Management of sternal fractures: 239 cases.

A review of the management of 239 patients with sternal fractures in a busy trauma center between October 1989 and May 2000 was undertaken to determine the incidence, significance, morbidity, and mortality of this injury. There were 140 men and 99 women with a mean age of 50.3 years (range, 15 to 93 years). Sternal fractures accounted for 8% of admissions for thoracic trauma. The causes were motor vehicle collisions in 215 patients (90%) and falls or direct blows in 24 (10%). Only 64 of 204 car accident patients (31%), 28 men and 36 women, were restrained by seat belts. Complications developed

A minimal transverse incision with low median sternotomy for pediatric congenital heart surgery.

OBJECTIVES: Median sternotomy is the incision of choice for most cardiac surgical procedures, but the full-length vertical skin incision generally leaves an unsightly scar. In certain patients undergoing short, low-risk procedures, cosmetic considerations are of relatively greater importance. METHODS: A minimal transverse curvilinear skin incision with low median sternotomy is described which gives adequate exposure for selected open-heart procedures. Since September 1997, this approach has been used in 22 pediatric patients undergoing open-heart surgery including five cases of Fallot's tetra

Risks of repeat sternotomy in pediatric cardiac operations.

BACKGROUND: Repeat sternotomy has been associated with increased perioperative risks. METHODS: We reviewed 165 patients undergoing 192 repeat sternotomies between January 1985 and January 1997 (group 1) and a control group of 184 patients (group 2). The operations in group 1 were valve procedures in 94 patients, Fontan procedure in 46, ventricular septal defect closure in 10, pulmonary arterioplasty in 17, and others in 25; in group 2 ventricular or atrial septal defect closure in 120 patients, tetralogy of Fallot repair in 26, valve procedures in 16, bidirectional Glenn anastomosis in 7, rep

Prospective study of anterior chest wall involvement in ankylosing spondylitis and psoriatic arthritis.

A prospective study of anterior chest wall involvement was conducted in 50 ankylosing spondylitis patients and 50 psoriatic arthritis patients in the absence of palmoplantar pustulosis. All patients underwent a physical examination, tomograms, and a radionuclide bone scan. Magnetic resonance imaging with gadolinium was done in some cases. Half the patients in both groups had anterior chest wall involvement. Enthesitis was the mechanism of the lesions. The manubriosternal symphysis and sternocostoclavicular joints were the most common sites of involvement, although other entheses in the region

Sternotomy approach for the modified Blalock-Taussig shunt.

BACKGROUND: Since 1990, sternotomy has been the preferred approach for construction of a modified Blalock-Taussig shunt (MBTS) at Children's Hospital, Boston, Mass. In retrospect, we sought to test the hypothesis that this approach yields less mortality and morbidity than the traditional thoracotomy approach. METHODS AND RESULTS: One hundred four primary MBTSs with polytetrafluoroethylene grafts were constructed in patients from January 1988 through December 1992. Fifty-two shunts were constructed by thoracotomy approach and 52 by sternotomy approach. Fifteen of the thoracotomy patients were

Characterization of the developmental toxicity of di-n-butyl phthalate in rats.

The objective of this study was to determine the characterization of the developmental toxicity of di-n-butyl phthalate (DBP) in rats. Pregnant rats were given DBP by gastric intubation at a dose of 0.75, 1.0 or 1.5 g/kg on days 7-9, 10-12 or 13-15 of pregnancy. Postimplantation loss was 100% for each period of dosing at 1.5 g/kg. A significant increase in the postimplantation loss was found in dams given DBP at doses of 0.75 and 1.0 g/kg regardless of the days of treatment. No evidence of teratogenicity was detected when DBP was given on days 10-12. Treatment on days 7-9 with DBP at doses of

Penetration of ofloxacin into heart valves, myocardium, mediastinal fat, and sternal bone marrow in humans.

Ofloxacin penetration into heart tissue (valve and myocardium), mediastinal fat, and sternal bone marrow was the object of a prospective nonrandomized study. Thirty-six patients undergoing mitral and/or aortic valve replacement were included. Patients were divided into two groups of 18 patients each. Group 1 patients were administered a single 400-mg intravenous dose of ofloxacin over a 30-min period upon anesthesia (n = 6) or at 1 h (n = 6) or 6 h (n = 6) prior to surgery. Group 2 patients received a 200-mg oral dose of ofloxacin every 12 h during the 48 h preceding surgery. In this group, th

The lateral chest radiograph in the assessment of nonpulmonary health and disease.

The lateral chest radiograph contains a wealth of data on the status of the thoracic cage, pleura, heart, pericardium, bronchi, and upper abdomen. The appearances of slightly oblique projections deviating from the "true" lateral projection are reviewed. Careful attention to these common variations of projection is advocated.

Buckling and bone modeling as factors in the development of idiopathic scoliosis.

STUDY DESIGN: Computational analysis using the finite-element method was used to examine a possible etiology of idiopathic scoliosis. OBJEC … thoracic cage deformation was obtained in the fourth buckling mode. In patients with scoliosis, the sternum essentially remains in its original position with respect to the vertebrae, but in the linear buckling model, it shifted in the direction of vertebral body rotation. In contrast to clinical data, the incremental deformation resulting from bone forma …

[A 36-year-old woman with acute onset left hemiplegia and anosognosia].

We report a 36-year-old woman with right hemiplegia, anosognosia, and rapidly deteriorating course. She was well until the end of January, 1995 when she had an onset of fever, sputum, and cough. A 5 x 5 tumor was found in her left lower lobe. She was admitted to the Pulmonary Medicine on May 24, 1995 when she was 36-year-old. General physical examination was unremarkable. Bone scintigraphy revealed increased uptake in the skull, sternum, right scapula, vertebrae, right femur, and in ribs. Cranial CT scan revealed a large mass lesion in the right frontal subcortical region with central low dens…

[Anatomical study of the ramification and intramuscular distribution of the intercostal nerves in man].

Minute dissection was performed on the ventral primary rami of the thoracic and the first lumbar nerves in the left half of an adult male cadaver in order to obtain more detailed data of the nerve supply to the muscles of the thorax and abdomen. The ribs, costal cartilages, sternum and vertebral column were removed to facilitate dissection. The origin, course and intramuscular distribution of the muscular branches of each nerve were examined in precise detail. The main findings are as follows: 1. The intercostal nerves The collateral branches were found in approximately 1/3 of the segments exa…

[Usability of Ultrasonography in the Diagnosis of Pediatric Fractures].

The study aims to evaluate the usability of ultrasonography in the diagnosis of pediatric fractures. Ultrasound examination of patients with fractures diagnosed using radiography at the Clinic of Pediatric and Trauma Surgery, Third Faculty of Medicine, Charles University and UTH between 27 May 2024 and 31 December 2024. We identified a total of 11 areas to be examined - clavicle, proximal humerus, elbow, forearm, metacarpal bones, fingers of the upper limb, sternum, femur, tibia, ankle, and metatarsal bones. The examination was performed independently by a surgeon and a radiologist, who select…
