Undiagnosed central anticholinergic syndrome may lead to dangerous complications.

This report describes two cases of central anticholinergic syndrome, the first after general anaesthesia and the other during a prolonged stay in the intensive care unit. The symptoms in both patients resolved soon after physostigmine administration. There was a delay in the diagnosis of central anticholinergic syndrome, which resulted in acute lung injury and unanticipated intensive care unit admission. It is suggested that in cases of abnormal mental recovery after anaesthesia or sedation, the diagnosis of central anticholinergic syndrome should be considered.

Comparison between intranasal and intravenous midazolam sedation (with or without patient control) in a dental phobia clinic.

Two new modes of sedation; patient-controlled sedation (PCS) and intranasal sedation (INS) were compared with the traditional bolus intravenous sedation (BIVS) while delivering dental care to apprehensive patients in a specialized dental fear clinic. Effective sedation was evaluated in a randomized, prospective study in 42 ASA 1 and 2 patients, in a factorial design. Eighteen patients were sedated with .5% midazolam INS. Ten patients received intravenous PCS via a patient-controlled analgesia pump containing midazolam, and 14 patients received intermittent intravenous boluses of 1 mg midazolam

Malignant hyperthermia and the general dentist: current recommendations.

Malignant hyperthermia is a potentially fatal disease that may be triggered by the administration of specific drugs or by stress. Although most often associated with general anesthesia, stress can be a significant stimulus and is therefore of concern to the general dentist. The decision as to how and where to treat these patients is complicated by conflicting recommendations from various sources. The aim of this article is to propose a protocol for the dentist to use in the treatment of patients with a history of malignant hyperthermia. The dentist must assess the patient for both their suscep

Review and Update of Anesthetic Management for Electroconvulsive Therapy: A Narrative Review.

Electroconvulsive therapy (ECT) is a well-established treatment for severe psychiatric disorders, and its role in modern-day psychiatry continues to expand with rising rates of complex mental illnesses. ECT has proven to be an effective treatment for conditions such as treatment-resistant depression, catatonia, bipolar disorder, schizophrenia, and other serious mental health disorders. Individuals who are resistant to or have contraindications against conventional pharmacologic therapies can particularly benefit from ECT. The procedure involves inducing a brief and controlled generalized seizu

Evaluation of gastric volume and pH changes in gastroscopy patients using chewing gum and candy: A prospective randomized double-blind study.

BACKGROUND: Whether chewing gum or candy consumption during preoperative fasting affects aspiration risk remains debated. This study investigated the effects of these products on gastric volume and pH, and evaluated procedure difficulty, satisfaction, and fasting-related symptoms. METHODS: This randomized, double-blind study enrolled 405 patients scheduled for elective gastroscopy, equally allocated to control, chewing gum, and candy groups. All groups adhered to standard fasting protocols; the chewing gum and candy groups additionally consumed the assigned products within 2 hours of sedation

The role of enteroscopy in the diagnosis and interventional management of small intestine diseases.

Evaluation of the small intestine is technically difficult because of its length and anatomy. Capsule endoscopy and device-assisted enteroscopy (DAE) have expanded diagnostic and therapeutic options in recent years. This narrative review summarizes the current indications, therapeutic procedures, safety, and role of enteroscopy compared with other small bowel imaging methods. A literature search was performed in the MEDLINE database covering publications from February 2001 to February 2025, using predefined inclusion and exclusion criteria. The main indications include small intestinal bleedin

Endoscopic Ultrasound-Guided Fine-Needle Aspiration of Suspected Locoregional Rectal Cancer Localizations: A Valuable Tool.

INTRODUCTION: Organ-preserving treatment for rectal cancer using local excision (LE) and/or chemoradiotherapy (CRT) is increasingly used. Locoregional metastasis precludes LE and locoregional regrowth, recurrence, or persistence after LE or chemoradiation (CRT) may prompt total mesorectal excision (TME). We believe that the time has passed to make such life-changing treatment decisions without pathological confirmation and investigated the use of linear endoscopic ultrasound with fine-needle aspiration (EUS-FNA). CASE PRESENTATIONS: We report 8 cases of suspected locoregional tumor growth (LR

Clinical Outcome and Recurrence of Open versus Laparoscopic Nissen Fundoplication in the Republic of Kazakhstan during 2010-2021.

Following the initial liver biopsy attempts, several techniques using a wide range of methodologies and materials were developed. Many studies on the evaluation of post-liver biopsy complications were conducted. However, their fundamental limitation was significant variance in patient demographics and methodology, which might account for the inconsistent outcomes. Therefore, a uniform methodology to perform percutaneous liver biopsies that result in comparable outcomes around the world is required. This study aimed to determine the precise complication rate following percutaneous liver biopsy

Hemostatic Profiles of Patients Who Underwent Transcatheter Versus Surgical Aortic Valve Replacement Versus Percutaneous Coronary Intervention.

Guidelines for transcatheter aortic valve replacement (TAVR) antithrombotic prophylaxis are extrapolated predominantly from percutaneous coronary intervention (PCI) data. Here, we examined temporal coagulation changes occurring in the early perioperative period to determine the pathobiologic validity of this supposition. This was a prospective observational study of consecutive patients who underwent transfemoral TAVR (n = 27), PCI (n = 12), or surgical aortic valve replacement (SAVR) requiring cardiopulmonary bypass and cross-clamping (n = 12). Blood samples were taken at 4 time points: T1 (b

Outpatient flexible endoscopic diverticulotomy for the management of Zenker's diverticulum: a retrospective analysis of a large single-center cohort.

BACKGROUNDS AND AIMS: Flexible endoscopic Zenker's diverticulotomy (EZD) is well established as a safe and effective technique. Because of rare but concerning adverse events, most centers admit patients for observation and barium swallow study. Our center routinely performs EZD as a day procedure, discharging appropriate patients on the same day after clinical review. This study evaluates outcomes of this cohort compared with previously published studies where patients are admitted for observation. METHODS: A retrospective analysis was performed of EZD procedures done at our center using a fl

High body mass index is a risk factor for difficult deep sedation in percutaneous mitral valve repair.

BACKGROUND: The safety and efficacy of deep sedation (DS) in MitraClip® procedures have been shown previously. However, clinical experience demonstrates that in some patients DS is difficult to achieve. We hypothesize that some patient characteristics can predict difficult DS. METHODS: We prospectively analysed 69 patients undergoing MitraClip® procedures using DS. Application of DS was graded as simple (group 1) or difficult (group 2) depending on a cumulative score based on one point for each of the following criteria: decrease in oxygen saturation, retention of carbon dioxide, disruptive b

Local anesthetics for brain tumor resection: current perspectives.

This review summarizes the added value of local anesthetics in patients undergoing craniotomy for brain tumor resection, which is a procedure that is carried out frequently in neurosurgical practice. The procedure can be carried out under general anesthesia, sedation with local anesthesia or under local anesthesia only. Literature shows a large variation in the postoperative pain intensity ranging from no postoperative analgesia requirement in two-thirds of the patients up to a rate of 96% of the patients suffering from severe postoperative pain. The only identified causative factor predicting

Percutaneous CT-Guided Biopsies of the Cervical Spine: Technique, Histopathologic and Microbiologic Yield, and Safety at a Single Academic Institution.

BACKGROUND AND PURPOSE: Cervical spine biopsies can be challenging due to the anatomy and the adjacent critical structures. Percutaneous image-guided biopsies can obviate the need for an open biopsy, however there have been few studies looking at the approaches, safety, and efficacy of percutaneous cervical spine biopsies. This retrospective study evaluated technical considerations, histopathologic and microbiologic yield, and safety in CT-guided cervical bone biopsies. MATERIALS AND METHODS: A retrospective review of cervical bone and/or bone/disc biopsies performed from January 2010 to Janu

Redefining the impact of oxygen and hyperventilation after the Norwood procedure.

OBJECTIVE: Postoperative management after the Norwood procedure is aimed at optimizing systemic oxygen delivery and mixed venous oxygen saturation. High levels of fraction of inspired oxygen and hyperventilation may increase pulmonary blood flow at the expense of systemic flow. This study determines the effects of these interventions on mixed venous saturation and systemic oxygen delivery in postoperative neonates. METHODS: We prospectively studied the effects of 100% fraction of inspired oxygen and hyperventilation in 14 neonates (median age 8 days) 1 to 3 days after the Norwood procedure, w
