The relationship between resting heart rate and incidence and progression of coronary artery calcification: the Multi-Ethnic Study of Atherosclerosis (MESA).

OBJECTIVE: Elevated resting heart rate has been independently associated with cardiovascular and all-cause mortality. The pathophysiological mechanisms by which this increased risk occurs are unclear. We hypothesized that elevated resting heart rate will be associated with increased development of atherosclerosis, as assessed by the incidence and progression of coronary artery calcium (CAC). METHODS: The Multi-Ethnic Study of Atherosclerosis is a prospective cohort study of participants free of clinical cardiovascular disease at entry. Among persons without CAC at baseline, the association be

PET, Proteomics, and Immunophenotyping of Arterial Inflammation and Checkpoint Inhibition.

IMPORTANCE: Immune checkpoint inhibitors (ICIs) have transformed cancer outcomes but may be associated with an increased risk of myocardial infarction and ischemic stroke. Whether this risk is mediated by inflammatory plaque activation and whether coadministered vascular endothelial growth factor inhibitors (VEGFIs) modify the effect remain unresolved. OBJECTIVE: To prospectively characterize the arterial and systemic inflammatory outcomes associated with ICIs, VEGFIs, and combined ICIs plus VEGFIs in patients with cancer. DESIGN, SETTING, AND PARTICIPANTS: This prospective longitudinal coho

Valvular and structural heart disease: American College of Chest Physicians Evidence-Based Clinical Practice Guidelines (8th Edition).

This chapter about antithrombotic therapy for valvular heart disease is part of the American College of Chest Physicians Evidence-Based Clinical Practice Guidelines (8th Edition). Grade 1 recommendations are strong and indicate that the benefits do, or do not, outweigh risks, burden, and costs. Grade 2 suggests that individual patient values might lead to different choices (for a full understanding of the grading see Guyatt et al, CHEST 2008; 133[suppl]:123S-131S). Among the key recommendations in this chapter are the following: for patients with rheumatic mitral valve disease complicated sing

[Pedicled aortic wall flap for the reconstruction of aortico-ventricular discontinuity--a case report].

A 55-year-old man was operated on urgently for aortic valve endocarditis complicated by an annular abscess at the base of the non-coronary leaflet extending down to the left ventricle. Rapidly progressive heart failure and presence of a friable-appearing vegetation on echocardiography were the indications for urgent operation. Preoperative electrocardiogram showed first degree A-V block. At operation the aortic valve was generally thickened with mild calcification about the commissures. Non-coronary cusp was severely deformed and was nearly detached at its base due to annular abscess formation

The Predictive Value of the Peripheral Arterial Calcium Scoring System on Computed Tomography Angiography in Patients With Chronic Limb-Threatening Ischemia Undergoing Below-the-Knee Endovascular Interventions.

INTRODUCTION: Endovascular below-the-knee (BTK) revascularization is safe and effective in patients with chronic limb-threatening ischemia (CLTI), but outcomes depend on the severity of limb ischemia, anatomical pattern, comorbidity, and degree of vessel calcification. The most commonly used peripheral calcification score, the peripheral arterial calcium scoring system (PACSS), is angiography based and is therefore limited by low sensitivity and interobserver agreement. This study aimed to determine the prognostic value of the original and a modified PACSS (mPACSS) based on computed tomography

Effect of Sex on Outcomes of Coronary Rotational Atherectomy Percutaneous Coronary Intervention (From the European Multicenter Euro4C Registry).

Data regarding the potential influence of gender on outcomes of rotational atherectomy (RA) percutaneous coronary intervention (PCI) are scarce and conflicting. Using the Euro4C registry, an international prospective multicentric registry of RA PCI, we evaluated the influence of gender on clinical outcomes of RA PCI. Between October 2016 and July 2018, 966 patients were included. In them, 267 (27.6%) were females. Female patients were older than males (77.7 years old ± 9.8 vs 73.3 ± 9.5 years old respectively, p < 0.001) had a poorer renal function (43,1% of females had a GFR < 60 ml/min:1.73m

Subclinical Vascular Disease and Subsequent Erectile Dysfunction: The Multiethnic Study of Atherosclerosis (MESA).

BACKGROUND: The association between subclinical cardiovascular disease and subsequent development of erectile dysfunction (ED) remains poorly described. HYPOTHESIS: Among multiple subclinical atherosclerosis and vascular dysfunction measurements, coronary artery calcium (CAC) score best predicts ED. METHODS: After excluding participants taking ED medications at baseline, we studied 1862 men age 45 to 84 years free of known cardiovascular disease from the Multi-Ethnic Study of Atherosclerosis (MESA) with comprehensive baseline subclinical vascular disease phenotyping and ED status assessed at

Associations Between Imaging-Detected Pulp Calcification and Systemic Diseases: A Systematic Review, Thematic Analysis, and Meta-Analysis.

BACKGROUND: Pulp calcification (PC) is a common imaging finding traditionally attributed to local factors or ageing. However, increasing evidence suggests that it may be associated with broader systemic biological disturbances. Clarifying this association may help determine whether dental pulp findings contribute to the interpretation of systemic health. OBJECTIVES: To evaluate whether imaging-detected PC is associated with systemic conditions by synthesising evidence across diverse disorders and identifying recurring biological and methodological patterns. METHODS: Comprehensive searches we

Calciphylaxis and nonhealing wounds: the role of the vascular surgeon in a multidisciplinary treatment.

OBJECTIVE: Calciphylaxis, a disorder of calcium-phosphate metabolism that can result in arterial calcification, skin and solid organ calcium deposits, and nonhealing ulcerations, is associated with significant morbidity and mortality. Although its most common cause is secondary hyperparathyroidism in patients with renal failure, vascular surgeons are frequently called on to evaluate these nonhealing extremity wounds. We reviewed our experience of a multidisciplinary approach in treating patients with calciphylaxis and nonhealing ulcers. PATIENTS AND METHODS: Over a 14-month period at a tertia

IGF1-mediated mesenchymal-endothelial transition as a potential regulatory target in calcific aortic valve disease.

BACKGROUND: Aortic valve interstitial cells (VICs) are considered a highly plastic heterogeneous mesenchymal cell population. Although previous studies have demonstrated their potential to differentiate into myofibroblasts, osteoblasts, chondrocytes, and adipocytes, their ability to differentiate into endothelial cells has not been confirmed. Notably, normal aortic valve tissue is avascular, but in the diseased valves of patients with calcific aortic valve disease (CAVD), we often find cells with an endothelial phenotype, but the origin of these cells is unclear. METHODS: We performed paraffi

Elevated Perioperative Morbidity After Parathyroidectomy for Calciphylaxis Patients: A Nationwide Retrospective Observational Study.

INTRODUCTION: Calcific uremic arteriolopathy (CUA), also known as calciphylaxis, is a rare and potentially lethal condition associated with longstanding renal failure. It is defined by calcium and parathyroid hormone dysregulation, which leads to vascular calcification, painful skin necrosis, and high rates of sepsis-driven mortality. Although the mainstay treatment is medical therapy and supportive care, severe or recalcitrant cases will require parathyroidectomy, a modality associated with high perioperative complication rates of up to 40%. The aim of the study was to evaluate the associatio…

Percutaneous approach to aortic disease: EVAR, TEVAR and newer techniques for more proximal disease.

Advances in device design, imaging and vascular-closure technology have driven a progressive shift from surgical femoral exposure to totally percutaneous strategies for endovascular aortic repair. This narrative review summarises contemporary evidence and practical considerations for percutaneous access across the spectrum of aortic disease, from infrarenal endovascular aneurysm repair (EVAR) to thoracic endovascular aortic repair (TEVAR), fenestrated/branched repair and endovascular treatment of the aortic arch. Percutaneous femoral access is now supported by randomized evidence, large regist

Positive Correlates of Sclerostin and Association with Peripheral Arterial Stiffness in Patients with Type 2 Diabetes Mellitus.

Background and Objectives: Sclerostin or dickkopf-1 (DKK1) inhibits the canonical Wnt/β-catenin signaling pathway, which regulates vascular calcification and may contribute to the development of arterial stiffness. The brachial-ankle pulse wave velocity (baPWV) measures peripheral arterial stiffness (PAS). This study aimed to investigate the correlation between sclerostin and DKK1 levels and PAS in patients with type 2 diabetes mellitus (T2DM). Materials and Methods: Biochemical data and sclerostin and DKK1 levels were analyzed in the fasting blood samples of 125 patients with T2DM. baPWV meas…

SureQuant™ IS-PRM Enables Cross-Species Targeted Quantification of Retinoid Metabolism and Signaling Proteins in the Heart.

Retinoic acid signaling is critical for cardiac development and homeostasis. Dysregulation of all-trans retinoic acid metabolism contributes to vascular atherogenesis, restenosis, calcification, and heart failure. Therefore, assessment of proteins involved in retinoid metabolism and signaling has gained interest for identifying potential biomarkers and therapeutic targets in cardiovascular disease. However, quantifying these proteins remains challenging due to limitations of antibody-based methods. We developed a targeted proteomics approach using SureQuant™ internal standard-triggered paralle

Matrix Gla Protein is a novel regulator of TGFβ-dependent fibroblast activation and kidney fibrosis.

Kidney fibrosis is the pathologic hallmark of chronic kidney disease (CKD) and is driven by fibroblast-to-myofibroblast transformation (FMT) … . Matrix Gla Protein (MGP) is a vitamin K-dependent extracellular protein best known for inhibiting vascular calcification; however, its role in kidney fibrosis remains unknown. Here, we demonstrate that MGP expression is markedly upregulated in fibroblasts from fibrotic kidneys in mouse models of chronic kidney injury and in human CKD samples. Using inducible global an …
