Evaluation of the Use of CT Attenuation for the Prediction of Subsequent Vertebral Fracture in Patients with Osteoporosis.

BACKGROUND: Subsequent vertebral fracture (SVF) is one of the most common complications of percutaneous vertebral augmentation (PVA), which leads to lower back pain in patients. Low bone mineral density (BMD) is an independent risk factor for SVF. BMD measured using computed tomography (CT) trabecular attenuation correlates closely with BMD. OBJECTIVES: This study aims to analyze the risk factors of SVF after PVA and to estimate the predictive role of CT trabecular attenuation. STUDY DESIGN: A retrospective review. SETTING: Department of spinal surgery in an affiliated hospital of a medical

Fatal cobalt toxicity after total hip arthroplasty revision for fractured ceramic components.

CONTEXT: Post-arthroplasty metallosis, which refers to metallic corrosion and deposition of metallic debris in the periprosthetic soft tissues of the body, is an uncommon complication. Systemic cobalt toxicity post-arthroplasty is extremely rare. The few known fatal cases of cobalt toxicity appear to be a result of replacing shattered ceramic heads with metal-on-metal or metal-on-polyethylene implants. Friction between residual shards of ceramic and cobalt-chromium implants allows release of cobalt into the synovial fluid and bloodstream, resulting in elevated whole blood cobalt levels and pot

Association between screw angulation and union after inverted-triangle cannulated screw fixation of femoral neck fractures.

PURPOSE: The aim of this study was to evaluate whether screw angulation relative to the inter-ischial(horizontal) axis and femoral anatomical axis was associated with union after inverted-triangle cannulated screw fixation of femoral neck fractures and to explore angle intervals associated with higher union rates. METHODS: Between January 2017 and December 2022, 83 patients operated on with closed reduction-percutaneous cannulated screws by the same surgical team were retrospectively analyzed. The angles of the calcar, anterosuperior, and posterosuperior screws were measured according to both

Comparison of Incremental Costs and Medicare Reimbursement for Intra- Versus Extra-Articular Distal Radius Fracture Surgery Using Time-Driven Activity-Based Costing.

PURPOSE: Balancing surgical costs of care with reimbursements is important for the solvency of surgical practices and cost containment in the health care system. This study aimed to compare the day of surgery costs and incremental Medicare reimbursement for open reduction and internal fixation (ORIF) of intra-articular and extra-articular distal radius fractures (DRFs). METHODS: Using time data and supply costs at a single, large academic health center, we used a time-driven activity-based costing analysis to calculate the total day of surgery costs for ORIF of DRFs for the three current proc

Early Outcome of Distal Femur Fracture Fixation in Adults: A Comparative Study Between Dynamic Condylar Screw and Distal Femoral Locked Plate.

BACKGROUND: Distal femoral fractures are difficult to treat due to their proximity to the knee joint, and different modalities have been developed over the years to overcome these challenges. Some studies have shown variable results of different operative modalities. However, there is little evidence to show the superiority of one over the other. OBJECTIVE: To compare the functional outcome of operative treatment between dynamic condylar screw (DCS) with a side plate and distal femoral locked compression plate (DFLCP) using the Schatzker and Lambert criteria. The mean time to union and compli

Peroneal artery safe zone in the posterolateral approach to the distal tibia: A CT angiogram based anatomical study.

OBJECTIVES: The posterolateral approach to the distal tibia is commonly used for stabilisation of ankle fractures as it allows good visualisation and direct reduction of the posterior distal tibia and malleolar fragments. This approach can also be used for internal fixation of an associated lateral malleolus fracture. The aim of our study is to describe the surgical anatomy of the peroneal artery (PA) in relation to the tibial plafond and the distal fibula; thereby suggesting a safe zone during proximal dissection of posterolateral approach. METHODS: Using Computed Tomography Angiographic (CT

Surgical Site Infection In Orthopaedic Implants And Its Common Bacteria With Their Sensitivities To Antibiotics, In Open Reduction Internal Fixation.

BACKGROUND: Surgical site infection in orthopaedic implants is a major problem, causing long hospital stay, cost to the patient and is a burden on health care facilities. It increases rate of nonunion, osteomyelitis, implant failure, sepsis, multiorgan dysfunction and even death. Surgical site infection is defined as pain, erythema, swelling and discharge from wound site. Surgical site infection in orthopaedic implants is more challenging to the treating orthopaedic surgeon as the causative organism is protected by a biofilm over the implant's surface. Antibiotics cannot cross this film to rea

Primary Wrist Hemiarthroplasty for Irreparable Distal Radius Fracture in the Independent Elderly.

Background Volar plating for acute distal radius fractures (DRF) in the elderly has been recommended. Some studies have suggested that open reduction with internal fixation (ORIF) in this situation results in frequent complications. Our purposes were to provide a definition of irreparable DRF in independent elderly patients and to review the results of a preliminary retrospective series of wrist hemiarthroplasty (WHA) in this patient population. Materials Between 2011 and 2014, 11 consecutive independent elderly patients (12 wrists) with irreparable intra-articular DRF were treated with primar

An economic analysis of outcomes and complications of treating distal radius fractures in the elderly.

PURPOSE: There is a lack of scientific data regarding which treatment provides the best outcome for distal radius fractures (DRFs) in the elderly. Currently, casting is used to treat the majority of these fractures, although open reduction and internal fixation (ORIF) has been used increasingly in recent years. Given the recent emphasis on the wise use of medical resources, we conducted a cost-utility analysis to assess which of 4 common DRF treatments (casting, wire fixation, external fixation, or ORIF) optimizes the cost-to-patient preference ratio. METHODS: We created a decision tree to mo

Distal Radius Fracture Fixation Using a Tourniquet versus Local Lidocaine with Epinephrine: A Prospective Randomized Trial.

PURPOSE: Our objective was to determine the effect of a tourniquet on postoperative pain and function following open reduction and internal fixation (ORIF) of the distal radius. We hypothesized that patients treated with local lidocaine and epinephrine and no-tourniquet would have improved postoperative pain and function when compared with patients treated with standard tourniquet use. MATERIALS AND METHODS: This was a prospective, blinded randomized trial of 35 patients aged 18 to 85 who sustained acute closed distal radius fractures treated with a volar plate. Randomization placed 16 patien

Biomechanical characteristic differences of two new types of intramedullary nail devices in the treatment of comminuted intertrochanteric fractures of femur: a comparative study based on finite element analysis.

OBJECTIVE: Given the recent application of two new types of intramedullary nail devices in the treatment of comminuted femoral intertrochanteric fractures (CFIFs), there is still a lack of deep understanding and comparative evaluation of their biomechanical properties. Therefore, this study aims to systematically compare the advantages and disadvantages of these two new devices with traditional proximal femoral nail antirotation (PFNA) and InterTan nails in the fixation of CFIFs through finite element analysis. METHODS: Based on the validated finite element model, this study constructed an ac

Approaches to Distal Upper-Extremity Trauma: A Comparison of Plastic, Orthopedic, and Hand Surgeons in Academic Practice.

BACKGROUND: Hand trauma call duties at university medical centers are traditionally split among plastic surgeons and orthopedic surgeons, frequently without additional fellowship training in hand and upper-extremity surgery. Differences in operative approach between these groups have never been specifically described. The University Health Consortium-Association of American Medical Colleges Faculty Practice Solutions Center database contains comprehensive, factual, billing and coding data from 90 academic medical centers in the United States and can be used to characterize the practice pattern

Thyroid storm during the recovery phase after non-thyroid surgery in a hyperthyroid patient: a case report and literature review.

BACKGROUND: Thyroid storm is a life-threatening endocrine emergency characterized by an acute exacerbation of thyrotoxicosis, often triggered by stressors such as surgery or infection, with a mortality rate of 8%-25%. Although the risk is well-documented in thyroid surgeries, perioperative thyroid storm following non-thyroid procedures is exceedingly rare, posing diagnostic and therapeutic challenges. This case report and literature review aim to highlight the clinical features and management strategies for perioperative thyroid storm in non-thyroid surgical patients through a case analysis an

Takotsubo syndrome as an overlooked and elusive cause of a single episode of dyspnea in young women: a case report.

BACKGROUND: Dyspnea is a common symptom in patients presenting to the emergency department. It has a variety of causes that range from non-urgent to life-threatening. One episode of dyspnea in a healthy young person is easy to overlook. However, if the symptoms occur after physically or emotionally stressful events, careful evaluation needs to be undertaken because it may be associated with Takotsubo syndrome, which is rarely expected but can be fatal. Herein, we report the case of Takotsubo syndrome in a healthy young woman who arrived at the emergency department after experiencing a short si

Penile density and globally used chemicals in Canadian and Greenland polar bears.

Industrially produced chemicals have been a major environmental concern across our entire Globe since the onset of rapid industrial development around the early 1900. Many of the substances being used are known to be endocrine disrupting chemicals (EDCs) and are also known to be long-range dispersed and to biomagnify to very high concentrations in the tissues of Arctic apex predators such as polar bears (Ursus maritimus). A major concern relating to EDCs is their effects on vital organ-tissues such as bone and it is possible that EDCs represent a more serious challenge to the species' survival
