Thresholds in the Relationship of Quadriceps Strength With Functional Limitations in Women With Knee Osteoarthritis.

OBJECTIVE: To investigate thresholds of strength below which individuals with knee osteoarthritis (OA) may have more difficulty carrying out physical functions of daily life. Individuals below such thresholds might benefit more from strengthening interventions than those with greater strength. METHODS: We studied individuals with symptomatic OA at baseline in the Multicenter Osteoarthritis Study who had knee extensor strength measured isokinetically at 60º/second. Participants underwent a 20-meter walk test and a sit-to-stand test and answered questions from the Western Ontario and McMaster U

The effects of a strength and neuromuscular exercise programme for the lower extremity on knee load, pain and function in obese children and adolescents: study protocol for a randomised controlled trial.

BACKGROUND: Childhood obesity is one of the most critical and accelerating health challenges throughout the world. It is a major risk factor for developing varus/valgus misalignments of the knee joint. The combination of misalignment at the knee and excess body mass may result in increased joint stresses and damage to articular cartilage. A training programme, which aims at developing a more neutral alignment of the trunk and lower limbs during movement tasks may be able to reduce knee loading during locomotion. Despite the large number of guidelines for muscle strength training and neuromuscu

Influences of physical fitness on bone mass in women with fibromyalgia.

The aim of this study was to provide information about the relationship of bone mineral content (BMC) and density (BMD) with some physical-fitness-related variables in a sample of women with fibromyalgia (FM) and age-matched women without FM. Twenty-eight women clinically diagnosed with FM (age 51.1 ± 8.4 yr, M ± SD) and 22 age-matched controls participated in the study. Whole-body BMC and BMD, lean mass, handgrip strength, quadriceps strength, and cardiovascular fitness were measured in all participants. The association between physical-fitness variables and bone-related variables was tested

Rectus femoris and vastus intermedius muscle thickness as a predictor of mortality in elderly patients with pneumonia.

OBJECTIVES: The high mortality rate among elderly patients with community-acquired pneumonia (CAP) underscores the need for the early identification of high-risk individuals. Sarcopenia has been demonstrated to be associated with mortality in various infectious diseases. In this study, we compared the roles of rectus femoris (RF) muscle thickness, vastus intermedius (VI) muscle thickness, paraspinal muscle area, and the CURB-65 score (which includes confusion, urea level, respiratory rate, blood pressure, and age ≥ 65 years) and the Pneumonia Severity Index (PSI) in predicting 30-day mortality

Anterior cruciate ligament reconstructed females who pass or fail a functional test battery do not exhibit differences in knee joint landing biomechanics asymmetry before and after exercise.

PURPOSE: A functional test battery (FTB) has been proposed to evaluate the readiness of return to activity after ACLR. However, there is limited evidence documenting the usefulness of an FTB. Therefore, the purpose of the current investigation was to compare knee joint landing biomechanics asymmetry during double-leg jump landing (DLJL) and single-leg jump cutting (SLJC) between healthy females and ACLR females who pass (ACLR-pass) or fail (ACLR-fail) an FTB before and after the completion of a sustained exercise protocol. Association of Protein Kinase B (AKT) DNA Hypermethylation with Maintenance Atypical Antipsychotic Treatment in Patients with Bipolar Disorder.

STUDY OBJECTIVE: Atypical antipsychotics cause insulin resistance that leads to an increased risk of diabetes mellitus and cardiovascular disease. Skeletal muscle is the primary tissue for uptake of glucose, and its dysfunction is considered one of the primary defects in the development of insulin resistance. Protein kinase B (AKT) plays an important role in overall skeletal muscle health and glucose uptake into the muscle. The objective of this study was to measure AKT isoform-specific gene methylation differences in the skeletal muscle of patients with bipolar disorder treated with atypical

The ability of medial patellofemoral ligament reconstruction to correct patellar kinematics and contact mechanics in the presence of a lateralized tibial tubercle.

BACKGROUND: Tibial tubercle (TT) transfer and medial patellofemoral ligament (MPFL) reconstruction are used after patellar dislocations. However, there is no objective evidence to guide surgical decision making, such as the ability of MPFL reconstruction to restore normal behavior in the presence of a lateralized TT. HYPOTHESIS: MPFL reconstruction will only restore joint contact mechanics and patellar kinematics for TT-trochlear groove (TG) distances up to an identifiable limit. STUDY DESIGN: Controlled laboratory study. METHODS: Eight fresh-frozen cadaveric knees (mean TT-TG distance, 10.

Revision anterior cruciate ligament reconstruction using an autogenous quadriceps tendon graft is associated with substantial improvement in patient-reported outcomes.

INTRODUCTION: Revision anterior cruciate ligament reconstruction (rACLR) presents multiple technical challenges, including having to contend with limited graft availability and compromised bone stock. The quadriceps tendon (QT) autograft-often preferred for its large diameter, high collagen content, and reduced donor-site morbidity compared to patellar tendons-has emerged as a promising option due to its biomechanical strength and reduced donor site morbidity. OBJECTIVE: To evaluate clinical and functional outcomes of rACLR using autologous QT grafts with a bone block, focusing on improvement

Vascularized motor nerve to vastus lateralis grafts for restoration of eye closure function in radical parotidectomy: Enhanced reliability in irradiated beds.

OBJECTIVES: Nerve grafting is commonly used as a reconstructive strategy to restore facial nerve continuity after radical parotidectomy, aiming to preserve motor end-plate function. However, patients undergoing nerve grafting frequently require adjuvant radiotherapy, and the reported success rates of conventional nerve grafts range from 30% to 60%. We evaluated the outcomes of reconstruction of the upper division of the facial nerve using a vascularized motor nerve to vastus lateralis (MNVL) graft to enhance facial reanimation in the context of preoperative or adjuvant radiotherapy. METHODS:

Soft-tissue quadriceps tendon autograft during primary anterior cruciate ligament reconstruction in Skeletally-immature patients vs. Hamstrings: A protocol for a multi-centre randomized controlled trial.

BACKGROUND: ACL tears in skeletally-immature patients are often treated surgically using soft-tissue autografts to offset increased risks of cartilage, meniscus and physeal injuries. Despite technological innovation, there remains a high failure rate among active young patients. We present a randomized controlled trial (RCT) to evaluate the soft-tissue quadriceps autograft compared to the standard hamstring autograft to treat primary ACL tears in the pediatric population. METHODS: This international RCT will evaluate 352 skeletally-immature patients between the ages of 10-18 years undergoing

Enhanced dimethylarginine degradation improves coronary flow reserve and exercise tolerance in Duchenne muscular dystrophy carrier mice.

Duchenne muscular dystrophy (DMD) is an X-linked disease caused by null mutations in dystrophin and characterized by muscle degeneration. Cardiomyopathy is common and often prevalent at similar frequency in female DMD carriers irrespective of whether they manifest skeletal muscle disease. Impaired muscle nitric oxide (NO) production in DMD disrupts muscle blood flow regulation and exaggerates postexercise fatigue. We show that circulating levels of endogenous methylated arginines including asymmetric dimethylarginine (ADMA), which act as NO synthase inhibitors, are elevated by acute necrotic m

In situ cross-sectional area of the quadriceps tendon using preoperative magnetic resonance imaging significantly correlates with the intraoperative diameter of the quadriceps tendon autograft.

PURPOSE: Preoperative assessment to determine the sizes of potential autografts is necessary for individualized anterior cruciate ligament reconstruction (ACLR). However, no study has investigated the prediction of the intraoperative diameter of the quadriceps tendon (QT) autograft based upon preoperative imaging. This study investigated the correlation between the intraoperative diameter of a QT autograft and in situ thickness or cross-sectional area (CSA) measured using preoperative MRI. METHODS: Thirty-one knees of 31 patients (mean age 20.9 ± 5.0 years) who underwent individualized anatom

Lower donor-site morbidity using QT autografts for ACL reconstruction.

PURPOSE: Comparing scar cosmesis and regional hypoesthesia at the incision site between quadriceps tendon (QT), bone-patellar tendon-bone (BPTB), and hamstring tendon (HT) for anterior cruciate ligament (ACL) reconstruction. METHODS: Ninety patients undergoing ACL reconstruction with QT, HT or BPTB were evaluated at 1-year post-op. Scar cosmesis was assessed using the patient and observer scar assessment scale (POSAS) and length of the incision. Sensory outcome was analyzed by calculating the area of hypoesthesia around the scar. The classical ACL reconstruction functional follow-up was measu

TLR9-Activating CpG-B ODN but Not TLR7 Agonists Triggers Antibody Formation to Factor IX in Muscle Gene Transfer.

Innate immune signals that promote B cell responses in gene transfer are generally ill-defined. In this study, we evaluate the effect of activating endosomal Toll-like receptors 7, 8, and 9 (TLR7, TLR7/8, and TLR9) on antibody formation during muscle-directed gene therapy with adeno-associated virus (AAV) vectors. We examined whether activation of endosomal TLRs, by adenine analog CL264 (TLR7 agonist), imidazolquinolone compound R848 (TLR7/8 agonist), or class B CpG oligodeoxynucleotides ODN1826 (TLR9 agonist), could augment antibody formation upon intramuscular administration of AAV1 expressi

Treatment of Extensor Tendon Disruption After Total Knee Arthroplasty: A Systematic Review.

BACKGROUND: Patellar or quadriceps tendon ruptures after total knee arthroplasty constitute a devastating complication with historically poor outcomes. With advances in soft tissue reconstruction and repair techniques, treatment has become more nuanced. Numerous graft options for reconstruction and suture techniques for repair have been described but there is no consensus regarding optimal treatment. METHODS: A search of PubMed, MEDLINE, Embase, and Scopus was conducted. Articles meeting inclusion criteria were reviewed. Type of intervention performed, type of injury studied, outcome measures
