Infective endocarditis after device closure of atrial septal defects: Case report and review of the literature.

We report a case of late infective endocarditis in an 8-year-old boy 3 years after transcatheter closure of an atrial septal defect with the Amplatzer Atrial Septal Occluder Device. Echocardiography showed a very thick pannus lining the left atrial disc of the prosthesis, with some mobile elements. MRI showed several cerebral microemboli. The patient had dental caries and blood cultures found Staphylococcus aureus. Patient was treated with gentamicin and oxacillin before surgical removal of the prosthesis. Twenty-one cases of infective endocarditis after atrial defect device closure have been

Infective endocarditis in a child caused by Cardiobacterium hominis after right ventricular outflow tract reconstruction using an expanded tetrafluoroethylene conduit.

Cardiobacterium hominis, a member of the HACEK group of organisms, is a rare cause of endocarditis. We report a case of infective endocarditis caused by C. hominis in a male child who had undergone right ventricular outflow tract (RVOT) reconstruction using an expanded polytetrafluoroethylene conduit for tetralogy of Fallot with pulmonary atresia. Two days before admission, the patient suffered from exertional shortness of breath. Right ventricular hypertension was confirmed and RVOT stenosis was suspected based on the echocardiography findings. A CT scan revealed vegetation above the cusp of

Observational study of bone-anchored hearing aid infection rates using different post-operative dressings.

INTRODUCTION: Avoidance of infection at the implant site is a crucial element to the success of bone-anchored hearing aid (BAHA) implantation. However, little evidence exists to suggest the best post-operative wound dressing to use. MATERIAL AND METHODS: We report our experience with 160 consecutive BAHA patients, using four types of post-operative BAHA wound dressing (Tri-adcortyl, Mepitel, Allevyn, and Acticoat 7 with Allevyn). Patients were reviewed at week one and week two post-operatively. Infection was defined as a positive wound swab culture or evidence of cellulitis at the BAHA site.

Personalized Approaches to Diagnostic and Therapeutic Strategies in Periprosthetic Fracture-Related Infections (PFRIs): Case Series and Literature Review.

Aim: Periprosthetic fracture-related infections (PFRIs) are a serious complication of total arthroplasty, with incidence rates increasing in line with the growing number of joint replacements. PFRI can lead to prolonged hospitalization, multiple surgical procedures and suboptimal functional outcomes. The diagnosis of PFRI remains challenging due to the overlap of clinical symptoms with other post-traumatic conditions, and identification of the pathogen often fails through conventional methods. This study also highlights the importance of a personalized medicine approach in managing PFRI, where

Globulin and albumin to globulin ratio precisely diagnose periprosthetic joint infection and determine the timing of second-stage reimplantation.

BACKGROUND: Periprosthetic joint infection (PJI) is one of the most challenging complications of total joint arthroplasty (TJI). An early and accurate diagnosis of PJI is associated with better treatment outcomes. However, whether the platelet-related markers and globulin-related markers can be used to assist the diagnosis of PJI remains elusive. METHODS: A total of 206 patients who underwent revision hip or knee arthroplasty in our institution were divided into two groups: 79 patients in PJI group and 127 patients in aseptic failure group. The levels of erythrocyte sedimentation rate (ESR),

Molecular Characterization of a Prevalent Ribocluster of Methicillin-Sensitive Staphylococcus aureus from Orthopedic Implant Infections. Correspondence with MLST CC30.

Staphylococcus aureus is the leading etiologic agent of orthopedic implant infections. Here a ribocluster of 27 S. aureus strains underwent further molecular characterization and subtyping by multilocus sequence typing (MLST) and spa-typing. This cluster had been detected by automated ribotyping (with the EcoRI restriction enzyme) of 200 S. aureus isolates from periprosthetic infections of patients who underwent revision at the Rizzoli Orthopaedic Institute. The ribocluster, consisting of agr type III strains, with a 74% co-occurrence of bone sialoprotein-binding (bbp) and collagen-binding (cn

Reversion From Methicillin Susceptibility to Methicillin Resistance in Staphylococcus aureus During Treatment of Bacteremia.

Approximately 3% of Staphylococcus aureus strains that, according to results of conventional phenotypic methods, are highly susceptible to methicillin-like antibiotics also have polymerase chain reaction (PCR) results positive for mecA. The genetic nature of these mecA-positive methicillin-susceptible S. aureus (MSSA) strains has not been investigated. We report the first clearly defined case of reversion from methicillin susceptibility to methicillin resistance among mecA-positive MSSA within a patient during antibiotic therapy. We describe the mechanism of reversion for this strain and for a

SdrF, a Staphylococcus epidermidis surface protein, contributes to the initiation of ventricular assist device driveline-related infections.

Staphylococcus epidermidis remains the predominant pathogen in prosthetic-device infections. Ventricular assist devices, a recently developed form of therapy for end-stage congestive heart failure, have had considerable success. However, infections, most often caused by Staphylococcus epidermidis, have limited their long-term use. The transcutaneous driveline entry site acts as a potential portal of entry for bacteria, allowing development of either localized or systemic infections. A novel in vitro binding assay using explanted drivelines obtained from patients undergoing transplantation and

Postoperative infections in craniofacial reconstructive procedures.

The rate of, and possible risk factors for, postoperative craniofacial infection is unclear. To investigate this problem, we reviewed 349 cases of craniofacial skeletal procedures performed from 1996 to 1999 at our institution. Infection rate was determined and correlated with the use of implants, operative site, and cause of deformity. The inclusion criteria consisted of all procedures requiring autologous or prosthetic implantation in craniofacial skeletal sites, as well as all procedures involving bone or cartilage resection, osteotomies, debridement, reduction and/or fixation. Procedures t

Application of autoantibody markers based on phage display immunoprecipitation sequencing technology in the diagnosis of periprosthetic joint infections.

BACKGROUND: Accurate diagnosis of periprosthetic joint infection (PJI) is critical yet challenging. The role of autoantibodies in PJI remains largely unexplored. In this study, we employed phage display immunoprecipitation sequencing (PhIP-Seq) to profile PJI-associated autoantibodies and assess their diagnostic utility in PJI. METHODS: We collected plasma samples from a cohort of 45 patients, including 25 with PJI, 9 with aseptic loosening, and 11 post-primary arthroplasty without complications. Initially, PhIP-Seq screening was performed on 10 samples (five PJI and five post-primary arthrop

Guideline-Directed Administration of Prophylactic Antibiotic-Loaded Bone Cement in Total Knee Arthroplasty Does Not Lower Infection Rate.

BACKGROUND: Periprosthetic joint infection (PJI) is a major complication following total knee arthroplasty (TKA). Antibiotic-loaded cement (ALBC) is commonly used as a preventative measure, though it remains controversial. Despite guidelines from organizations including the American Academy of Orthopedic Surgeons (AAOS) and the International Consensus Meeting on Musculoskeletal Infection (ICM), ALBC usage in primary TKA varies. The purpose of this study was to evaluate the effectiveness of guideline-directed ALBC utilization in reducing PJI in primary TKA. PATIENTS AND METHODS: A retrospectiv

Single versus dual antibiotic-loaded bone cement in hip fracture arthroplasty: a cohort study of 56,434 hip arthroplasties from the Dutch Arthroplasty Register.

BACKGROUND AND PURPOSE: Hip fracture treatment with (hemi)arthroplasty carries a risk of periprosthetic joint infection (PJI), which may be reduced by using dual antibiotic-loaded bone cement (ALBC). We aimed to compare the rate of all-cause revision, revision for infection, and mortality following single or dual ALBC use in hip fracture arthroplasty. METHODS: From the Dutch Arthroplasty Register, we identified all cemented hemiarthroplasties (HA) and total hip arthroplasties (THA) performed in the period 2007-2024 for proximal femoral fracture. Crude cumulative incidences of all-cause and in

Role of synovial biopsy in diagnosis of chronic periprosthetic infection in total hip and knee arthroplasties.

INTRODUCTION: Infection is a significant and challenging complication of total hip and knee arthroplasties. Diagnosing chronic periprosthetic joint infection (PJI) can be difficult, particularly when joint aspiration yields inconclusive results or no intra-articular effusion is available for aspiration. Synovial biopsy represents a potential diagnostic option. This study evaluated the reliability of minimally invasive surgical biopsy for diagnosing chronic PJI. MATERIALS AND METHODS: This retrospective, single-centre, observational study was conducted over a 10-year period. It included consec

Real-World Outcomes of Leadless Pacemaker Implantation After Transvenous Lead Removal for Infected Cardiac Implantable Electronic Devices in the United States.

Leadless pacemakers (LPMs) have emerged as a promising alternative to transvenous systems, particularly in patients requiring transvenous lead removal (TLR) due to cardiac implantable electronic device (CIED) infections. However, real-world outcomes following LPM implantation in this setting remain understudied. Using the National Readmissions Database from 2016 to 2021, we identified 41,058 adult patients who underwent LPM implantation, of whom 3,244 (7.9%) received the device following TLR for CIED infection. Compared to patients undergoing de novo implantation, those in the post-TLR cohort

Procedural Characteristics and Outcomes of Transvenous Cardiac Device Extraction in Patients With Left Ventricular Assist Devices.

INTRODUCTION: Durable left ventricular assist devices (LVADs) are susceptible to infection. The presence of a transvenous cardiac implantable electronic device (CIED) may further elevate this risk, making it necessary to remove the CIED system. This report presents the procedural characteristics and outcomes of CIED system extraction in patients with LVADs. METHODS: This is a single-center retrospective study that included patients with durable LVADs who underwent transvenous CIED extraction at our institution from 2017 to 2023. RESULTS: Twenty patients with LVAD (HeartMate II = 14; HeartMat
