Surgical management of spontaneous cerebrospinal fluid leakage through temporal bone defects--case series and review of the literature.

Increasing numbers of cases of idiopathic cerebrospinal fluid (CSF) leakage through temporal bone defects (TBD) have been recently reported, mainly in otolaryngologic journals. Those cases are referred to as spontaneous temporal bone encephaloceles (TBE). Three surgical approaches have been advocated for this condition: the transmastoid approach (TMA), the middle cranial fossa approach (MCFA), or a combination of both. We conducted a retrospective study of all 11 consecutive patients who underwent 12 middle cranial fossa craniotomies for the treatment of CSF leakage through TBD in our institut…

Iatrogenic cholesteatoma arising at the bony-cartilaginous junction of the external auditory canal: a late sequela of intact canal wall mastoidectomy.

OBJECTIVE: The objective of this study is to describe the presentation and management of a rare site of cholesteatoma recurrence at the bony-cartilaginous junction after intact canal wall (ICW) mastoidectomy. STUDY DESIGN: Retrospective case series SETTING: Tertiary referral center PATIENTS: Patients with cholesteatoma formation arising from the bony-cartilaginous (BC) junction of the external auditory canal (EAC) requiring surgical intervention were retrospectively identified across a 5-year period. INTERVENTION(S): All patients were treated surgically to eradicate the disease and reconst…

Ossicular chain reconstruction improves bone conduction threshold in chronic otitis media.

This study aimed to assess the elevation of bone conduction threshold in patients with chronic otitis media and to investigate the mechanism of this phenomenon. One hundred and six patients with unilateral chronic otitis media who had undergone a tympanomastoidectomy were reviewed retrospectively. The differences in the bone conduction thresholds between the diseased and normal sides were assessed and compared according to the duration of the disease and the presence of cholesteatoma. Post-operative changes in the bone conduction threshold were also assessed. The mean bone conduction threshold…

Surgical treatment of the high jugular bulb by compressing sinus sigmoideus: two cases.

If the jugular bulb normally surrounded by a bony layer in jugular fossa is anatomically over the inferior surface of the bony annulus, in the middle ear or over the basal turn of cochlea, it is then named as high jugular bulb (HJB). It may be dehiscent or aberrant. It is reported to occur in 5% of the temporal bone specimens. In accordance with the literature jugular bulb compression, jugular vein ligation and embolization are suggested in such cases. In both of the presented cases, there was bleeding from jugular bulb during surgery and jugular bulb was compressed with bone wax and Surgicel,

Multiple paragangliomas of the head and neck.

Multiple paragangliomas of the head and neck are rare conditions. The incidence of multiple paragangliomas is reported to the approximately 10% of the total patients, but in familial cases it increases up to 35-50%. In the head and neck region, the most common association is represented by bilateral carotid body tumors or by carotid body tumor associated with tympanic-jugular glomus. The presence of three synchronous glomus tumors is really rare, as well as association with vagal glomus and carotid body. In this paper the authors present a patient affected ipsilaterally by a carotid body tumor

Otogenic intracranial abscess: concurrent craniotomy and mastoidectomy--changing trends in a developing country.

OBJECTIVES: To evaluate (1) the clinical profile, treatment, and outcome of adult and pediatric patients presenting with intracranial abscess of otogenic origin and (2) the advantages of concurrent craniotomy and mastoidectomy. DESIGN: A prospective case series. SETTING: An academic tertiary referral center in India. PATIENTS: Thirty-six patients clinically diagnosed as having intracranial abscess that was secondary to suppurative otitis media and confirmed by computed tomographic scanning. INTERVENTION: Concurrent craniotomy and mastoidectomy. RESULTS: Children were more commonly affecte…

Otologic sequelae after pneumococcal meningitis: a survey of 164 consecutive cases with a follow-up of 94 survivors.

As part of a comprehensive study on sequelae after pneumococcal meningitis, 94 of 111 consecutive survivors were re-examined 4 to 16 years after discharge. Twenty-three patients had otological sequelae after pneumococcal meningitis. In these patients, 17 had hearing losses, 7 had tinnitus, 9 had vertigo, 13 had vestibular areflexia, 4 had loss of smell, and 3 had loss of taste. Among the patients with hearing loss, 4 were bilaterally deaf, 6 were unilaterally deaf, and 2 had mild and 5 had slight hearing losses. From correlations with extensive data from the medical records, preadmission antib

Herein, we report the fabrication of a surface-fluorinated indium oxide (In2O3) nanomastoid sensitive material via SF6 reactive ion etching. This engineered material mimics the multiscale hierarchical architecture of lotus leaves, integrating micron-sized mastoid protrusions with nanoscale fluorinated domains. Non-invasive vagus nerve magnetic stimulation combined with rehabilitation training for vocal cord paralysis and dysphagia following brain injury: a prospective case series study.

BACKGROUND: Vocal cord paralysis combined with dysphagia following brain injury is a severe complication associated with aspiration pneumoni … ocal cord paralysis and dysphagia. All patients received a 21-day combined protocol comprising left mastoid nVNMS (5 Hz, 20 min/session, once daily), low-frequency pulse electrical stimulation for swallowing (20 min/session, once daily), and manual swallowing training (30 min/session, once daily). Development of the Surgical Implantation and Fixation of an Implanted Middle Ear Microphone, the "UmboMic," in Cadaveric Sheep.

PURPOSE: The UmboMic is a recently developed middle ear microphone that functions by detecting the sound-induced motion of the umbo [1]. To … acted the umbo in the middle-ear cavity and the non-sensing tail end was secured to the surrounding mastoid bone. First, pre-surgical micro-CT scans of the temporal bones and open-source software were used to simulate surgical drilling, and to virtually plan and fit the UmboMic fixation system. Then, in physical sheep temporal bones, sound-induced umbo mo …
