Correlation between spectral domain optical coherence tomography findings and fluorescein angiography patterns in diabetic macular edema.

OBJECTIVE: To study the relationship between spectral domain optical coherence tomography (SD-OCT) findings and fluorescein angiography (FA) patterns in patients with diabetic macular edema (DME). DESIGN: Retrospective, observational, cross-sectional study. PARTICIPANTS: We included 59 eyes from 59 patients with DME that had SD-OCT/scanning laser ophthalmoscope (SLO) and FA performed on the same day. Eyes with macular edema owing to other ocular diseases were excluded. METHODS: The relationship between SD-OCT and FA findings was evaluated by superimposing and aligning the SLO images onto th

Costs and Its Determinants in Pituitary Tumour Surgery.

PURPOSE: Value-based healthcare (VBHC) provides a framework to improve care by improving patient outcomes and reducing healthcare costs. To support value-based decision making in clinical practice we evaluated healthcare costs and cost drivers in perioperative care for pituitary tumour patients. METHODS: We retrospectively assessed financial and clinical data for surgical treatment up to the first year after surgery of pituitary tumour patients treated between 2015 and 2018 in a Dutch tertiary referral centre. Multivariable regression analyses were performed to identify determinants of higher

Chlorambucil-induced postclosure exencephaly and axial skeletal abnormalities in rat fetuses.

Open neural tube defects (NTD) are reported to arise from failure of the embryonic neural folds to close or rupture of a previously closed neural tube. The critical period for dysmorphogenesis of the neural tube by either mechanism is of clinical importance. We had previously reported that single doses of cyclophosphamide (CPA), administered to pregnant rats resulted in reopening of the closed neural tube. The objective of the present study was to determine if other antineoplastic drugs also had similar ability to cause rupture of the closed neural tube. Therefore, single doses of chlorambucil

Think twice before diagnosing tension pneumothorax: a retrospective analysis of pseudotension pneumothorax.

OBJECTIVES: Tension pneumothorax (TP) is a clinical emergency that requires immediate treatment. However, there are specific cases that may be misdiagnosed as TP in clinical settings, potentially leading to iatrogenic injury. The goal of this study was to analyse and summarize instances of misdiagnosis in order to improve the clinician's ability to recognize and distinguish pseudotension pneumothorax (PTP). METHODS: Cases misdiagnosed as TP were retrospectively analysed by literature retrieval, and the related clinical characteristics were summarized. RESULTS: A total of 45 cases were misdia

The Problem to a Solution: A Case of Local Anesthetic-Related Acute Pancreatitis.

Although rare, local anesthetic use has been associated with adverse central nervous system and cardiovascular adverse events. One complication is local anesthetic systemic toxicity (LAST), wherein the anesthetic agent inadvertently enters systemic circulation resulting in widespread inhibition of fast-gated sodium channels. Organs dependent on aerobic metabolism, such as the heart and brain, are especially susceptible to toxic injury resulting in cardiovascular collapse. Lipid emulsion therapy is a mainstay treatment of LAST; however, it may inadvertently cause lipid-induced necrotizing pancr

Autosomal dominant polycystic kidney disease: clinical and genetic aspects.

Autosomal dominant polycystic kidney disease (ADPKD) is an inherited systemic disease caused by at least three different genes. The renal and extrarenal clinical manifestations, and the systemic complications due to cystic and non-cystic abnormalities in ADPKD patients have been widely investigated. Cellular and molecular aspects of cystogenetic mechanisms concern epithelial tubular cell proliferation, remodelling of extracellular matrix, fluid secretion and accumulation, and relations between cell proliferation and apoptosis. In vitro studies on cystogenesis suggest a key role of cell-to-cell

Impact of Percutaneous Nephrostomy on the Efficacy of in Situ Shock Wave Lithotripsy for Upper Ureteral Stones.

Arginine vasopressin (AVP) is a neuropeptide hormone that plays an important role in circulatory and sodium homeostasis, and regulating serum osmolality. Several clinical conditions have been associated with inappropriately elevated levels of AVP including heart failure, cirrhosis of the liver and the syndrome of inappropriate secretion of antidiuretic hormone. Three receptor subtypes that mediate the actions of AVP have been identified (V(1A), V(2) and V(1B)). Activation of V(1A) receptors located in vascular smooth muscle cells and the myocardium results in vasoconstriction and increased aft

The Diagnostic Accuracy of Magnetic Resonance Imaging for Maternal Acute Adnexal Torsion during Pregnancy: Single-Institution Clinical Performance Review.

BACKGROUND: For acute adnexal torsion of pregnant women, appropriate treatment based on an accurate diagnosis is especially important for fertility preservation and timely treatment. The 2017 American College of Obstetricians and Gynecologists (ACOG) Committee Opinion No. 723 announced its practice-changing guidelines to ensure that diagnostic magnetic resonance imaging (MRI) conducted during the first trimester and gadolinium exposure at any time during pregnancy are safe for fetal stability. Unfortunately, few studies have been performed to evaluate the usefulness of the diagnostic accuracy

Clinical Manifestation and Management of ADPKD in Western Countries.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is the most common hereditary kidney disease in Western countries. The prevalence is between 2.4/10,000 and 3.9/10,000. ADPKD represents a systemic disease resulting in deterioration in renal function. Until now, mutations in two genes (PKD1 and PKD2) have been identified. Recently, the European Medicines Agency (EMA) approved the use of the vasopressin V2 receptor antagonist tolvaptan to slow the progression of cyst development and renal insufficiency connected with ADPKD in adult patients with chronic kidney disease stages 1-3

Reduced α-2,6 sialylation regulates cell migration in endometriosis.

STUDY QUESTION: Is endometriosis associated with aberrant sialylation patterns and what is the potential impact of such anomalies on cell migratory properties? SUMMARY ANSWER: The reduced α-2,6 sialylation patterns in the peritoneal fluid of endometriosis-affected women and in stromal and epithelial cells from endometriotic lesions could be associated with enhanced cell migration. WHAT IS KNOWN ALREADY: Endometriosis is considered to be a benign disease although, like cancer, it has the characteristic of being an invasive disease with cells that have an enhanced capacity to migrate. Aberrant

First impressions about Adherus, a new dural sealant.

PURPOSE: The aim of the study is to report our first impressions about Adherus, a novel dural sealant, used in neurosurgical endoscopic transnasal procedures. METHODS: We retrospectively reviewed the clinical and surgical records of the first 11 patients with intraoperative high-flow cerebrospinal fluid leak treated with the aid of Adherus at our center between February and October 2014. The healing at the level of the dural plasty was monitored and evaluated radiologically and with regular endoscopic inspections. RESULTS: With a median follow-up of 210 days, no postoperative CSF leak or sur

Interhemispheric multiloculated ependymal cyst with dysgenesis of the corpus callosum: a case in a preterm fetus.

INTRODUCTION: The authors report a case of interhemispheric ependymal cyst accompanied with agenesis of the corpus callosum in a fetus. DISCUSSION: Routine ultrasound and subsequent magnetic resonance imaging of a 20-year-old woman at 33 weeks and 1 day of gestation detected a large interhemispheric cystic lesion in the fetal cranial cavity. Caesarian section was carried out at 36 weeks because of the progressive enlargement of the fetal head. The cyst was multiloculated and a cyst peritoneal shunt placement resulted in collapse of the drained cyst components followed by enlargement of others

Cervical Foraminal Epidural Blood Patch for the Targeted Treatment of Refractory Cerebrospinal Fluid Leakage From a Dural Sleeve.

Epidural blood patches (EBPs) are routinely used to treat symptoms (eg, headaches) associated with spontaneous intracranial hypotension. Although cerebrospinal fluid leakage commonly involves the periforaminal areas of the cervical or thoracic spine, EBPs have been historically performed at the lumbar level. Recent evidence suggests that targeting the causative spinal segment may provide greater clinical benefits. While previous reports have targeted foraminal leaks with segmental thoracic or cervical injections, we present a case report detailing the novel use of a navigable epidural catheter

Neuroendoscopically assisted cyst-cisternal shunting for a quadrigeminal arachnoid cyst causing typical trigeminal neuralgia.

A quadrigeminal cistern arachnoid cyst is a very rare cause of typical trigeminal neuralgia. A 62-year-old woman presented with right facial pain of 8 years duration. Neuroradiological findings revealed a cystic mass in the quadrigeminal region that compressed the cerebellum downward and the brainstem anteriorly and was associated with hydrocephalus. She had neuroendoscopically-assisted cyst-cisternal shunting via a small craniotomy. Postoperatively, the trigeminal neuralgia disappeared. The origin of the trigeminal neuralgia may have either been a marked distortion of the pons that caused str

Transclival cerebrospinal fluid fistula in a patient with Marfan's syndrome.

Marfan's syndrome is a disease associated with reduced structural integrity of connective tissues. We report a 36-year-old patient with Marfan's syndrome who presented with rhinorrhoea, occipital headache and vomiting. Physical examination revealed typical Marfan's syndrome features including dolicocephalous, mandibular micrognathia, tall stature, disproportionately long limbs and digits, and hypermobility of the joints. A high-resolution CT scan demonstrated pneumoencephalous, cerebrospinal fluid (CSF) filling the sphenoidal sinus, and a small bone defect of the clivus. Surgery performed thro

Thoracic amebiasis.

Pleuropulmonary amebiasis is the common and pericardial amebiasis the rare form of thoracic amebiasis. Low socioeconomic conditions, malnutrition, chronic alcoholism, and ASD with left to right shunt are contributing factors to the development of pulmonary amebiasis. Although no age is exempt, it commonly occurs in patients aged 20 to 40 years, with an adult male to female ratio of 10:1. Children rarely develop thoracic amebiasis: when it does occur there is an equal sex distribution. The infection usually spreads to the lungs by extension of an amebic liver abscess. Infection may pass to the
