heart surgery with deep hypothermia, cardiopulmonary bypass, and circulatory arrest. To begin to identify mechanisms of brain injury during infant cardiac surgery, we used near-infrared spectroscopy to study the relationship between cerebral intravascular (hemoglobin) and mitochondr

OBJECTIVES: The short- and long-term effects of valvuloplasty on exercise capacity, ventilation and skeletal muscle oxygenation were investigated to determine whether a dissociation between hemodynamic improvement and exercise capacity occurs in patients with mitral stenosis.
BACKGROUND: Percutaneous balloon mitral valvuloplasty in patients with mitral stenosis results in immediate hemodynamic improvement at rest and with exercise. Improved exercise capacity has been described at 3 months after valvuloplasty. In patients with left ventricular dysfunction, acute therapeutic interventions that

OBJECTIVE: To investigate whether or not postasphyctic cerebral hypoperfusion and decreased cerebral metabolism occur in the perinatally asphyxiated neonate, as has been reported in adults and newborn animals.
METHODS: Using near-infrared spectroscopy, we monitored changes in oxyhemoglobin (HbO2), deoxyhemoglobin (HbR), total hemoglobin (HbO2 + HbR, which represents changes in cerebral blood volume [CBV]), and cytochrome oxidase (Cytaa3, which indicates changes in oxidation level of this intracerebral mitochondrial enzyme). Thirty-one neonates (gestational age > 34 weeks), divided into three

Although the treatment of acute coronary syndromes (ACS) has advanced considerably, the ability to detect, predict, and prevent complications of atherosclerotic plaques, considered the main cause of ACS, remains elusive. Several imaging tools have therefore been developed to characterize morphological determinants of plaque vulnerability, defined as the propensity or probability of plaques to complicate with coronary thrombosis, able to predict patients at risk. By utilizing both intravascular and noninvasive imaging tools, indeed prospective longitudinal studies have recently provided conside

Traumatic brain injury (TBI) is a major medical and socio-economic problem, and is the leading cause of death in children and young adults. The critical care management of severe TBI is largely derived from the "Guidelines for the Management of Severe Traumatic Brain Injury" that have been published by the Brain Trauma Foundation. The main objectives are prevention and treatment of intracranial hypertension and secondary brain insults, preservation of cerebral perfusion pressure (CPP), and optimization of cerebral oxygenation. In this review, the critical care management of severe TBI will be

OBJECTIVES: The lack of accurate measurement of hemodynamics and oxygen transport has limited our understanding of Norwood physiology and postoperative management. We used measured oxygen consumption to characterize hemodynamics and oxygen transport after the classic Norwood procedure.
METHODS: Fourteen neonates had continuous respiratory mass spectrometry to measure oxygen consumption (VO2). Arterial, superior vena caval, and pulmonary venous saturations were measured at 2- to 4-hour intervals for 72 hours postoperatively. Systemic (Qs) and pulmonary (Qp) blood flows, systemic vascular resis

Winter wheat is grown worldwide primarily for grain, yet it also serves as an important cool-season forage, supporting livestock production when other feed is scarce. Wheat can be utilized as grazed pasture or harvested as hay, silage, and straw, and is widely used as dual-purpose (forage + grain) and double-cropping systems. During vegetative growth, wheat forage is highly palatable and nutritive, typically containing 20-30% crude protein (CP), and >80% digestibility, supporting rapid liveweight gains in beef cattle. However, forage quality is highly dynamic: as wheat matures, CP and energy d

OBJECTIVES: Speech recognition outcomes vary widely in cochlear implant recipients, but the reasons for this variability are not well explained by patient history factors alone. Neuroplastic reorganization after both deafness and cochlear implant switch-on can affect how new recipients adapt to electrical hearing. In this study, we evaluate the associations between speech recognition outcomes and stimulus-evoked activation of cortical language processing areas.
DESIGN: Forty-three newly implanted post-lingually deaf adult cochlear implant recipients (17 female, mean age at implantation: 58.5

BACKGROUND: Balance dysfunction in older adults is a major risk factor for falls. Conventional subjective scales and single-modality measures may not adequately capture the integrated motor, cortical, and cognitive processes underlying postural control. Using a multimodal feature set, this study aimed to develop an interpretable framework for dysfunction-related classification and for exploring heterogeneity in balance-related functional patterns.
METHODS: A total of 81 community-dwelling older adults (≥ 60 years old) were recruited and divided into balance dysfunction group (n = 38, BBS ≤ 45

During spaceflight, the lack of gravity-induced hydrostatic pressure gradients in the body results in a headward fluid shift hypothesized to play a role in pathophysiological changes in the cardiovascular, ocular, and central nervous systems. Here, 24 subjects (n = 8 control) underwent 60 days of strict 6° head-down tilt (HDT) bedrest, an analog of spaceflight where we tested the ability of daily 30-min artificial gravity (AG) exposure via short-arm human centrifugation (intermittent AG, n = 8, and continuous AG, n = 8) to reverse the HDT-induced headward fluid shift.
