Chronic Hand Eczema Guidelines From an Expert Panel of the International Eczema Council.

BACKGROUND: Assessment of chronic hand eczema (CHE) is complex and warrants standardization. OBJECTIVE: We sought to guide clinicians on the assessment of CHE. METHODS: An electronic questionnaire regarding the diagnosis and assessment of CHE was completed by councilors (n=45) of the International Eczema Council, an international group of clinicians and researchers with expertise in CHE. The survey consisted of 52 statements for consensus. RESULTS: Overall, nine statements (17.3%) had strong, twenty-three (44.2%) moderate, 12 (23.1%) low, and 8 (15.4%) very low levels of agreement. Five sta

Reliability and Longitudinal Course of Itch/Scratch Severity in Adults With Atopic Dermatitis.

BACKGROUND: Itch is a complex and burdensome symptom in atopic dermatitis (AD). Severity of scratching/excoriation (SCORAD-scratch) has been found to be a valid measure of itch in AD. However, little is known about the longitudinal course of scratching/excoriations in AD. METHODS: A prospective, dermatology practice-based study was performed of adults with AD (N = 399). The patients were assessed at baseline and approximately 6, 12, 18, and 24 months. RESULTS: Severity of excoriations correlated best with the Numerical Rating Scale-worst itch (Spearman correlation, ρ = 0.50), followed by a P

Mental Health Symptoms and Functional Impairment in Children With Atopic Dermatitis.

BACKGROUND: The impact of childhood atopic dermatitis (AD) on social and behavioral issues is not well understood. OBJECTIVE: This study sought to determine the prevalence and predictors of social and behavioral symptoms and functional impairment among US children with AD. METHODS: The 1996 to 2015 Medical Expenditure Panel Surveys were analyzed, including a representative, cross-sectional study of 2553 US children with AD. Behavioral and functional issues were examined using Columbia Impairment Scale (CIS) scores. RESULTS: Childhood AD was associated with behavioral and functional problems

Atopic Dermatitis Is Associated With Multiple Behavioral Problems in US Children and Adolescents.

BACKGROUND: Atopic dermatitis (AD) is a chronic, inflammatory skin disease associated with itch, sleep disturbance, psychosocial distress, anxiety, and depression. OBJECTIVE: We aimed to understand the association between AD and aberrant childhood behaviors. METHODS: We used data from the Fragile Families and Child Wellbeing Study, a longitudinal birth cohort study of 4898 urban children. RESULTS: Atopic dermatitis was associated with the 75th or greater percentile of mean behavioral scores at 5 years (multivariable logistic regression; adjusted odds ratio [aOR] [95% confidence interval {95

Psoralen-Induced Phytophotodermatitis.

Phytophotodermatitis is a cutaneous reaction that occurs after exposure to plant-derived furocoumarins and ultraviolet A light. Psoralen is the most common phototoxic furocoumarin and is present in varying levels within many different plant species. This article focuses on the diagnosis and management of psoralen-induced phytophotodermatitis along with other clinical applications.

Dupilumab in Elderly Patients With Severe Atopic Dermatitis.

BACKGROUND: Atopic dermatitis (AD) in the elderly has been poorly investigated, although its incidence is gradually increasing mainly in industrialized countries. Age-specific factors in older patients must be considered when selecting treatment options. OBJECTIVES: To evaluate the efficacy and tolerability of dupilumab in treating elderly patients with severe AD. METHODS: This was a retrospective, multicenter study involving 26 elderly patients (age, ≥65 years) with severe AD who were treated with dupilumab for at least 16 weeks. Absolute and percentage frequencies were used to evaluate qua

Fragrance Skin Sensitization Evaluation and Human Testing: 30-Year Experience.

BACKGROUND: The human repeated insult patch test (HRIPT) has a history of use in the fragrance industry as a component of safety evaluation, exclusively to confirm the absence of skin sensitization at a defined dose. OBJECTIVE: The aim of the study was to document the accumulated experience from more than 30 years of conducting HRIPTs. METHODS: A retrospective collation of HRIPT studies carried out to a consistent protocol was undertaken, with each study comprising a minimum of 100 volunteers. CONCLUSIONS: The HRIPT outcomes from 154 studies on 134 substances using 16,512 volunteers were ob

Wearable Woes: Allergens in Diabetic Devices.

Novel diabetic devices are being developed to help manage diabetes and improve the quality of life of patients with diabetes. Both insulin pumps and glucose monitors are becoming increasingly convenient, long-lasting, and discrete for patients, but this often requires the use of strong external cutaneous adhesives and increased contact time with the skin. As a consequence, these devices have been associated with a variety of dermatologic reactions, namely, irritant and allergic contact dermatitis. Some of these reactions can be severe, precluding the use of these devices, which puts patients'

Formaldehyde Release From Predispersed Tattoo Inks: Analysis Using the Chromotropic Acid Method.

BACKGROUND: Allergic contact dermatitis to tattoo ink may last from weeks to years. Formaldehyde is a strong sensitizer that may be present in predispersed tattoo inks. OBJECTIVES: The aim of this study was to evaluate the presence of formaldehyde in predispersed tattoo inks using the chromotropic acid method. METHODS: Tattoo inks from 39 companies were evaluated. Inclusion criteria included availability to purchase inks online through US tattoo product wholesalers or individual Web sites. Brands were grouped based on prevalence of use: common, uncommon, or rare. For common brands, 8 colors

Usefulness of Patch Testing With Patient's Own Products in the Diagnosis of Allergic Contact Dermatitis.

BACKGROUND: The usefulness of using patient's own products in patch tests for the diagnosis of allergic contact eczema is well known. However, most of the literature is based on case series published decades ago, and they are focused on cosmetics and fragrances. OBJECTIVE: The aim of the study was to evaluate the usefulness of using patient's own products in patch tests for the diagnosis of contact eczema in a contact dermatitis unit, describing the most frequently positive own products, as well as the most frequently responsible allergens. METHODS: In a 17-year period, 3514 patients were pa

Nickel: Intrinsic Skin Sensitization Potency and Relation to Prevalence of Contact Allergy.

Nickel remains the most commonly identified contact allergen. However, it has proven difficult to demonstrate significant skin-sensitizing activity for nickel in toxicology tests, which typically have indicated a weak skin sensitization potential. Information indicates that in vivo assays are not predictive of dermal sensitization hazard or potency for nickel due to a human-specific mechanistic route for nickel sensitization that animals lack. A similar rationale will apply to in vitro alternatives-although these currently have limited ability to determine intrinsic potency. Generally, in sili

Can We Separate Oral Lichen Planus from Allergic Contact Dermatitis and Should We Patch Test? A Systematic Review of Chronic Oral Lichenoid Lesions.

This systematic review summarizes characteristics and treatment outcomes of dental amalgam-associated oral lichenoid lesions (OLLs) and oral lichen planus (OLP). Embase and MEDLINE were searched for original studies on OLLs or OLP associated with dental amalgam. Data extraction was completed from 44 studies representing 1855 patients. Removal of amalgam restorations led to complete resolution in 54.2% (n = 423/781), partial resolution in 34.8% (n = 272/781), and no resolution in 11.0% (n = 86/781) of the patients with OLLs, whereas complete resolution occurred in 37.1% (n = 72/194), partial re
