[Treatment of clavicle fractures in children and adolescents : Conservative and surgical treatment options with a focus on the figure-of-eight style brace and intrafocal intramedullary nail osteosynthesis].

OBJECTIVE: Conservative treatment using a backpack bandage (RSV) for clavicle fractures in children and adolescents serves to restore anatomy and function. The technique used in adult patients with elastic stable intramedullary nailing (ESIN) from the medial end of the clavicle involves the risk of growth disturbance of the growth plate, which has been open for a very long time; in addition, a cosmetically disturbing scar usually forms there. Treatment with an intrafocal intramedullary nailless osteosynthesis allows length and axis to be restored within the age-specific correction limits using

Atypical Femoral Fracture in a Patient on Imatinib: A Case Report.

CASE: A 74-year-old woman with chronic myeloid leukemia on long-term imatinib therapy and a history of bisphosphonate use presented with an atypical femoral fracture after low-energy trauma. She underwent staged intramedullary nailing of both femurs. CONCLUSION: This case highlights the potential synergistic effect of bisphosphonates and imatinib on bone metabolism, leading to an increased risk of atypical femoral fractures. Vigilant monitoring of bone health in such patients is crucial for early detection and prevention.

Evaluation of Risk Factors for Revision Surgery After Proximal Femoral Nailing for Intertrochanteric Fractures.

Background and Objectives: Intertrochanteric femur fractures are very common, especially in the elderly population, and cause serious morbidity and mortality. Today, the most commonly used implants in the treatment of these fractures are proximal femoral nails (PFNs). This study aimed to analyze the clinical and radiological results of patients who underwent surgical treatment with a proximal femoral nail (PFN) for intertrochanteric femur fractures and later required revision surgery for various reasons. Materials and Methods: Patients who underwent surgical treatment PFN due to intertrochante

Integrated Dual Lag Screws Have Higher Reoperation Rates for Fixation Failure Than Single Lag Component Cephalomedullary Nails: A Retrospective Study of 2,130 Patients with Intertrochanteric Femoral Fractures.

BACKGROUND: Previous studies comparing reoperation risk between integrated dual lag screw (IDL) and single lag component (SL) cephalomedullary nails (CMNs) in the treatment of intertrochanteric femoral fractures have demonstrated mixed results. The purpose of this study was to assess the rates of reoperation for fixation failure and all-cause reoperation in a large, multi-institutional cohort of patients with an intertrochanteric fracture treated with an IDL or SL CMN. We hypothesized that there would be no difference between the groups with respect to either of the reoperation rates. METHODS

Lag Screw Exchange for Impinging Lateral Hardware Following Intramedullary Nailing of Intertrochanteric Hip Fractures - A Case Series Demonstrating Efficacy.

BACKGROUND: This study aimed to demonstrate the feasibility of lag screw exchange for painful lateral soft tissue impingement in patients initially treated with cephalomedullary nailing (CMN) for an intertrochanteric hip fracture. METHODS: Ten patients initially treated with CMN for unstable intertrochanteric fractures presenting with persistent pain and radiographic evidence of lag screw lateral migration were treated with exchange of original screw with shorter lag screw buried in the lateral cortex to prevent impingement. Patients were evaluated for resolution of pain and achievement of pr

[Intramedullary nailing of coated and uncoated nails in infected tibial pseudarthrosis : Results of a retrospective examination of 56 patients].

BACKGROUND: In surgery for sepsis it is a well-established principle that no internal osteosynthetic material should be implanted in cases of chronic osteomyelitis. Therefore, the surgical treatment with intramedullary nails is so far used only rarely in cases of chronic osteomyelitis. OBJECTIVE: This study analyzed whether the implantation of tibial intramedullary nails is an effective treatment for chronic osteomyelitis and how high is the rate of reinfection. MATERIAL AND METHODS: A retrospective analysis of patients with an infected pseudarthrosis of the tibia in whom a gentamycin-coated

The clinical efficacy of Shengyu decoction in the treatment of anemia after PFNA for elderly intertrochanteric fracture: A protocol of prospective, randomized, controlled trial.

BACKGROUND: Femoral intertrochanteric fractures (ITF) occur frequently in the elderly, accounting for 45% of all hip fractures. Postoperative anemia after fracture tends to cause ischemia, hypoxia in cells, tissues and organs, increasing the rate of blood transfusion, risk of infection, disability and mortality. Meanwhile, traditional Chinese medicine is widely used in the treatment of anemia for activating blood circulation and removing blood stasis. METHODS: This study is a prospective, outcome assessor-, and data analyst-blinded randomized controlled clinical trial. The objective of this p

Fast track care for pertrochanteric hip fractures: How does it impact length of stay and complications?

INTRODUCTION: The medical and economic impact of treating pertrochanteric hip fractures is growing. We hypothesized that fast track surgery of pertrochanteric fractures would reduce the length of stay (LOS) without compromising the quality of care. MATERIALS AND METHODS: This was a prospective, observational, single-center cohort study conducted between 2014 and 2016 at the Angers Teaching Hospital in France. The enrolled patients had an isolated A1 or A2 proximal femur fracture (AO classification) that was treated surgically by intramedullary nailing and required post-acute rehabilitation (P

Iatrogenic Injuries in Percutaneous Pinning Techniques for Fifth Metacarpal Neck Fractures.

BACKGROUND: There is little information regarding anatomic structures at risk during closed percutaneous treatment of fifth metacarpal neck fractures. This study evaluates a variety of common percutaneous techniques with the hypothesis that each approach presents unique risks to tendons and neurovascular structures. METHODS: Seven cadaveric hands were used for this study. The senior author, a board-certified hand surgeon with more than 20 years of experience, used a mini-C arm to pass 1.6-mm Kirschner wires (K-wires). The 4 percutaneous techniques employed were anterograde, retrograde, cross-

Provisional pin fixation: An efficient alternative to manual maintenance of reduction in nailing of intertrochanteric fractures.

INTRODUCTION: During nailing of intertrochanteric fractures, there is always a risk of reduction loss despite achieving an acceptable reduction status after a percutaneous procedure. Most surgeons usually attempted to maintain the reduction with a manual endeavor. However, we experienced varying amounts of intraoperative reduction loss and had to perform readjustments several times. These struggles motivated us to attempt provisional pin fixation as an alternative method. The purpose of this study was to analyze the factors affecting intraoperative reduction loss, especially in comparison betw

Correction of multiple complex lower limb deformities by intramedullary nailing.

A female patient presented to our institution suffering from long bone post-traumatic multiple deformities. Her walking was limited, painful and teetering. It had worsened over the past months. The right leg was shortened by 6 cm compared to the contralateral. The right femur was 18° varus, the right tibia was 16° valgus, the left tibia was 12° varus. Both knee joints showed marked radiographic signs of osteosclerosis. After a thorough study with simulations using paper models, we carried out three osteotomies, two open wedge and one cuneiform, stabilizing them with intramedullary nails. We co

Biomechanical evaluation of interlocking lag screw design in intramedullary nailing of unstable pertrochanteric fractures.

OBJECTIVES: Intramedullary nails with special lag screw designs may provide improved mechanical performance and alleviate clinical problems. We hypothesize that the proximal design of trochanteric nails affects mechanical performance. METHODS: Ten pairs of human cadaveric femora were implanted with 2 different short intramedullary nails without (Gamma3) and with an interlocking lag screw (Intertan). An unstable, multifragmentary, pertrochanteric fracture was created. Bones were tested in a cyclic testing protocol with increasing loads until failure simulating 1 leg stance. Stiffness, failure

Computer aided long bone fracture treatment.

Intraoperative fluoroscopy is the tool for intraoperative control of long bone fracture reduction and osteosynthesis. Limitations of this technology include: High radiation exposure to the patient and the surgical team, limited field of view, image distortion, limitation to 2-D representations, and cumbersome updating of verification images. Fluoroscopy based navigation systems partially address these limitations by allowing fluoroscopic images to be used for real-time surgical localization and instrument tracking. In a clinical study on computer guidance by virtual fluoroscopy for distal lock

The clinical performance of a small diameter tibial nailing system with a mechanical distal aiming device.

We present the clinical and radiographic results of a prospective study with the Orthofix tibial nailing system. The ease and safety of distal locking with the use of an improved targeting system was also evaluated. Sixty fresh tibial fractures in 60 patients with a mean age of 37.3 years (range 17-73 years) were treated. Eighteen of the fractures were grade I open fractures. All operations were performed in a conventional operating theatre on a simple transparent operating table, with reduction of the fracture performed under manual traction and manipulation of the fracture site. Hand reaming

Elastic stable intramedullary nailing (ESIN) provides good long-term results in pediatric long-bone fractures.

The present study evaluates the long-term outcome of elastic stable intramedullary nailing (ESIN) in children with long-bone shaft fractures. 70 patients were examined, the present mean age of patients was 12.7 (+/- 3.26) years, mean time interval since surgery was 3.1 (+/- 1.49) years. Following ESIN at the lower extremity (23 femur fractures, 14 tibia fractures), keloid formation was remarkable in one patient, range of motion (ROM) was adequate in all but one boy (extension deficit of the hip of 10 degrees). Overall leg length discrepancy was +4.1 mm (+/- 5.2 mm) following ESIN for femur fra

[Osteosynthesis using the "palm-tree" nail technic in fractures of the surgical neck of the humerus].

Presented in 1974, this technique consists of internal fixation of the humeral neck by means of three divergent K-wires. These are introduced through a proximal entry point located on the lateral aspect of the humeral diaphysis at the lower end of the "V" shaped insertion of the deltoid which is one of the three subcutaneous areas of this bone. The two principles of this technique are: anchorage of the epiphysis by three divergent pins driven into the cancellous bone of the head and introduction of the pins from a very proximal point that allows easy access across the fracture without the disa
