The cortical organization of listening effort: New insight from functional near-infrared spectroscopy.

Everyday challenges impact our ability to hear and comprehend spoken language with ease, such as accented speech (source factors), spectral degradation (transmission factors), complex or unfamiliar language use (message factors), and predictability (context factors). Auditory degradation and linguistic complexity in the brain and behavior have been well investigated, and several computational models have emerged. The work here provides a novel test of the hypotheses that listening effort is partially reliant on higher cognitive auditory attention and working memory mechanisms in the frontal lo

Comparison between adaptive and adjustment procedures for binaural loudness balancing.

Binaural loudness balancing is performed in research and clinical practice when fitting bilateral hearing devices, and is particularly important for bimodal listeners, who have a bilateral combination of a hearing aid and a cochlear implant. In this study, two psychophysical binaural loudness balancing procedures were compared. Two experiments were carried out. In the first experiment, the effect of procedure (adaptive or adjustment) on the balanced loudness levels was investigated using noise band stimuli, of which some had a frequency shift to simulate bimodal hearing. In the second experime

The Effect of Interaural Mismatches on Contralateral Unmasking With Single-Sided Vocoders.

OBJECTIVES: Cochlear-implant (CI) users with single-sided deafness (SSD)-that is, one normal-hearing (NH) ear and one CI ear-can obtain some unmasking benefits when a mixture of target and masking voices is presented to the NH ear and a copy of just the masking voices is presented to the CI ear. NH listeners show similar benefits in a simulation of SSD-CI listening, whereby a mixture of target and masking voices is presented to one ear and a vocoded copy of the masking voices is presented to the opposite ear. However, the magnitude of the benefit for SSD-CI listeners is highly variable across

Outcomes in Endoscopic Ear Surgery.

Endoscopic ear surgery (EES) provides several advantages compared with traditional binocular microscopy, including a wide-field view, improved resolution with high magnification, and visual access to hidden corridors of the middle ear. Although binocular microscopic-assisted surgical techniques remain the gold standard for most otologists, EES is slowly emerging as a viable alternative for performing otologic surgery at several centers in the United States and abroad. In this review, we evaluate the current body of literature regarding EES outcomes, summarize our EES outcomes at the Massachuse

Difficulty understanding speech in noise by the hearing impaired: underlying causes and technological solutions.

A primary complaint of hearing-impaired individuals involves poor speech understanding when background noise is present. Hearing aids and cochlear implants often allow good speech understanding in quiet backgrounds. But hearing-impaired individuals are highly noise intolerant, and existing devices are not very effective at combating background noise. As a result, speech understanding in noise is often quite poor. In accord with the significance of the problem, considerable effort has been expended toward understanding and remedying this issue. Fortunately, our understanding of the underlying i

[Implantation of a vibratory mass transducer on the round window: a report of two cases].

The Vibrant Soundbridge (VSB, Med-El, Austria) is a semiimplantable hearing aid usually attached to the long process of the incus to vibrate the ossicular chain in patients with moderate to severe mixed hearing loss. We implanted a VSB vibratory transducer on the round window membrane of the left ear in two cases not treated effectively by tympanoplastic surgeries. Pure tone audiography did not differ significantly in the two cases pre- or postoperatively, indicating that the small mass transducer does not adversely affect middle-ear vibration. Postoperative hearing thresholds with VSBs were s

Improving speech perception for hearing-impaired listeners using audio-to-tactile sensory substitution with multiple frequency channels.

Cochlear implants (CIs) have revolutionised treatment of hearing loss, but large populations globally cannot access them either because of disorders that prevent implantation or because they are expensive and require specialist surgery. Recent technology developments mean that haptic aids, which transmit speech through vibration, could offer a viable low-cost, non-invasive alternative. One important development is that compact haptic actuators can now deliver intense stimulation across multiple frequencies. We explored whether these multiple frequency channels can transfer spectral information

Efficient and sustained optogenetic control of sensory and cardiac systems.

Optogenetic control is used to manipulate the activity of specific cell types in vivo for a variety of biological and clinical applications. Here we report ChReef, an improved variant of the channelrhodopsin ChRmine. ChReef offers minimal photocurrent desensitization, a unitary conductance of 80 fS and closing kinetics of 30 ms, which together enable reliable optogenetic control of cells at low light levels with good temporal fidelity and sustained stimulation. We demonstrate efficient and reliable red-light pacing and depolarization block of ChReef-expressing cardiomyocyte clusters. We used a

Translabyrinthine Excision of a Transmodiolar Intralabyrinthine Schwannoma Mimicking Meniere's Disease: A Case Report.

Intralabyrinthine schwannomas (ILSs) are rare tumors involving the otic capsule. Notably, they are often misdiagnosed because their symptoms mimic those of other, more common inner ear pathologies. Diagnosis requires high-resolution contrast-enhanced magnetic resonance imaging (MRI), which reveals filling defects (using a T2-weighted MRI sequence) or focal enhancement (using a T1-weighted MRI sequence with gadolinium enhancement) in the inner ear. A 52-year-old male patient with intractable vertigo or single-sided deafness should raise suspicion of this clinical entity as a differential diagno

Functional modelling of interaural time difference discrimination in acoustical and electrical hearing.

OBJECTIVE: Interaural time differences (ITDs) are important for sound source localisation. We present a model to predict the just noticeable differences (JNDs) in ITD discrimination for normal hearing and electric stimulation through a cochlear implant. APPROACH: We combined periphery models of acoustic and electric stimulation with a novel JND in the ITD estimation stage, which consists of a shuffled cross correlogram and a binary classifier characterisation method. Furthermore, an evaluation framework is presented based on a large behavioural dataset. MAIN RESULTS: The model correctly pred

Vowel production of Mandarin-speaking hearing aid users with different types of hearing loss.

In contrast with previous research focusing on cochlear implants, this study examined the speech performance of hearing aid users with conductive (n = 11), mixed (n = 10), and sensorineural hearing loss (n = 7) and compared it with the speech of hearing control. Speech intelligibility was evaluated by computing the vowel space area defined by the Mandarin Chinese corner vowels /a, u, i/. The acoustic differences between the vowels were assessed using the Euclidean distance. The results revealed that both the conductive and mixed hearing loss groups exhibited a reduced vowel working space, but
