Gait analysis and muscle weight analysis after lower extremity fractures in a small animal model.

BACKGROUND: Besides adequate healing of bone and soft tissues, mobility represents a significant factor in functional outcome after lower extremity fractures. Although gait analysis is gaining clinical interest and importance in the rehabilitation of patients with fractures, it is rarely used in experimental fracture healing research. The aim of this study is to establish an accurate gait analysis method for fracture healing research in small animal models and to evaluate the influence of a lower extremity fracture on gait pattern and muscle atrophy in rats. RESEARCH QUESTION: How does an int

Risk of Knee Sepsis After Treatment of Open Tibia Fractures: A Multicenter Comparison of Suprapatellar and Infrapatellar Approaches.

OBJECTIVES: The suprapatellar approach for medullary nailing of the tibia is increasing. This requires intra-articular passage of instruments, which theoretically places the knee at risk of postoperative sepsis in the setting of open fracture. We hypothesized that the risk of knee sepsis is similar after suprapatellar or infrapatellar nailing of open tibia fractures. DESIGN: Retrospective, multicenter. SETTING: Three urban level 1 trauma centers. PATIENTS: All patients treated with medullary nailing for open diaphyseal tibia fractures (OTA 42) from 2009 to 2015. Patients younger than 18 yea

Comparison of tissue oxygenation and compartment pressure following tibia fracture.

OBJECTIVE: We investigated the ability of direct continuous measurement of intramuscular tissue oxygenation (PmO(2)) to detect acute ischaemia in the leg in patients at risk for acute extremity compartment syndrome. Following tibia fracture treated by intramedullary nailing, we compared the proportions of PmO(2) and compartment pressure (CP) measurements that met the warning criteria for compartment syndrome. METHODS: Participants included 10 patients sustaining acute isolated closed tibia shaft fractures treated by intramedullary nailing. A tissue oxygenation probe and a CP probe were percut

Intramedullary osteosynthesis of complex proximal femoral fractures with the Targon PF nail.

We evaluated the performance of the Targon PF nailing system in the treatment of complex proximal femoral fractures with a retrospective analysis of prospectively collected data for 94 patients with a complex proximal femoral fracture (subtrochanteric fracture or fracture with reversed fracture line pattern) treated with a Targon PF nail. There were no intra-operative femoral fractures. Late complications requiring reoperation were one femoral fracture and one fracture around the nail secondary to tumour. Seven nails had to be dynamised and a further six 'self-dynamised' at the site of the dis

Effect of limited and standard reaming on cortical bone blood flow and early strength of union following segmental fracture.

OBJECTIVE: To quantitatively determine the extent to which limited and standard intramedullary reaming disrupts cortical circulation and to evaluate the effect on the biomechanical properties of the united fracture. DESIGN: Midshaft tibial osteotomies to create a 2.5-centimeter segment of diaphyseal bone were performed in ten adult dogs. Before insertion of a locked intramedullary nail, the tibia was reamed to either 7.0 millimeters (n = 5) or 9.0 millimeters (n = 5). Blood flow was measured using laser Doppler flowmetry during the nailing procedure and at eleven weeks postnailing. Bending st

Osteosynthesis of the injured chest wall. Use of the AO (Arbeitsgemeinschaft für Osteosynthese) technique.

Open reduction and osteosynthesis with AO (Arbeitsgemeinschaft für Osteosynthese) technique, using 3.5 mm reconstruction plates and 3.5 mm cancellous screws, were performed in all cases of chest wall injury considered for surgical stabilization since 1990, viz. 11 with posttraumatic flail chest and one with painful nonunion of two ribs. In the ten survivors with flail chest, stability was achieved without secondary dislocation, giving good pain relief, improved respiratory mechanics and reduced duration of ventilatory support and intensive care requirements. Lasting pain relief was obtained al

A Newly Designed Intramedullary Nail for the Treatment of Diaphyseal Forearm Fractures in Adults.

BACKGROUND: The treatment of diaphyseal forearm fractures using open reduction and plate fixation is generally accepted as the best choice in many studies. However, periosteal stripping, haematoma evacuation may result in delayed union, nonunion and infection. Refracture after plate removal is another concern. To overcome these problems intramedullary nails (IM) with different designs have been used with various outcomes. However previous IM nails have some shortcomings such is rotational instability and interlocking difficulties. We evaluated the results of newly designed IM nail in the treat

Congenital Pseudarthrosis of the Tibia: Results, at Skeletal Maturity, of the Charnley-Williams Procedure.

BACKGROUND: This study assessed the outcomes, at skeletal maturity, for 34 patients in whom congenital pseudarthrosis of the tibia (CPT) had been treated with intramedullary (IM) rod fixation. METHODS: The results in skeletally mature patients in whom type-4 CPT had been treated with an IM rod at an average of 11.9 years earlier were reviewed. The rod procedures varied according to whether both the tibia and the fibula were resected and both bones (type A) or just the tibia (type B) were fixed with an IM rod or whether only the tibia was resected and the fibula received no surgery (type C). Determining the development stage of the ossification centers around the elbow may aid in deciding whether to use ESIN or not in adolescents' forearm shaft fractures.

Background and purpose - Elastic stable intramedullary nailing (ESIN) is the preferred method of operative stabilization of unstable pediatric forearm shaft fractures. However, the decision whether to use ESIN or open reduction and internal fixation (ORIF) in older children or teenagers is not always straightforward. We hypothesized that the development stage of the elbow would aid in evaluating the eligibility of the patient for ESIN.Patients and methods - All eligible children, aged <16 years who were treated with ESIN in Oulu University Hospital, during 2010-2019 were included (N = 70). Safety aspects in surgical treatment of pathological fractures of the proximal femur - modular endoprosthetic replacement vs. intramedullary nailing.

BACKGROUND: Pathologic fractures of the femoral intertrochanteric and subtrochanteric region require special consideration in terms of biomechanically stable fixation and durability of the implant. In addition, the type of surgery might also influence patient survival. We conducted this retrospective study to evaluate the safety of modular proximal femur replacement compared to intramedullary nailing with patient survival being the primary and complications the secondary endpoint. METHODS: We retrospectively studied the records of 20 consecutive patients with actual pathologic fracture due to

Positional difference in deep femoral artery during intramedullary nailing for proximal femoral fractures: a within-subject comparative study.

BACKGROUND: We aimed to evaluate the positional changes in the deep femoral artery (DFA) during intramedullary nail surgery. We hypothesized that the femur and DFA may be closer due to the medial compression of the thigh on a traction table during the surgery. METHODS: This within-subject comparative study included 20 patients with femoral trochanteric fracture (five males, 15 females; mean age: 83 ± 10.2) who underwent intramedullary nailing. Procedures were performed by five orthopedic surgeons. We performed computed tomography (CT) scans on the healthy thigh in the neutral and surgical lim

Quantifying Proximal Tibial Physeal Injury in Rigid Intramedullary Nailing in Adolescent Patients.

BACKGROUND: Rigid intramedullary nailing (RIMN) is increasingly used to treat tibia fractures in skeletally immature patients; however, the volume of injury to the proximal tibial physis and subsequent risk of abnormal growth is unknown. This study aimed to determine volume of proximal tibial physeal injury during RIMN of the tibia in skeletally immature adolescents. METHODS: The volume of injury to the proximal tibial physis was measured through computer simulation modeling of RIMN using three-dimensional (3D) models. Sixteen normal pediatric knee CT scans (ages 10 to 13 y) with open physes

Tibiotalocalcaneal nailing allows early weightbearing without increased early complication risk in unstable geriatric malleolar ankle fractures.

PURPOSE: Geriatric ankle fractures present a clinical challenge due to frailty and poor bone quality. Unstable injuries often require prolonged non-weightbearing after traditional fixation, a limitation that may exacerbate sarcopenia, medical complications, and functional decline in the elderly. Tibiotalocalcaneal (TTC) nailing has emerged as an alternative that may permit earlier weightbearing. This study investigates complications and discharge disposition in geriatric patients who undergo fixation with TTC and conventional constructs. METHODS: A retrospective review was conducted of patien

Comparative outcomes of proximal femur intramedullary nailing vs. cemented bipolar hemiarthroplasty for treating intertrochanteric fractures in patients aged 75 and older: analysis of risk factors for postoperative all-cause mortality.

BACKGROUND: As the population continues to age, the occurrence of intertrochanteric femoral fractures (IFFs) has steadily increased. The main aims of this investigation were to evaluate and compare the clinical outcomes, ambulatory ability, overall survival, and all-cause mortality between two cephalic screws combined with compression proximal-femoral intramedullary nailing internal fixation (IF) and long-stemmed cemented bipolar hemiarthroplasty (LCHA) in patients aged 75 years and older. The secondary objective was to investigate the relative independent risk factors contributing to postoper

Gait and gluteus medius alterations after Trochanteric-Entry intramedullary nailing: A retrospective quantitative analysis : Gait and gluteus medius alterations after Trochanteric-Entry intramedullary nailing: A retrospective quantitative analysis.

BACKGROUND: Trochanteric-entry intramedullary nailing (IMN) is widely used for the treatment of femoral shaft fractures. However, concerns remain regarding potential iatrogenic injury to the gluteus medius (GMed), a key hip abductor muscle essential for gait stability. Limited clinical studies have simultaneously investigated postoperative muscle morphology and gait biomechanics in this context. This retrospective study aimed to quantitatively evaluate gait and gluteus medius muscle alterations following trochanteric-entry intramedullary nailing in patients with healed femoral fractures.
